Book excerpt: Contexte : sur Bordeaux Métropole, le groupe « Femmes à la rue » devenu « Groupe Périnatalité », constitué des différents acteurs sociaux, médicaux et psychologiques, s'est créé en 2014. Au sein de ce réseau, et depuis maintenant bientôt 4 ans, est née une volonté de réaliser un temps de rencontre et de coordination pluridisciplinaire entre différents professionnels spécifiquement destiné aux femmes enceintes en situation de précarité/vulnérabilité. Ce temps de rencontre, matérialisé sous forme de réunion de concertation pluridisciplinaire (RCP) est organisé dans le but d'élaborer un parcours personnalisé de soins, centré et adapté aux besoins des femmes enceintes précaires/vulnérables. Ainsi est née la RCP « Périnatalité - Grande vulnérabilité » du réseau Périnat Aquitaine (RPA). Après quelques années de recul, il nous est apparu nécessaire de réaliser un « état des lieux « et d'évaluer l'intérêt d'une telle approche pluridisciplinaire sur le parcours de soins de ces femmes et de leurs enfants. Objectif : décrire le parcours de soins et le profil médico-psycho-social des femmes 1 à 3 ans après étude de leur situation en RCP et évaluer la place de ce passage en RCP dans leur trajectoire de vie. Matériels et méthodes : étude observationnelle descriptive transversale à partir des femmes dont les dossiers ont été étudiés en RCP de novembre 2014 à juin 2016. Résultats : concernant les motifs d'orientation en RCP, moins de la moitié des femmes (7 femmes, soit 44%) a été orientée pour des raisons d'accès aux soins difficiles ou des difficultés observées en matière de continuité des soins. Sur le plan social, la majorité des femmes incluses était en situation régulière, avait une couverture sociale et un logement fixe et stable. En revanche, peu d'entre elles ont un entourage social disponible. Sur le plan médical, toutes les femmes déclarent être en assez bonne, bonne voire très bonne santé ; 13 d'entre elles (81%) ont un Médecin généraliste traitant qui est le principal acteur du suivi médical (n=13 ; 81%) ; une seule femme a renoncé à des soins pour des raisons financières et la plupart n'ont pas eu besoin de recourir à un service d'Urgences depuis leur inclusion. Sur les plans psychologique/psychiatrique, on constate de lourds antécédents puisqu'on ne peut a priori exclure un EDM que chez 4 femmes (25%) et un ESPT que chez 6 d'entre elles (38%). En revanche, concernant les toxiques, leur consommation est inexistante ; de même, les consommations d'alcool et de tabac sont pour ainsi dire aussi inexistantes. Conclusion : la nécessité de travailler en réseau est cruciale. C'est ce que nous avons souhaité mettre en avant à travers ce travail. Il faut impérativement poursuivre des recherches visant l'harmonisation des réseaux de santé en périnatalité et la formalisation de staff parentalité ou staff médico-psycho-sociaux dans la prise en charge des femmes enceintes en situation de vulnérabilité.

I) Décrire la population adressée à ces réunions pluridisciplinaires. II) Étudier le fonctionnement de ces réunions. III) Étudier la concordance entre les décisions prises en anténatal et les prises en charge effectives à l'accouchement et durant le séjour en suites de couches. Méthodes : il s'agit d'une étude rétrospective de cohorte portant sur 140 dossiers présentés en réunion pluridisciplinaire au cours des années 2012, 2014, 2016 et 2018. Les fiches de synthèse élaborées à l'issue de ces staffs ont été consultées, ainsi que les dossiers psychiatriques des patientes. Résultats : la sélection des dossiers est conforme dans 98% des cas et la traçabilité des informations dans la fiche de synthèse est supérieure à 80% pour la moitié des indicateurs de processus. Le taux de conformité global entre les décisions prises en réunion et leur mise en œuvre en post-partum est de 68%. Les décompensations aigües psychiatriques et les signalements ont pu être anticipés. Conclusion : le dispositif est efficient dans le repérage précoce des situations à risque. Les décisions prises lors des réunions pluridisciplinaires en anténatal sont en majorité appliquées en post-partum dans le service des suites de couches. Dans les cas où elles ne sont pas appliquées, le dispositif offre une grande réactivité aux professionnels en post-partum.

En 2007, la Haute Autorité de Santé a estimé que le médecin généraliste pouvait suivre des femmes enceintes à faible niveau de risque. Notre objectif est d'étudier le parcours de soin des femmes enceintes en situation de précarité ayant accouché à la maternité d'Argentan en 2014 (niveau 1). Méthode : nous avons sélectionné les patientes selon un faisceau de critères de précarité. Puis nous avons comptabilisé leurs consultations à la maternité, et au cabinet du médecin traitant pendant leur grossesse. Nous avons enfin croisé ces données en fonction de 5 critères : bénéficiaire de la CMU, intervention de la PMI, présentation au staff médico-psycho-social, éloignement de la maternité, mère célibataire.Résultats : 160 patientes ont été sélectionnées. 148 parcours de soin ont été analysés. Pour 54 patientes, le médecin généraliste a posé le diagnostic de grossesse. Ils réalisaient en moyenne 1,6 consultations (de 0 à 18). Les patientes consultaient en moyenne 9,6 fois à la maternité, dont 2,1 fois en urgences, et 7,6 fois de manière programmée. Seul l'éloignement influençait le nombre de consultations. Conclusion : les médecins généralistes jouent un rôle non négligeable pour le diagnostic de grossesse. Puis les patientes se dirigent vers la maternité, même en plus des consultations programmées. La collaboration ville-hôpital tarde en matière de périnatalité, au détriment d'un suivi de plus grande proximité.

Les femmes « isolées » concernant 90 % des patientes de l'étude. Toutes les patientes ont vécu au moins un événement traumatique. Plus du tiers un Etat Dépressif Caractérisé ; et presque un quart un Trouble Anxieux Généralisé.Nous avons mené une réflexion sur la prévention et les soins précoces en psychiatrie périnatale de ces femmes pour mieux penser leur accompagnement au sein de nos dispositifs de soins sur les plans médical, psychologique et social. Une étude de plus grande ampleur sera nécessaire afin de préciser ces résultats en vue d'une meilleure prise en compte des facteurs de vulnérabilité dès la grossesse afin d'améliorer la prévention des risques encourus chez la femme et l'enfant à naître, dans ce contexte de.

Brown surveys the developments in theory and practice that follow from Freud’s original observations and traces this evolution from its conception to contemporary analytic field theory. Brown emphasizes that these unconscious transformational processes occur spontaneously, in the blink of an eye, through the "unconscious work" in which the analyst and patient are engaged. The Analyst's Reveries finds increasing veneration for the analyst’s use of their reveries, while revealing important differences amongst post-Bionians in how reverie is defined and used clinically. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation.
