Book excerpt: Le patient douloureux chronique apparaît souvent comme un patient complexe pour les médecins généralistes : souffrant, il semble enfermé dans le cercle vicieux de la douleur-maladie. Le médecin généraliste l'adresse souvent en structure douleur en dernier recours, quand il ne sait plus comment faire, dans l'espoir d'un changement. Ce travail a cherché à interroger le processus du changement chez des patients douloureux chroniques au décours d'une consultation de la douleur, en faisant émerger des hypothèses sur ce qui peut l'induire, le composer, le consolider ou le freiner. Il a été réalisé au moyen d'une étude qualitative, s'inspirant de l'analyse phénoménologique interprétative. Le changement mis en évidence porte principalement sur la représentation et le vécu de la douleur par le patient, ainsi que sa posture par rapport à celle-ci. La validation de la souffrance apparaît comme un point de départ nécessaire au changement. La compréhension des mécanismes complexes et multifactoriels de la douleur chronique permet un changement de représentation du patient sur sa douleur. Quittant la recherche effrénée d'une cause, il peut redéfinir et orienter ses attentes initiales vers des objectifs concrets centrés sur ses ressources avec l'aide de l'équipe pluridisciplinaire. Devenir acteur dans la gestion de ses douleurs lui permet de retrouver du pouvoir et de l'autonomisation. Il coconstruit les solutions dans une relation de partenariat avec le médecin et l'équipe. Le patient a besoin de temps pour s'adapter, repenser sa vie, sa place dans la société et réinventer une vie avec la douleur qui ait du sens. C'est donc bien un chemin qu'est amené à emprunter le patient qui souffre de douleur chronique pour, pas à pas, changer ses représentations de la douleur, de son image, ses comportements et réinvestir sa vie en la réinventant. Ce chemin peut être initié et prendre appui sur l'accompagnement vers un changement proposé en consultation douleur, en partenariat avec le médecin traitant et l'équipe de soins primaire.

Book excerpt: Introduction : La prise en charge en réanimation peut être à l'origine de séquelles somatiques et psychiatriques regroupées au sein du syndrome post-réanimation. La douleur chronique fait partie intégrante de cette entité mais est encore sous-évaluée. Matériel et méthode : L'étude a été conduite dans 26 hôpitaux français entre Avril 2021 et Janvier 2022. Tout patient hospitalisé dans une unité de réanimation plus de 48h et capable d'évaluer sa douleur à la sortie était éligible à une inclusion. Le critère de jugement principal était une ENS≥ 3/10 à 3 mois de la sortie. Les critères secondaires comprenaient l'ENS≥ 3/10 à la sortie de réanimation, le type de douleur à la sortie et au suivi à 3 mois et l'utilisation d'antalgiques à 3mois. Résultats : 1078 patients ont été inclus. 259 ont été perdus de vue. A 3 mois de la sortie de réanimation, 388 patients (47%) présentaient une douleur chronique. Seulement 34 patients (8%) présentaient des caractéristiques neuropathiques. La traumatologie était la population la plus à risque avec 70% des patients déclarant une ENS ≥ 3/10 à 3 mois. Le sexe féminin, la prise d'anti-dépresseur et le decubitus ventral étaient associés à un risque augmenté de douleur chronique. Conclusion : La douleur chronique post réanimation est une complication fréquente. L'incidence est élevée quel que soit les motifs d'admission même médicaux suggérant que la prise en charge en réanimation est un facteur de risque en lui-même.

Ce mécanisme est néanmoins toujours pathologique, car la douleur envahit le patient et retentit dans son corps, son psychisme et dans tout son système relationnel. Dans certains cas, la douleur chronique pourrait venir symboliser un processus de deuil. Elle contribuerait à bloquer l'élaboration psychique nécessaire lors de tout travail de deuil, la représentation psychique de la perte entravée par la douleur incarnée dans le corps. Mais elle semblerait également protéger le sujet d'une décompensation mélancolique et porterait donc un potentiel de transformation pouvant relancer les processus d'élaboration psychique liés à la perte. La douleur chronique nécessite une prise en charge multidisciplinaire au sein de laquelle le psychiatre a une place essentielle.
