Book excerpt: La conciliation médicamenteuse est un processus rigoureux dont le but est de sécuriser la prise en charge du patient et de réduire le risque d'iatrogénie lié aux erreurs médicamenteuses lors d'une hospitalisation. La conciliation prévient et intercepte les erreurs médicamenteuses grâce à la communication et la transmission d'informations entre les différents professionnels de santé de ville et hospitaliers. Elle permet de garantir la continuité des traitements en prenant en compte les traitements antérieurs des patients lors de la rédaction de la prescription initiale hospitalière et de la prescription de sortie. Nous avons étudié la faisabilité et l'intérêt d'une conciliation médicamenteuse d'entrée et de sortie dans un service de gériatrie. L'objectif principal était de déterminer la proportion de prescriptions présentant des divergences à l'entrée et à la sortie du service de gériatrie. Nous avons démontré que la conciliation réalisée par un externe en pharmacie permettait de détecter des discordances entre le traitement hospitalier et le traitement habituel du patient ou prescrit en sortie d'hospitalisation. Cependant, il a été matériellement impossible de réaliser une conciliation pour tous les patients admis dans le service de gériatrie. D'autres études sont donc nécessaires afin de définir le profil des patients prioritaires pour lesquels une conciliation médicamenteuse doit être réalisée.
