Book excerpt: My thoughts about the Hemolytic Uremic Syndrome (HUS) got started in 1961 along with my attempt to return to Argentina. As I sought my way in Buenos Aires, I visited Carlos Gianantonio whom I had met in Caracas the year before during the Pan American pediatric meetings. At that time he was actively working on HUS which had become an epidemic in Buenos Aires and other parts of Argentina. I was impressed by the team effort and devotion of his group to such heavy demands. They obviously were meeting the challenge at an amazingly high level under a very crippling physical situation with shortages of space, laboratories and equipment. His group together with Dr. Becu, at the time the pathologist at the Children's Hospital of Buenos Aires (we had met through his mother who was instrumental in arranging my return to Buenos Aires), wrote some of the classic papers on HUS. Through the years as Dr. Gianantonio became more involved in general pediatrics, the administrative aspects and its orientation in Latin America, he became known for his deep philosophical questions as to what we are doing and where we are going. His questions have obvious implications regarding an agressive approach to our pediatric nephrology patients.

Book excerpt: Acute kidney injury (AKI) is a frequent clinical syndrome among hospitalized patients, independently associated with both short- and long-term mortality. Previous investigations attempted to identify effective interventions to prevent AKI or promote kidney function recovery in patients with AKI. Most were unsuccessful. Hence, additional studies are required in the field of AKI research. In this Special Issue, we are making a call to action to stimulate researchers and clinicians to submit their studies on AKI conducted in nephrology, internal medicine, critical care, and other disciplines that will provide additional knowledge and skills in the field of AKI research, ultimately to improve patient outcomes.

The topic of these proceedings deals with clinical and experimental aspects of acute renal failure. The Bari Seminars in Nephrology were initiated in 1984 and will be held every two years. It is attended by a large number of international clinical scientists in the disciplines of nephrology and related fields. The next Bari Seminars in Nephrology will take place during April 20-24 in 1988 and the theme of the gathering will be Drugs, Systemic Diseases and the Kidney. Part of the Pittsburgh Critical Care series, Continuous Renal Replacement Therapy provides concise, evidence-based, bedside guidance about this treatment modality, offering quick reference answers to clinicians' questions about treatments and situations encountered in daily practice. Organized into sections on theory, practice, special situations, and organizational issues, this volume provides a complete view of CRRT theory and practice. Tables summarize and highlight key points, and key studies and trials are included in each chapter. The second edition has been updated to include a new chapter on the use of biomarkers to aid in patient selection and timing, extensive revisions on terminology and nomenclature to match current standards, and the most up-to-date information on newly developed CRRT machines.

Available in PDF, EPUB and Kindle. Book excerpt: The last 20 years has seen an enormous increase in our knowledge about the management of patients with terminal renal failure. Despite this, even the most successful dialysis and transplant patients require long term specialist supervision so that renal units will have an incremental work load until the death rate of patients undergoing treatment equals the rate of intake of new patients. Furthermore, innumerable conditions which were once regarded as contraindications to therapy may no longer be seen in this light, so that the number of new patients coming forward for treatment each year is increasing rapidly. Dialysis and transplantation are expensive forms of treatment, in terms of staff, facilities and consumables, and it is therefore inevitable that there will be problems in providing treatment for all who need it. These will be particularly acute in times of economic crisis. This book contains the proceedings of a conference which was set up to explore the difference between the supply and the demand for treatment in the United Kingdom, to compare the situation with that in other countries, to explore possible solutions and possibly assign responsibility for the shortfall and to examine the practical and moral implications of our failure to treat the treatable.

This book was released on 2013-05-14 with total page 218 pages. Available in PDF, EPUB and Kindle. Book excerpt: Associated with both acute kidney injury (AKI) and cardio-renal syndromes (CRS), new biomarkers represent both a popular area of investigation and a new opportunity for advancement of therapy. This book contains the resolutions of the most recent ADQI conferences on biomarkers in AKI (Dublin) and on cardio-renal syndromes (Venice). The first part answers specific questions about new biomarkers and their use and utility in AKI: What are the most suitable candidate molecules and physiologic measures, how solid and evidence based is the discovery phase? How can we incorporate the new biomarkers in the AKI conceptual model describing the evolution from susceptibility to insult, decreased GFR and organ death? Even if we have a positive biomarker pattern and we can identify patients at risk or patients with early or even subclinical AKI, how is this information affecting our clinical behavior and practice? The second part is dedicated to the appraisal of the current knowledge about the pathophysiological mechanisms involved in different forms of CRS: it contains contributions on the state-of-the-art knowledge and practice of CRS, particularly focusing on the pathophysiology of the five subtypes. Acute and chronic mechanisms of damage are explored in depth, with particular attention to the primacy of organ involvement and the subsequent pathways of organ crosstalk.Presenting the most recent research in the field of biomarkers, AKI and CRS, this publication is an important educational tool for advanced investigators and clinical experts, but also for students and fellows.

Available in PDF, EPUB and Kindle. Book excerpt: The kidneys participate in all vital processes of the body to maintain overall homeostasis and health. When kidneys are injured during surgical interventions, metabolic and hemodynamic control is disrupted, leading to dysfunction associated with greater mortality, length of hospital stay and cost. Peri-operative Kidney Injury presents the epidemiology, risk factors, diagnosis, treatment and outcomes associated with kidney injury during the peri-operative period. Concepts and principles of care to prevent kidney complications during surgical procedures are provided to equip health care professionals along with strategies to manage acute kidney injury and associated challenges when they occur. Chapters detail diverse surgical settings, ranging from the more common, such as abdominal, cardiac and vascular surgeries, to the intricately complex, including the use of the left ventricular assist device and organ transplants. This practical and comprehensive text blends the evidence-based standards of care with cutting edge advances in the field, while also providing the reader with a peek into innovations on the horizon.
