Book excerpt: The purpose of the literature review is to examine the efficacy of corneal collagen crosslinking for progressive keratoconus in pediatric patients. Collagen crosslinking (CXL) is a technique that utilizes riboflavin and ultraviolet-A as an effective treatment option for progressive keratoconus in pediatric patients. Several articles have shown topographic results with statistically significant improvement in K readings and visual acuity on the patients. The procedure has proven to be safe on the pediatric population, even do there are studies conducted every day for improvement of it. CXL acts by reducing the mean refractive cylinder nd flatening the cornea, and appears effective in halting the progession of the disease. Ultimately, what is being study in the literature review is that crosslinking stabilizes the condition and, thus, reduces the need for corneal grafting in these young patients for a better and healthy vision.

Early diagnosis of keratoconus is of major importance for effective treatment. That is why corneal topography and tomography has become the gold standard examination for diagnosis of corneal ectasia. Corneal topography is a non-contact imaging tool that gives information about the anterior surface of the cornea. Corneal tomography evaluates both anterior and posterior corneal surfaces and is considered mandatory for preoperatory evaluation before refractive surgery. Until recently, Kmax was the main indicator for the progression of keratoconus, but recent studies showed that posterior elevation represents the first signal of an ectatic corneal disease. Cross-linking is considered the gold standard in the treatment of keratoconus and newer techniques allow this procedure to be selective on the areas of corneal thinning.

The review of published literature on corneal cross-linking post-operative improvement in keratoconus patient supports the recommendation Corneal Cross-linking application for it's safety, comfort, efficacy, and the long term visual improvement.
