Book excerpt: Despite recent advances in adjuvant therapies of cancer, the regi mens of postoperative adjuvant chemotherapy treatment which are presently available fail to cure the majority of cancer patients. Pre operative (neoadjuvant) chemotherapy represents a new approach in drug scheduling, based on sound theoretical, pharmacokinetic, and experimental principles. The preoperative timing of chemotherapy before definitive sur gery is not a minor change in the therapy of cancer. To be successful, large numbers of practitioners and their patients must participate. Substantial alterations of many aspects of the present management of cancer will have to follow. Therefore, before such therapy can be fully and routinely implemented, results of the novel treatment and its rationale have to be carefully evaluated. In preoperative treatment, other features will likely gain impor tance. For the first time, clinicians have a chance to follow the in vivo response of the tumor exposed to preoperative chemotherapy. The subsequent histological assessment of the tumor sample may likely become an important prognostic guide, permitting more re fined individual approaches to the planning of postoperative adju vant treatment. The value of such a treatment strategy can already be appreciated in the clinical setting, as seen from the therapy of osteosarcoma. Furthermore, preoperative chemotherapy might render previously inoperable tumors operable and hence resectable with a curative intention. The preoperative reduction of tumor bulk may also effectively decrease the need for more radical operations, permitting a more uniform adoption of conservative surgery.

Book excerpt: Expert laboratory and clinical researchers from around the world review how to design and evaluate studies of tumor markers and examine their use in breast cancer patients. The authors cover both the major advances in sophisticated molecular methods and the state-of-the-art in conventional prognostic and predictive indicators. Among the topics discussed are the relevance of rigorous study design and guidelines for the validation studies of new biomarkers, gene expression profiling by tissue microarrays, adjuvant systemic therapy, and the use of estrogen, progesterone, and epidermal growth factor receptors as both prognostic and predictive indicators. Highlights include the evaluation of HER2 and EGFR family members, of p53, and of UPA/PAI-1; the detection of rare cells in blood and marrow; and the detection and analysis of soluble, circulating markers.

Book excerpt: The European School of Oncology came into existence to respond to a need for informa tion, education and training in the field of the diagnosis and treatment of cancer. There are two main reasons why such an initiative was. considered necessary. Firstly, the teaching of oncology requires a rigorously multidisciplinary approach which is difficult for the Univer sities to put into practice since their system is mainly disciplinary orientated. Secondly, the rate of technological development that impinges on the diagnosis and treatment of cancer has been so rapid that it is not an easy task for medical faculties to adapt their curricula flexibly. With its residential courses for organ pathologies and the seminars on new techniques (laser, monoclonal antibodies, imaging techniques etc.) or on the principal therapeutic controversies (conservative or mutilating surgery, primary or adjuvant chemotherapy, radiotherapy alone or integrated), it is the ambition of the European School of Oncology to fill a cultural and scientific gap and, thereby, create a bridge between the University and Industry and between these two and daily medical practice. One of the more recent initiatives of ESO has been the institution of permanent study groups, also called task forces, where a limited number of leading experts are invited to meet once a year with the aim of defining the state of the art and possibly reaching a consensus on future developments in specific fields of oncology.

Book excerpt: The optimal management of breast cancer patients relies on the expertise of a team of medical specialists including radiologists, surgeons, radiation therapists and medical oncologists. Much of the progress in breast cancer management made over the last several years reflects the translation of observations made in the laboratory to the clinic. Critically evaluating the impact of new treatment approaches relies on a commitment to well-designed clinical trials. In this volume, Advances in Breast Cancer Management, a renowned group of breast cancer experts have been asked to provide their perspective on management issues that directly effect patients on a day-to-day basis. Dr. Melody Cobleigh discusses the consequences of estrogen deprivation and the ways of ameliorating secondary symptoms and the potential long-term morbidity. Drs. Haigh and Guiliano review the sentinel lymph node biopsy technique including results from their extensive experience. Dr. Abram Recht places into perspective the potential benefit of post-mastectomy radiotherapy and reviews recent trials that address this issue. Dr. Dennis Slamon takes from us from the laboratory to the clinic in explaining the development of Herceptin as a paradigm for therapy targeted to specific molecular characteristics of breast cancer tumor cells. Drs. Nieto, Shpall, Crump and Pritchard offer different perspectives on the future of high-dose chemotherapy with stem cell transplantation as a treatment for breast cancer patients. Drs.

Book excerpt: Lymph node (LN) status is the most important prognostic variable used to guide ER positive (+) breast cancer treatment. While a positive nodal status is traditionally associated with a poor prognosis, a subset of these patients respond well to treatment and achieve long-term survival. Several gene signatures have been established as a means of predicting outcome of breast cancer patients, but the development and indication for use of these assays varies. Here we compare the capacity of two approved gene signatures and a third novel signature to predict outcome in distinct LN negative (-) and LN+ populations. We also examine biological differences between tumours associated with LN- and LN+ disease. Proceeds from the sale of this book go to support an elderly disabled person.

Book excerpt: The results of randomized trials evaluating the use of early or adjuvant systemic treatment for patients with resectable breast cancer provide an eloquent rebuttal to those who would argue that we have made no progress in the treatment of cancer. Many of the tumors that we have been most successful in curing with chemotherapy and other newer forms of treatment are relatively uncommon. In contrast, breast cancer continues to be the single most common malignancy among women in the western world, is increasingly a cause of death throughout Asia and Third-World countries, and remains one of the most substantial causes of cancer mortality world wide. The use of mammography as a means of early detection has been shown to reduce breast cancer mortality by 25-35% among those popu lations in which it is utilized. The use of adjuvant systemic treatment in appropriate patients provides a similar (and additional) reduction in breast cancer mortality. Few subjects have been so systematically studied in the history of medicine, and it seems fair to conclude that the value to adjuvant systemic therapy in prolonging the lives of women with breast cancer is more firmly supported by empirical evidence than even the more conventional or primary treatments using various combinations ofsurgery and radiotherapy.

Book excerpt: Although there are numerous technical-scientific books on breast cancer in the global bibliography, such books deal exclusively with the nature of the disease in majority populations of the Western societies, with little or no reference to the nature of the disease in the minority populations in such societies. Similarly, the nature of breast cancer in black women of the less privileged societies, and in women of ethnic groups living in countries of similar socio-economic status, is virtually unknown. For various epidemiological reasons, breast cancer incidence is rapidly increasing in these counties, more so than currently is the case in developed countries. Thus, the global burden of cancer is shifting gradually to these areas of the world, and may equal or even surpass the breast cancer burden in the Western societies within the foreseeable future. This book is unique because it bucks the trend of virtually all other breast cancer books by addressing specifically the breast cancer experience of women of African descent and their lifestyle counterparts in other societies of the world.

Randomized trials have demonstrated the equivalence of preoperative therapy to standard adjuvant therapy with respect to disease-free and overall survival, with a higher proportion of women undergoing conservative surgery instead of a mastectomy. There are multiple questions about the optimal integration of preoperative system therapy into breast cancer management. This comprehensive handbook on preoperative therapy provides guidance to the clinician for patient management, and it outlines important areas of controversy that require further research.

Book excerpt: A look at the recent oncology literature or a search of the common databases reveals a steadily increasing number of nomograms and other prognostic models. These models may predict the risk of relapse, lymphatic spread of a given malignancy, toxicity, survival, etc. Pathology information, gene signatures, and clinical data may all be used to compute the models. This trend reflects increasingly individualized treatment concepts, the need for approaches that achieve a favorable balance between effectiveness and side-effects, and the goal of optimal resource utilization reflecting prognostic knowledge. In order to avoid misuse, it is important to understand the limits and caveats of prognostic and predictive models. This book provides a comprehensive overview of such decision tools for radiation oncology, stratified by disease site, which will enable readers to make informed choices in daily clinical practice and to critically follow the future development of new tools in the field.>

Book excerpt: In the last couple of decades, the study of the cancer genome and the progressive implementation of next-generation sequencing platforms have provided the Scientific and Oncology communities with a multitude of data, technologies, diagnostic, prognostic, and predictive tools that have been revolutionizing the way we can study, diagnose and treat cancer, including breast tumors. For example, genomic tests can now refine the prognosis of early-stage breast cancer patients beyond standard clinicopathological features and help guide escalated or de-escalated treatment choices. The identification of the molecular intrinsic subtypes might also be helpful in guiding treatment choices in advanced hormone receptor-positive disease. The identification of germline mutations in BRCA1 or BRCA2 has led to the development and introduction of PARP inhibitors for the treatment of advanced and early-stage breast cancer, along with personalized follow-up and prophylactic surgical procedures for patients with or without cancer, carrying such mutations.
