En prothèse fixée implanto-portée, parmi les problèmes encourus lors des phases chirurgicales et prothétiques, on constate des complications fréquentes au niveau de la jonction implant-pilier comme des dévissages, des déformations ou des fractures. La pérennité de cette connexion, primordiale pour la stabilité du complexe implant-pilier-prothèse, dépend de certains facteurs : le respect des facteurs de risque bioinécanique, l'ajustage et le serrage optimisés des pièces, la résistance aux contraintes masticatoires, l'étanchéité à l'interface, la biocompatibilité de la surface externe des éléments. Les propriétés mécaniques et biologiques de la jonction sont en rapport avec le projet prothétique, la méthodologie clinique, les techniques de laboratoire, mais également avec le type de système utilisé. A ce titre, les fabricants présents sur le marché proposent divers systèmes de jonction implant-pilier et les données qu'ils fournissent vantent les qualités de leurs propres systèmes. A partir de ces données et de notre étude bibliographique, nous proposons une classification des systèmes et une comparaison relative, basée sur des considérations biomécaniques, géométriques et biologiques. On remarque des divergences notables, néanmoins il semble que les connexions internes profondes présentent des avantages particuliers par rapport aux connexions de type externe comme l'hexagone externe. Mais de nouvelles techniques de solidarisation des pièces méritent d'être expérimentées par des études objectives afin de garantir, à l'avenir, la pérennité de cette interface.
