Book excerpt: This book is a collection of discussions and presentations made by leading experts during a workshop to review the results of recent empirical studies on the effectiveness of chemopreventive agents on the reduction of cancer risk. The topics selected were intended to cover the main areas of research in this field over the last 5-10 years, as well as important issues concerning the design of research studies, cost-effectiveness of chemoprevention, and ethical concerns. A set of conclusions and recommendations are included to provide a guide to formulating future public health policy and research strategy.

Book excerpt: Despite significant advances in cancer treatment and measures of neoplastic progression, drug effect (or early detection, overall cancer incidence has increased, pharmacodynamic markers), and markers that measure cancer-associated morbidity is considerable, and overall prognosis as well as predict responses to specific therapy. cancer survival has remained relatively flat over the past All these biomarkers have the potential to greatly augment several decades (1,2). However, new technology the development of successful chemoprevention therapies, allowing exploration of signal transduction pathways, but two specific types of biomarkers will have the most identification of cancer-associated genes, and imaging of immediate impact on successful chemopreventive drug tissue architecture and molecular and cellular function is development—those that measure the risk of developing increasing our understanding of carcinogenesis and cancer invasive life-threatening disease, and those whose mo- progression. This knowledge is moving the focus of cancer lation can “reasonably predict” clinical benefit and, therapeutics, including cancer preventive treatments, to therefore, serve as surrogate endpoints for later-occurring drugs that take advantage of cellular control mechanisms clinical disease. Thus far, the biomarker that best measures to selectively suppress cancer progression. these two phenomena is intraepithelial neoplasia (IEN) Carcinogenesis is now visualized as a multifocal, because it is a near obligate precursor to cancer.

Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Available in PDF, EPUB and Kindle. Book excerpt: This book describes the principles and the application of the existing molecular methodology for the detection of early cancer. Discussion focuses on the molecular changes characterizing preneoplastic lesions; molecular targets for early detection; validation of molecular targets; and new diagnostic technology for early detection. The advantages of molecular detection over current methods are examined, as well as the importance of identifying and characterizing preneoplastic lesions. In addition to the uses of highly specific molecular probes to detect early cancer, this book demonstrates the many ways in which molecular markers serve oncology. While meeting pressing needs in the practice of medicine, molecular detection of early cancer scientifically necessitates a confrontation with the biology of cancer, such as the genetic determinants of progression, regression, dormancy, and invasion. This work not only discusses the diagnostic value of these molecular methods but views their practical benefits against a background of conventional morphology.

Book excerpt: This volume directs attention to the goal of chemoprevention. Topics include analyses of at-risk populations, problems in early detection, experimental approaches to cancer prevention, and preventative trials. The most recent approaches to chemoprevention are highlighted, with a particular focus on the use of diet-derived compounds. Special emphasis is placed on the prevention of breast, colon, and prostate cancer.

Book excerpt: The sixth annual research conference of the American Institute for Cancer Research was held August 31 and September 1, 1995, at the Loews L'Enfant Plaza Hotel in Washing ton, DC. In view of the promising leads in the diet/nutrition and cancer research field, the conference was devoted to "Dietary Phytochemicals in Cancer Prevention and Treatment. " The number of sessions was increased over that in previous conferences in order to accommodate the topics of interest. The conference overview, entitled "Plants and Cancer: Food, Fiber, and Phytochemicals," provided a framework for the following sessions. In addition, the attendees were reminded that for several decades epidemiologists have noted a lower risk of lung, esophageal, stomach, and colon cancer in populations consuming diets high in fruits and vegetables. However, isolation and ingestion of individual protective factors are not the preferred action since the complexity of the food and the matrix in which nutritional factors are embedded are important. The individual sessions then provided more insight as to why eating fruits and vegetables is associated with a lower risk of cancer. The first of these sessions was on "Isothiocyanates" that induce both the Phase I and Phase II enzymes that increase detoxification and conjugation reactions, thus causing more rapid removal of any xenobiotic or carcinogen. Thus, less carcinogen is available for interaction with DNA or other critical cellular macromolecules.

Book excerpt: A basic textbook addressed to medical and public health students, clinicians, health professionals, and all others seeking to understand the principles and methods used in cancer epidemiology. Written by a prominent epidemiologist and experienced teacher at the London School of Hygiene and Tropical Medicine, the text aims to help readers become competent in the use of basic epidemiological tools and capable of exercising critical judgment when assessing results reported by others. Throughout the text, a lively writing style and numerous illustrative examples, often using real research data, facilitate an easy understanding of basic concepts and methods. Information ranges from an entertaining account of the origins of epidemiology, through advice on how to overcome some of the limitations of survival analysis, to a checklist of questions to ask when considering sources of bias. Although statistical concepts and formulae are presented, the emphasis is consistently on the interpretation of the data rather than on the actual calculations. The text has 18 chapters. The first six introduce the basic principles of epidemiology and statistics. Chapters 7-13 deal in more depth with each of the study designs and interpretation of their findings. Two chapters, concerned with the problems of confounding and study size, cover more complex statistical concepts and are included for advanced study. A chapter on methodological issues in cancer prevention gives examples of epidemiology's contribution to primary prevention, screening and other activities for early detection, and tertiary prevention. The concluding chapters review the role of cancer registries and discuss practical considerations that should be taken into account in the design, planning, and conduct of any type of epidemiological research.
