Our society wishes to ensure quality health care for all older people, but there is growing concern about our ability to maintain and improve quality in the face of efforts to contain health care costs. Medicare: A Strategy for Quality Assurance answers the U.S. Congress' call for the Institute of Medicine to design a strategic plan for assessing and assuring the quality of medical care for the elderly. This book presents a proposed strategic plan for improving quality assurance in the Medicare program, along with steps and timetables for implementing the plan by the year 2000 and the 10 recommendations for action by Congress. The book explores quality of careâ€"how it is defined, measured, and improvedâ€"and reviews different types of quality problems. Major issues that affect approaches to assessing and assuring quality are examined. Medicare: A Strategy for Quality Assurance will be immediately useful to a wide audience, including policymakers, health administrators, individual providers, specialists in issues of the older American, researchers, educators, and students.

Book excerpt: This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

Available in PDF, EPUB and Kindle. Book excerpt: In recent years, the attention of the scientific and social community has not solely been on producing new findings, but increasingly also on the related issues of the reliability, safety, and efficacy of the discoveries made, as well as the efficient and effective use of resources. The adoption of management models and tools can help scientists to improve their research, ensuring valuable, robust and dependable outcomes. Quality disciplines have been widely used for decades in industrial and business fields, building a knowledge base that can be translated and exploited, much to the advantage of scientific research. However, quality references in scientific research are still extremely rare and largely limited to an international guideline and a few sector-specific standards. Despite WHO and EU Commission campaigns, there are still precious few practical texts that offer researchers guidance on quality principles and provide simple tools and methodologies for their daily work. The book, starting from the problem of the reproducibility of scientific results and the substantial contribution that the Quality approach can make to research (Chapter 1), introduces the reader to key principles and basic concepts of Quality and illustrates both general and research-specific quality standards, paving the way for further discussion (Chapter 2). In turn, Chapter 3 presents detailed applications of Quality principles in various aspects of research, from study and ethics to materials and equipment management. Chapters 4 and 5, respectively, are devoted to Quality tools and Quality methodologies, as well as soft skills, all of which are valuable to scientific experimentation and study management. The concepts and practical tools discussed are extensively illustrated with examples from actual applications in scientific research.

Book excerpt: Although regularly introducing new products or services is the lifeblood of most industries, bringing them to market can be fraught with peril. Timing, cost, and quality all play important roles in a successful product launch and avoiding expensive — often in more than just dollars — recalls and redesigns. Quality Assurance: Applying Methodologies for Launching New Products, Services, and Customer Satisfaction details continual improvement (CI), a proven process for avoiding common problems and creating customer satisfaction. The book explores the three fundamental approaches required to create a truly CI culture in any organization: a) consistent philosophy of improvement by management, b) receptive organizational culture, and c) the entire culture of the organization must be willing to make decisions based on measurement and data. It outlines the seven principles: research/plan, assure, explain, prioritize, demonstrate, confirm, and show. However, as with CI itself, this attitude must be incorporated into the processes of any organization and create products or services for the market place that will delight customers rather than just satisfying them. Time and cost constraints are the biggest culprits here, not any one person’s lack of due diligence. When this happens, organizations must look at the bigger picture internally and identify it as a system problem. Based on the author’s 35 years of experience, this book covers the essential items for doing the right thing the first time especially during launching a good product and/or service to the customer. It identifies key indicators and methodologies that will help you attain excellent performance, delivery, and cost with both the customer and supplier. In other words, by following these methodologies and indicators, the job will get done right the first time.
