Book excerpt: Introduction: La plupart des modèles visant à prédire le mauvais pronostic fonctionnel à 3 mois (mRS >2) après un infarctus cérébral (IC) ont été construits à partir des données disponibles à l'admission. Ils sont souvent complexes et leurs performances prédictives sont imparfaites. Notre objectif était de déterminer s'il est possible de construire un modèle prédictif simple à partir des données recueillies à 24h, qui permette une excellente discrimination (ASC-ROC>0,90) pour prédire le mRS>2. Méthodes: Deux cohortes de patients pris en charge pour un IC≤6h ont été formées : patients traités ou non par thrombolyse intraveineuse (TIV+ ; TIV-). Pour chaque cohorte, des modèles pronostiques cliniques ont été créés à partir des données disponibles à l'admission, puis à 24 heures, à l'aide de régressions logistiques pas à pas. L'intérêt d'ajouter la variable DWI-ASPECTS≤5 au sein de chaque modèle a ensuite été évalué. Les modèles ont été comparés grâce à leur ASC-ROC. Résultats: Dans la cohorte TIV+ (n=421), la discrimination du modèle clinique obtenu avec les données à 24h (NIHSS, âge) était meilleure que celle du modèle à l'admission (NIHSS, âge, glycémie, mRS préexistant) : ASC-ROC=0,92 contre 0,79 (p0,001). Dans la cohorte TIV- (n=324), la discrimination du modèle clinique obtenu avec les données à 24h (NIHSS, âge) était également meilleure que celle du modèle à l'admission (NIHSS, âge, glycémie) : ASC-ROC=0,94 contre 0,88 (p=0,008). L'ajout de la variable DWI-ASPECTS ne permettait d'améliorer la prédiction qu'à l'admission chez les TIV+. Conclusion: Un modèle prédictif comportant simplement l'âge et le score NIHSS à 24h permet une excellente prédiction du mRS à 3mois 2.
