Book excerpt: Therapeutic architecture can be described as the people-centered, evidence-based discipline of the built environment, which aims to identify and support ways of incorporating those spatial elements that interact with people physiologically and psychologically into design. Architecture is an important factor in people's lives when they are well; when they experience ill-health and are less able to cope it becomes even more important. This book explores the design of specialized residential architecture for people with mental health problems. It sets out to show how building design can support medical and health related procedures and practices, leading to better therapeutic outcomes and an enhanced quality of life. Based on almost two decades of research, it aims to understand how architectural design interacts with the therapeutic milieu, the care programs, and actually living in the spaces. The book is divided into two main parts covering theory and research. Part one consists of three chapters: a brief introduction to old practices, current medical psychosocial and architectural thinking, and alternative thinking. Part two explores the research and conclusions derived from fieldwork. This book provides a fascinating insight into the effect that architectural design can have on all of us, but particularly on those with mental health problems. "Dr. Evangelia Chrysikou explains the many aspects of mental health and its relation to the quality of the built environment and I strongly recommend this very enjoyable book to anyone who would like to find out more about this important topic." - Prof. Alan Dilani, Ph.D. , International Academy for Design and Health "This book provides important, evidence-based data that will help to drive the design of new and refurbished psychiatric facilities and will no doubt become a highly-regarded resource for medical planners and architects." - Jo Makosinski , Editor, Building Better Healthcare

Book excerpt: Spurred by ideals of individual liberty that took hold in the Western world in the late nineteenth century, psychiatrists and public officials sought to reinvent asylums as large-scale, totally designed institutions that offered a level of freedom and normality impossible in the outside world. This volume explores the “caged freedom” that this new psychiatric ethos represented by analyzing seven such buildings established in the Austro-Hungarian monarchy between the late 1890s and World War I. In the last two decades of the Habsburg Empire, architects of asylums began to abandon traditional corridor-based plans in favor of looser formations of connected villas, echoing through design the urban- and freedom-oriented impulse of the progressive architecture of the time. Leslie Topp considers the paradoxical position of designs that promoted an illusion of freedom even as they exercised careful social and spatial control over patients. In addition to discussing the physical and social aspects of these institutions, Topp shows how the commissioned buildings were symptomatic of larger cultural changes and of the modern asylum’s straining against its ideological anchorage in a premodern past of “unenlightened” restraint on human liberty. Working at the intersection of the history of architecture and the history of psychiatry, Freedom and the Cage broadens our understanding of the complexity and fluidity of modern architecture’s engagement with the state, with social and medical projects, and with mental health, psychiatry, and psychology.

Book excerpt: Studies confirm that the physical environment influences health outcomes, emotional state, preference, satisfaction and orientation, but very little research has focused on mental and behavioural health settings. This book summarizes design principles and design research for individuals who are intending to design new mental and behavioural health facilities and those wishing to evaluate the quality of their existing facilities. The authors discuss mental and behavioural health systems, design guidelines, design research and existing standards, and provide examples of best practice. As behavioural and mental health populations vary in their needs, the primary focus is limited to environments that support acute care, outpatient and emergency care, residential care, veterans, pediatric patients, and the treatment of chemical dependency.

Book excerpt: St. Elizabeths has been a mental health hospital in Washington, D.C., since 1852, when it was established by the United States Congress as the Government Hospital for the Insane. St. Elizabeths, along with other hospitals, experienced rapid expansion in its first century, hitting a peak of almost eight thousand patients by the 1960s. Deinstitutionalization in the second half of the twentieth century emptied out the historic buildings on campus. This well-illustrated book follows an exhibition at the National Building Museum, tracing the hospital's evolution over time, highlighting the ways that this specialized architecture and landscape served the mentally ill. It continues the story of St. Elizabeths, a National Historic Landmark, through its current redevelopment as a federal campus and mixed-use neighborhood.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides an overview of a diverse array of preventive strategies relating to mental illness, and identifies their achievements and shortcomings. The chapters in this collection illustrate how researchers, clinicians and policy makers drew inspiration from divergent fields of knowledge and practice: from eugenics, genetics and medication to mental hygiene, child guidance, social welfare, public health and education; from risk management to radical and social psychiatry, architectural design and environmental psychology. It highlights the shifting patterns of biological, social and psychodynamic models, while adopting a gender perspective and considering professional developments as well as changing social and legal contexts, including deinstitutionalisation and social movements. Through vigorous research, the contributors demonstrate that preventive approaches to mental health have a long history, and point to the conclusion that it might well be possible to learn from such historical attempts. The book also explores which of these approaches are worth considering in future and which are best confined to the past. Within this context, the book aims at stoking and informing debate and conversation about how to prevent mental illness and improve mental health in the years to come. Available in PDF, EPUB and Kindle. Book excerpt: In psychiatry there is no sharp boundary between the normal and the pathological. Although clear cases abound, it is often indeterminate whether a particular condition does or does not qualify as a mental disorder. For example, definitions of subthreshold disorders and of the prodromal stages of diseases are notoriously contentious. Philosophers and linguists call concepts that lack sharp boundaries, and thus admit of borderline cases, 'vague'. Although blurred boundaries between the normal and the pathological are a recurrent theme in many publications concerned with the classification of mental disorders, systematic approaches that take into account philosophical reflections on vagueness are rare. This book provides interdisciplinary discussions about vagueness in psychiatry by bringing together scholars from psychiatry, psychology, philosophy, history, and law. It draws together various lines of inquiry into the nature of gradations between mental health and disease and discusses the individual and societal consequences of dealing with blurred boundaries in medical practice, forensic psychiatry, and beyond. --

Book excerpt: ***WINNER OF A NAUTILUS 2018 SILVER MEDAL BOOK AWARD*** Innovations in Behavioural Health Architecture is the most comprehensive book written on this topic in more than 40 years. It examines the ways in which healthcare architecture can contribute, as a highly valued informational and reference source, to the provision of psychiatric and addictive disorder treatment in communities around the world. It provides an overview of the need for a new generation of progressively planned and designed treatment centres – both inpatient and outpatient care environments – and the advantages, challenges, and opportunities associated with meeting the burgeoning need for treatment settings of this type. Additional chapters address the specifics of geriatric psychiatry and its architectural ramifications in light of the rapid aging of societies globally and provide a comprehensive compendium of planning and design considerations for these places in both inpatient and outpatient care contexts. Finally, the book presents an expansive and fully illustrated set of international case studies that express state-of-the-art advancements in architecture for behavioural healthcare.
