Book excerpt: L’objectif de cette étude était d’évaluer la prévalence de la pseudo-accommodation chez des enfants opérés de cataracte congénitale bilatérale sans amblyopie et d'en définir les mécanismes. Nous avons inclus 40 yeux de 23 enfants âgés de 6 à 18 ans, avec une acuité visuelle supérieure à 8/10 de loin corrigée et P2 de près avec addition. 36 avaient une acuité visuelle de près au moins égale à P4 avec la seule correction de loin (sans addition). La comparaison avec les enfants qui ne pseudo-accommodaient pas était difficilement interprétable du fait du faible effectif de ce groupe. Nous avons alors inclus un autre groupe, d’adultes, pseudophakes, avec une acuité visuelle supérieure à 8/10 de loin et P2 de près avec addition. Sur les 28 yeux inclus, 26 ne pseudo-accommodaient pas, c’est à dire avaient une acuité visuelle de près sans addition inférieure à P6. Afin de déterminer les facteurs expliquant la pseudo-accommodation chez les enfants, nous avons comparé le groupe d’enfants avec pseudo-accommodation et d’adultes sans pseudo-accommodation sur différents paramètres : réfraction, longueur axiale, topographie cornéenne, aberrométrie, diamètre pupillaire, déplacement de l’implant après cyclopentolate. L’équivalent sphérique, la puissance de l’implant, la multifocalité cornéenne et les aberrations cornéennes étaient significativement plus importants dans le groupe avec pseudo-accommodation. Par contre le diamètre pupillaire et le déplacement de l’implant étaient statistiquement comparables dans les deux groupes. Ainsi différents paramètres entreraient en jeu dans le mécanisme de la pseudo-accommodation chez l’enfant, expliquant sa forte prévalence dans cette population

Les enfants étaient opérés en moyenne à 2,98 ans. Chez les enfants âgés de moins de 2 ans lors de la chirurgie, la myopisation est en moyenne –9,15 D et –2,13 D pour les autres. La variabilité de la myopisation diminue avec l'augmentation de l'âge lors de la chirurgie. L'acuité visuelle moyenne est 0,55, avec 75,5% des enfants ayant >= 0,5. 4 changements d'implant ont été nécessaires en raison d'une anisométropie majeure permettant une acuité visuelle finale moyenne de 0,7. En conclusion, une chirurgie avec implantation primaire réalisée précocement associée à un traitement intensif et prolongé de l'amblyopie permet d'obtenir des résultats fonctionnels très bons à long terme. Viser l'emmétropie ou minimiser l'hypermétropie post-opératoires semble intéressant. Prévoir les variations réfractives de l'oeil opéré pour un individu donné reste difficile et peu fiable pour les très jeunes enfants. Un changement d'implant est envisageable lorsque l'anisométropie devient un obstacle à la rééducation de l'amblyopie.

Si la nécessité d'opérer les formes obturantes n'est plus débattue, il demeure des controverses concernant sa prise en charge. Matériels et méthodes : Notre étude est rétrospective, observationnelle, monocentrique. Nous avons inclus les enfants implantés au CHRU de Lille entre 2004 et 2012. Nous avons évalué l'acuité visuelle et la réfraction cinq ans après la chirurgie, et tenté d'identifier des facteurs de risque pronostiques de bonne acuité. Résultats : L'étude inclut 31 enfants. La moyenne des acuités visuelles à cinq ans est à +0,93 logMAR. La persistance de la vascularisation foetale, la microphtalmie et le nystagmus semblent être des facteurs de risque de mauvaise acuité visuelle. La moyenne des acuités cinq ans après la chirurgie est respectivement à +1,35 (p=0,065), +1,00 et +1,21 (p=0,25). Les enfants ayant bénéficié d'une vitrectomie antérieure ont une acuité moyenne à +1,00. Le seul facteur significativement associé à une meilleure vision est la poursuite de l'occlusion à cinq ans. Les enfants concernés ont une acuité visuelle moyenne à +0,69 contre +1,38 chez ceux l'ayant abandonné (p=0,028). Les enfants opérés avant 6 mois ont une acuité moyenne à +1,06 contre +0,85 chez les autres (p=0,76). Cela semble lié aux comorbidités oculaires chez les patients diagnostiqués tôt. Concernant les résultats réfractifs, la réfraction moyenne cinq ans après la chirurgie est à +1,12 dioptries. Le shift myopique moyen est à 4,26 dioptries. Conclusion : Les progrès techniques permettent d'obtenir de bons résultats anatomiques et réfractifs cinq ans après la chirurgie, en l'absence de comorbidités oculaires. Les efforts doivent être concentrés sur l'éducation visuelle et son assiduité.

Book excerpt: Objectif : proposer une formule logarithmique pour calculer la puissance de l'implant intra-oculaire chez l'enfant permettant de viser l'emmétropie à l'âge de 15 ans et de prévenir le développement d'une myopie forte à l'âge adulte. Lieu : service d'ophtalmologie, Centre Hospitalo-Universitaire de Marseille, France. Type d'étude : étude rétrospective. Méthodes : les enfants opérés de cataracte congénitale et implantés avant l'âge de 15 ans entre 2005 et 2020 ont été inclus. La réfraction et biométrie ont été recueillies avant implantation et au dernier contrôle. L'équivalent sphérique sous cycloplégie a été calculé en post-opératoire immédiat et au dernier contrôle. L'erreur réfractive avec la nouvelle méthode a été estimée grâce à la soustraction de l'équivalent sphérique final et de la différence entre les cibles réfractives de l'implant choisi avec l'ancienne méthode et celui choisi avec la nouvelle méthode dans la biométrie finale. L'erreur réfractive avec la méthode de Dahan, Enyedi et Trivedi a aussi été estimée et comparée à la nouvelle méthode. Résultats : l'analyse a été faite sur 35 yeux de 26 enfants. Au dernier contrôle, l'âge médian était 8 ans et l'équivalent sphérique médian était -1,75 D (EI -3,69 à -0,44). L'erreur réfractive médiane prédite était 1,09 D (EI -1,38 à 2,15) avec la nouvelle méthode, -1,37 D (EI -3,75 à 0,06) avec la méthode Dahan, -0,30 D (EI -1,85 à 1,17) avec la méthode Enyedi et 0,84 D (EI -2,22 à 1,81) avec la méthode Trivedi. Conclusion : la nouvelle méthode présente le résultat réfractif le plus hypermétrope lors du dernier contrôle, permettant donc de compenser la myopie induite par la croissance de l'œil entre 8 et 15 ans. Ainsi, cette nouvelle formule pourrait aider à minimiser la myopie chez les enfants implantés dans la cataracte congénitale.

Valient efforts have been made by some individuals at great personal sacrifice and by individual societies or organizations to prevent and cure blindness and to rehabilitate those who have lost their sight. Only recently, however, have such efforts been consolidated into a major international force. In 1969 the 22nd World Health Assembly of WHO adapted a resolution requesting the Director-General to undertake a study on the information which is at pre sent available on the extent and/or causes of preventable and curable blind ness. In 1972 a working group was convened by the World Health Organiza tion in Geneva to outline an attack on the prevention of blindness. At the Paris meeting of the International Congress of Ophthalmology in July of 1974, Mr. JOHN WILSON, who for many years had been one of the leaders in the prevention and cure of blindness on an international basis through his organization of the Royal Commonwealth Society for the Blind, was elected President of the International Association for Prevention of Blindness.
