Book excerpt: INTRODUCTION: Among youth, marginalized groups, such as Latino YLWH (LYLWH), may face multiple social stigmas, including such concerns as limited English-language proficiency, sexual-minority status, and immigration status that impact engagement across the continuum of HIV care (van Servellen, Chang, & Lombardi, 2002). One necessary step toward increasing understanding of the facilitators and barriers to this group's engagement in HIV care is to examine engagement in care through a socioecological framework to better understand each of the multiple factors impacting Latino youth's engagement in HIV care (Bronfenbrenner, 1986; Hosek, Harper, Lemos et al., 2005; Mugavero, Norton & Saag, 2011). Through the use of key informants such as such as healthcare providers' experiences and perceptions of barriers and facilitators we can yield important and insightful information on how to better address the needs of these youth. APPROACH: This study used a psychological phenomenological approach, through inductive and deductive analysis, to explore the socioecological factors that specifically affect engagement in care for LYLWH from providers' perspective. I conducted qualitative interviews with 26 healthcare providers who provide services to LYLWH. RESULTS: The following themes emerged as important factors impacting engagement in care for LYLWH. Individual-level themes specific to LYLWH included mental health issues, substance use, adjustment to HIV diagnosis, and HIV education. Intrapersonal-level themes specific to LYLWH included the systematic impacts of family, providers, and one-on-one relationships with peers and romantic or sexual partners. Pertaining to community-level factors impacting engagement in care for LYLWH, providers discussed the role of compounded stigma surrounding HIV and LGBT issues, along with HIV-related stigma, poverty/unstable housing, and social-community norms. Sociocultural and policy-level factors include traditional Latino cultural beliefs, religious beliefs, acculturation, U.S. immigration policy, U.S. healthcare access and the healthcare system. In sum, study results reveal that individual (intrapersonal), interpersonal (microsystem), community (exosystem), and sociocultural/policy-level (macrosystem) factors are critical considerations for engagement in HIV care. IMPLICATIONS: It is clear from the current study that LYLWH engage across the continuum of care in distinctive ways, resulting from community-level factors and the sociocultural and policy-level factors that directly and indirectly affect their ability to maintain optimal engagement in their HIV care. Additionally, these macrosystemic factors relate to other syndemic experiences such as mental health disorders and substance abuse, poverty, and HIV and LGBT stigma affecting Latino youth in general in our current society. By addressing these factors, society can positively affect the health of youth, inclusive of LYLWH.

Book excerpt: It is widely recognised that adolescents and young people living with HIV (AYPLHIV) on antiretroviral therapy (ART) have poorer adherence and retention in care compared with adults. Adolescent-focused HIV services are urgently needed to improve their health and well-being. To increase ART adherence Africaid Zvandiri has trained AYPLHIV as Community Adolescent Treatment Supporters (CATS) in Zimbabwe. Their role is to support AYPLHIV across the HIV cascade through a variety of complementary services integrated within government and private sector clinical care packages, and social protection services.Operations research in a rural Zimbabwe district have found the CATS model improves adherence, retention and psychosocial well-being among HIV positive peers. As a result, at the end of 2017, it was established in 51 of 63 districts (81%) across all 10 Zimbabwe provinces. Zvandiri provides direct services for 86,481 CAYPLHIV in Zimbabwe, through a team of 1167 CATS integrated in 613 clinics. Africaid, in partnership with the International HIV/AIDS Alliance, PATA, REPSSI, Y+,CANGO, TACOSODE and the Ministry of Health from Zimbabwe, Mozambique, Swaziland and Tanzania engaged in a consortium to scale up the CATS model these countries under the READY + project. In the first 18 months, 287 AYPLHIV 18-24 years, have been trained as CATS across the four countries, 70% of whom are young women. They are integrated within clinics and mentored in the provision of differentiated services through home visits, clinic-based sessions and support groups. They have reached 13,132 young people with services.Advocacy has been pivotal to progress made. Coordinated efforts with the Ministries of health resulted in approval for the implementation and standardisation of the CATS model; the acceptance of consortium tools and guidelines as part of the technical assistance package delivered to health service providers; and the acceptance of young people as service providers. Taken together, it has worked to provide a supportive environment for CATS to provide support to their peers. The CATS model has been shown to be replicable and acceptable in three new countries. Book excerpt: Background: The transition from pediatric to adult care has emerged as a potential contributor to poor clinical outcomes and challenges to retention in care among adolescents living with HIV (ALHIV). While structured processes and tools that bridge the gap between pediatric and adult care can help ensure HIV-infected adolescents and young adults are retained in care and supported through this developmental transition, additional context-specific data are needed to improve the transition process. This study elicited perspectives of policy-makers regarding barriers and facilitators to successful transition outcomes among ALHIV in Kenya. Methods: We conducted 20 individual in-depth interviews (IDIs) with a diverse group of policy-makers in Kenya. IDIs were conducted using a semi-structured guide, designed to elicit policy-maker perspectives on current policies, tools and practices for transitioning adolescents from pediatric to adult care. IDIs were conducted in English, audio-recorded and transcribed verbatim. Transcripts were coded using ATLAS.ti, and the socio-ecological model was used to characterize themes related to multilevel optimization of the transition process for ALHIV. Results: Policy-makers identified multilevel and interrelated factors impacting HIV-infected adolescents’ transition from pediatric to adult care. At the interpersonal level, the influence of adolescents’ primary caregivers was paramount and contributed to adherence and retention in care throughout the transition trajectory. At the institutional level, key themes related to successful transition included the need for gradual, structured, and inclusive preparation of adolescents, and the pivotal role of healthcare workers (HCWs) in the development of a comprehensive care relationship between HCWs, caregivers and adolescents. Key themes at the structural level were the need for expanded transition guidelines and improved HCW training. Conclusions: There is an urgent need for expanded transition tools and guidelines focused on strategies to improve caregiver involvement, engage adolescents in illness management, and provide commensurate support and guidance for HCWs. As relatively little research on adolescents’ transition from pediatric to adult care has been conducted in SSA, additional context-specific studies are needed to fill this gap and inform regional policy development.

STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.
