Book excerpt: The objective of the EURO-QUAL programme has been to improve the quality of care for the individual patient. At the same time the need for harmonisation of health care in Europe was recognised. This Quality Improvement System (QIS) for orthodontic care in Europe comprises of 'Guidelines for how to implement a QIS' and reference material from the project teams: Financial Resources, Professional Development, Consumer Satisfaction, and Industrial Co-operation. In this respect the following topics are discussed: * The differences and similarities in local (national) quality initiatives. * The use of databases, as a technique to improve quality by comparing one's own performance against that of colleagues. * The influence of the different national systems of funding on both the amount of care provided as well as the quality of care. * The contents and the quality of the educational programmes, at all levels of orthodontic care. * Promotion of patients' rights and responsibilities before, during and after the orthodontic treatment. * A common goal and common language with the industrial partners by which the interaction can be optimised and communication will be enriched. This book provides the background information for the appropriate use of the European Orthodontic Quality Manual (EOQM). Strategies for the implementation of a QIS in orthodontics, taking into account the complex of the outlined aspects, make this book interesting for the orthodontic professional as well as the stakeholders (education, funding, industry, and consumers) in orthodontic care.

Book excerpt: The benefits for the professional working within a quality culture are manifold, but even more important, the patient is the final arbiter of quality care. Therefore patient centred aspects of treatment are focused upon in this manual. A quality manual intended for European use has to respect the national regulations and European diversity, and therefore needs to be flexible. The EOQM has been structured according to the treatment of the individual patient, which is what all professionals have in common, and which also is the most important target for quality improvement in orthodontic practice. The EOQM is primarily designed as a stimulus for the orthodontic professional to assess and improve the quality of orthodontic care. A Quality Improvement System (QIS) provides support for starting and maintaining activities. The manual provides tools to systematically assess and develop quality-related actions at individual (patient) treatment level and practice level. A systematic and comprehensive description of the treatment process is essential in order to provide a basis for establishing a QIS. Implementing a QIS will influence the provision of orthodontic care by: * Demonstrating the potentials for development of the organisational elements of orthodontics. * Planning and integrating systematic activities to monitor and improve treatment at the practice level and the patient-related level. Orthodontic treatment process is the central element in this manual. The route taken by an individual patient is described step by step. * Flowcharts describe the orthodontic treatment of the individual patient step by step. * Guidelines indicate what the professional should do at the various steps in order to follow the generally accepted objectives of orthodontics as described in the policy statements. * Indicators are tools to measure the effectiveness and efficiency of treatment. The indicators provide the professional with facts about own performance. The EOQM, provides the orthodontic professional with a model for incorporating quality improvement into daily practice, which is based on European orthodontic consensus and current principles of quality in health care. It can be incorporated in the quality initiatives that are already being developed nationally and vice versa.

Book excerpt: The three keywords, Orthodontics, Europe and Quality represent the important issues in this book, describing the EURO-QUAL, BIOMED 1 Programme. In this Programme, individuals and representatives of institutions have been asked to contribute to the discussion on the development of a quality system for orthodontic care. The philosophy of Total Quality Management was adopted and specific emphasis was given to patient quality. When considering how the structure and quality of European orthodontic care continuously can be improved, there are certain conditions which are difficult to modify. From the start this has been realised and respected and therefore the guidelines for the quality are flexible and can be adapted to different conditions. Available knowledge has been bundled and an attempt has been made to understand the reality of orthodontic care in Europe: the product and the process.

Abstract: Abstract of thesis entitled CHANGES IN CHILDREN'S ORAL HEALTH RELATED QUALITY OF LIFE FOLLOWING ORTHODONTIC TREATMENT Submitted by ZHANG MAN For the degree of Doctor of Philosophy at The University of Hong Kong in September 2007 Malocclusion is a common oral disorder and the demand for orthodontic care is increasing among children. There is a dearth of information as to how malocclusion and its treatment impacts on the life quality of children, which has obvious implications in assessing orthodontic treatment need and evaluating outcomes from orthodontic care. This prospective longitudinal study was conducted to identify associations between orthodontic treatment need and child oral health-related quality of life (COHQoL); to determine the level of agreement between parents' and children's perceptions of COHQoL; to determine changes in COHQoL as a result of undergoing fixed orthodontic appliance therapy and to determine changes in COHQoL following orthodontic treatment. Children (aged 11-14) seeking orthodontic treatment were recruited in this study and followed-up over a three year period. Children's parents were also recruited at baseline. Children self-completed the Child Perceptions Questionnaire (CPQ) at baseline (pre-treatment status and expectations of treatment), during orthodontic treatment (at one week, one month, three months, six months, twelve months, and eighteen months after insertion of fixed appliance), and 6-months post-treatment. Their parents completed the Parental Perception Questionnaire (PPQ) and Family Impact scale (FIS) prior to treatment. Children's pretreatment occlusal models were assessed by three commonly used occlusal indices: Index of Orthodontic Treatment Need (IOTN) including Aesthetic Components (AC) and Dental Health Components (DHC), Index of Complexity, Outcome and Need (ICON) and the Dental Aesthetic Index (DAI). Improvements in occlusion following orthodontic treatment was determined by assessing differences in ICON scores obtained from pre- and post- treatment orthodontic casts. CPQ scores and AC scores were significant correlated (r=0.27, p0.05). FIS scores were significantly correlated with scores of ICON (r= 0.26, pDuring orthodontic treatment significant changes in CPQ scores occurred (pThere was significant reduction in CPQ scores 6-months following orthodontic treatment compared to pre-treatment (p0.05). Perceived improvement in oral health was associated with improvement in COHQoL (reduction in CPQ scores) a

Book excerpt: ABSTRACT: Helping patients achieve an optimal quality of life through patient-centered treatment planning should be the ultimate goal of all oral health care providers. However, this issue extends beyond the realm of the individual clinician's office. This text presents quality-of-life research from various fields, including psychology, public health, and general health care; discusses how a patient-centered approach can be applied to basic oral and craniofacial research, clinical dental practice, community dental health issues, and dental education; and addresses how oral health-related quality of life relates to treating and understanding different patient populations, such as children with special needs, medically compromised patients, patients with oral cancer, and patients with chronic facial pain. Also discussed is how factors such as race/ethnicity, gender, and age can affect oral health-related quality-of-life concerns and treatment strategies. Finally, the book offers an outlook on the role that oral health-related quality of life will play in future research and dental education.

Integrated Clinical Orthodontics covers areas of complexity in clinical orthodontics, specifically the role of the orthodontist as a member of a multidisciplinary team. The book outlines and details the management of congenital orofacial deformities, sleep disorders, esthetic smile creation and temporomandibular joint problems, and additionally and importantly includes specific protocols for effective communication with experts in other specialties.

Subjects and method: The pre and post treatment casts were randomly selected from the University of Louisville and the University of Wales. Main outcome measured: An occlusal index was used to assess the treatment need, complexity and outcome of the cases. Statistical analyses: Kolmogorov-Smirnov two sample tests, the analysis yielded binomial proportions, an exact p value and odds ratio were used to explore outcomes measured. Results: There is a statistically significant difference between the two centers with regard to the distribution of the post treatment improvement (p = 0.0142); and the extraction rates (p = 5.671e005, OR = 4.6). Conclusion: An objective appraisal of the quality of orthodontic care provided in the both centers has been undertaken. The observed differences can be explained partly by the differences in the health care system and treatment philosophy.

Available in PDF, EPUB and Kindle. Book excerpt: Orthodontics aims to enhance the esthetic and functional components of the oral cavity and dentofacial complex. The purpose of this study was to examine quality of life (QoL) related issues in seeking orthodontic treatment. We will also examine how QoL is affected amongst varied gender, race, and age groups. Methods. We identified a random group of patients from 14-65 years with varied ethnicities. The patients completed a 22 question survey and the investigator evaluated their malocclusion via the ICON index. The sample was divided according to gender, race, and age groups for comparisons. Conclusions. QoL impact was relatively low. There is no correlation between the severity of the malocclusion and perceived QoL. Females have decreased QoL values, despite the severity of malocclusion. Younger patients have more severe malocclusions, but have a lower QoL impact. No significant differences in overall QoL or ICON were present between racial groups.

Choosing an orthodontist is kind of like selecting a parachute--the moment you realize you've made the wrong decision, it's too late to change your mind. ? The braces themselves are only part of the journey. It's also important to consider the quality of care, the relationship with the provider and team, the costs of treatment, and your expectations about the journey. When patients' braces are removed, their smile--the smile that was created through dedication and perseverance--will be with them when they do well in school, at their graduation ceremony, at their ? first kiss, at their? first job interview, on their wedding day, when they greet their children. It will be with them when they celebrate any event in life. With decades of experience, Dr. Doug Depew wrote Change Your Smile, Change Your Life: Your Guide to Orthodontic Treatment, a guide on everything you need to know about orthodontics. Don't waste another second--improve your health by learning if orthodontics is right for you.
