Book excerpt: Suite à une lésion cérébrale acquise de l’hémisphère droit, des difficultés pragmatiques-communicationnelles, relevées chez plus de la moitié des patients, impactent négativement leurs activités et relations, générant un handicap social. Cependant, l’intérêt pour ces troubles est récent. Peu d’outils d’évaluation et d’intervention sont à disposition des orthophonistes qui s’appuient sur une pratique basée sur les preuves nécessitant des outils spécifiques et validés scientifiquement. Dans cette optique, un programme d’intervention orthophonique visant la rééducation des habiletés pragmatiques-communicationnelles en situation de conversation auprès de patients cérébrolésés droits a été élaboré par une équipe de recherche québécoise. Notre étude vise à évaluer les apports de ce programme sur l’évolution des habiletés pragmatiques-communicationnelles (prosodie, débit, contact visuel, maintien du thème, expressions faciales, quantité et qualité de l’information et tours de parole) d’une participante cérébrolésée droit. Nous lui avons proposé une évaluation avant et après l’intervention associant des épreuves d’évaluation de la communication et de conscience du trouble et une analyse approfondie de la conversation afin d’identifier ces performances. Nos résultats semblent en accord avec la littérature : le programme permet une amélioration de la majorité des habiletés entraînées mais elle s’avère faible pour la prosodie et les expressions faciales. Nous ne notons pas de distinction marquée entre l’évolution des habiletés entraînées et non-entraînées dans l’intervention. Le questionnaire de conscience des troubles utilisé n’a pas permis de statuer sur les liens entre la conscience du trouble de la patiente et ses troubles communicationnels. Suite à cette étude, nous soulevons la problématique de l’évaluation des troubles communicationnels des patients cérébrolésés droits, en soulignant l’intérêt d’associer des épreuves d’évaluation de la communication et une analyse approfondie de la conversation. Ce travail pourrait être poursuivi avec une étude de groupe pour statuer sur l’efficacité du programme, et inclure le partenaire privilégié de communication afin de généraliser les acquis

Le contexte quotidien du patient et sa qualité de vie doivent alors être considérés dans l'évaluation et la prise en charge. En effet, l'intervention orthophonique doit avoir pour objectif, au-delà de l'amélioration des troubles, la satisfaction dans la communication, garante d'un maintien des liens sociaux et d'une réintégration sociale. Les préconisations de rééducation auprès de cette population mentionnent la pertinence du contexte de groupe et du jeu de rôle pour l'expérimentation de situations d'interaction proches du quotidien. Nous avons alors dégagé les intérêts potentiels de l'art dramatique dans la prise en charge des troubles des habiletés pragmatiques. Afin d'en démontrer l'intérêt effectif, nous avons animé un Groupe de Rééducation Pluridisciplinaire utilisant des techniques d'art dramatique (GRP communication). Nous avons alors évalué les patients avant et après l'intervention pour apprécier l'évolution de leurs performances pragmatiques en situation de test et en situation de communication quotidienne. L'appréciation de l'impact des troubles au quotidien a nécessité la création d'un questionnaire, le COMSO. La qualité de vie a également été mesurée. Notre étude a révélé, pour tous les patients, une amélioration des habiletés pragmatiques et de la qualité de vie, principalement dans son domaine social. En l'absence de sujets contrôles, nous concluons en une présomption d'efficacité du GRP communication.

Le présent travail de recherche a donc pour objectif de pallier ce manque et présente un matériel d'intervention pour la réhabilitation des habiletés pragmatiques expressives. Ainsi ce matériel propose de travailler trois objectifs en conversation: le maintien du sujet, la prise en compte du savoir partagé et la bonne gestion de l'échange. Ces objectifs sont travaillés selon trois principes directeurs: la sensibilisation du patient à ses troubles, la progression de la difficulté et la prise en compte des troubles cognitifs associés. Dans une première étude, le matériel a été mis à l'épreuve auprès de participants contrôles. Les résultats de cette étude ont permis de réajuster les exercices et les tâches du matériel. Puis dans une deuxième étude, le matériel réajusté a été soumis à des orthophonistes expérimentés qui l'ont mis à l'épreuve auprès de patients cérébrolésés droits et traumatisés crâniens présentant des troubles pragmatiques expressifs. Selon les orthophonistes, le matériel est pertinent, généralisable à une majorité de patients présentant ces troubles pragmatiques et répond au besoin clinique.

En parallèle de la mise en pratique du programme auprès d’un groupe de patients, le matériel a été analysé et critiqué par 34 orthophonistes ce qui va permettre d’ajuster le programme au plus près des besoins cliniques.
