Book excerpt: As of December 2012, Operation Enduring Freedom (OEF) in Afghanistan and Operation Iraqi Freedom (OIF) in Iraq have resulted in the deployment of about 2.2 million troops; there have been 2,222 US fatalities in OEF and Operation New Dawn (OND)1 and 4,422 in OIF. The numbers of wounded US troops exceed 16,000 in Afghanistan and 32,000 in Iraq. In addition to deaths and morbidity, the operations have unforeseen consequences that are yet to be fully understood. In contrast with previous conflicts, the all-volunteer military has experienced numerous deployments of individual service members; has seen increased deployments of women, parents of young children, and reserve and National Guard troops; and in some cases has been subject to longer deployments and shorter times at home between deployments. Numerous reports in the popular press have made the public aware of issues that have pointed to the difficulty of military personnel in readjusting after returning from Iraq and Afghanistan. Many of those who have served in OEF and OIF readjust with few difficulties, but others have problems in readjusting to home, reconnecting with family members, finding employment, and returning to school. In response to the return of large numbers of veterans from Iraq and Afghanistan with physical-health and mental-health problems and to the growing readjustment needs of active duty service members, veterans, and their family members, Congress included Section 1661 of the National Defense Authorization Act for fiscal year 2008. That section required the secretary of defense, in consultation with the secretary of veterans affairs, to enter into an agreement with the National Academies for a study of the physical-health, mental-health, and other readjustment needs of members and former members of the armed forces who were deployed in OIF or OEF, their families, and their communities as a result of such deployment. The study consisted of two phases. The Phase 1 task was to conduct a preliminary assessment. The Phase 2 task was to provide a comprehensive assessment of the physical, psychologic, social, and economic effects of deployment on and identification of gaps in care for members and former members, their families, and their communities. The Phase 1 report was completed in March 2010 and delivered to the Department of Defense (DOD), the Department of Veterans Affairs (VA), and the relevant committees of the House of Representatives and the Senate. The secretaries of DOD and VA responded to the Phase 1 report in September 2010. Returning Home from Iraq and Afghanistan: Assessment of Readjustment Needs of Veterans, Service Members, and Their Families fulfills the requirement for Phase 2.

Book excerpt: Despite DOD and VA efforts to address the concerns of these and other Persian Gulf veterans, those we surveyed still expressed concerns about their health and dissatisfaction with services from DOD and VA. Most respondents were still in the reserves and almost all reported that they had health problems they believed were caused by their service in the Persian Gulf. Most also reported that these problems limited their physical and social activities to some extent. Because all of the individuals we surveyed had already expressed some concern about their health, the fact that they reported having health problems after returning from the Persian Gulf is not surprising. Over half of the respondents had taken advantage of medical services, either the special examinations or other health care, available to them through VA or DOD. Many, however, were dissatisfied with the medical care they received or were unaware of services available. DOD and VA have undertaken a variety of efforts to address the concerns raised by Persian Gulf veterans, including expanding the health examinations available to them. Also, in response to recent legislation, both agencies are expanding outreach efforts to better ensure that veterans are aware of services available. Whether these efforts will satisfy these veterans concerns remains to be seen.

Objective: To compile a master bibliography of the Persian Gulf War and related topics for researchers. Approach: This document is the second revision of Topical Bibliography of Published Works Regarding the Health of Veterans of the Persian Gulf War, published in late 1995 and included in the Defense Technical Information Center's Gulflink World Wide Web site. The original version had 1,779 references and was framed around the National Library of Medicine's Current Bibliographies in Medicine 94-3, a bibliography of the Persian Gulf War and associated topics, prepared by Jacqueline Van de Kamp, M.L. S, Specialized Information Services, National Library of Medicine; and John H. Ferguson, M.D., Office of Medical Applications of Research, National Institutes of Health. The second revision, prepared by Colleen MeDonough, had 2,997 references and was published in November 1997. We have added considerably to the Van de Kamp-Ferguson work, and continue to monitor for related published works. Recently, we have received assistance in compiling our newspaper and magazine sections from Edwina Underwood of the Interagency Support Office of die Persian Gulf Veterans Coordinating Board, from Ulrich Brandenberger (Environmental Protection Unit, Swiss Re Zurich), and from OPPT Newsbreak. Results: This document currently lists 4,462 references, divided into 29 categories. A primary concern is whether Gulf War veterans are receiving effective treatments for their health problems. Section 105 of the Veterans Program Enhancement Act of 1998 mandates that the Department of Veterans Affairs (VA) ask the Institute of Medicine (IOM) to convene a committee that would identify a method for assessing treatment effectiveness and describe already-validated treatments for Gulf War veterans' health problems, including the problem of medically unexplained symptoms. The specific charge to the committee is to (1) identify and describe approaches for assessing treatment effectiveness; (2) identify illnesses and conditions among veterans of the Gulf War, using data obtained from the VA and the Department of Defense (DoD) Gulf War Registries, as well as information in published articles; and (3) for these identified conditions and illnesses, identify validated models of treatment (to the extent that such treatments exist), or identify new approaches, theories, or research on the management of patients with these conditions if validated treatment models are not available.

The Persian Gulf War, for instance, had an immense impact on veterans of all wars. The author was a national faculty member for joint VA-DOD training programs to enhance mental health response readiness for receiving anticipated medical and psychiatric casualties from the Persian Gulf War. What he found was a resurgence of selective amnesia and denial about the true impact of war. Scurfield notes, "Chillingly, what happened in Vietnam in 1968--69 regarding psychiatric casualties has enormous parallels to what is happening today regarding U.S. psychiatric casualties from the Iraq War."
