Despite its importance in virtually all types of acute severe illness, hypoxaemia is often not well recognized or well managed, more so in settings where resources are limited. Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy. Book excerpt: INTRODUCTION : Le sepsis sévère et choc septique sont associés à une morbi-mortalité importante. Deux études princeps ont démontré l'intérêt d'une prise en charge précoce optimale. L'objectif de ce travail est d'évaluer leur prise en charge médicale et paramédicale aux urgences du CHSA et de les comparer aux recommandations internationales. METHODE : Notre étude prospective, réalisée du 4 novembre 2013 au 31 mai 2014, inclut consécutivement les patients admis aux urgences du centre hospitalier de Maubeuge, présentant les critères de sepsis sévère et choc septique selon la classification de Bone. Les données sont recueillies grâce à un formulaire, divisé en trois parties : l'IOA, l'IDE de salle et le médecin. RESULTATS : 56 patients ont été inclus, d'âge médian 67,3 ans, de sexe ratio 0,93. Les critères d'inclusion les plus fréquemment retrouvés sont : une tachycardie chez 51 patients (91,07%), une hyperthermie chez 46 patients (82,14%) et une hypotension artérielle chez 28 patients (50%). Les 26 patients les plus graves (46,42%), ont été directement orientés au déchocage. Le délai moyen de prise en charge par l'IDE de salle est de 44 minutes. La diurèse, la fréquence respiratoire et la température ne sont monitorées respectivement que chez 14 patients (25%), 37 patients (66,07%) et 42 patients (75%). Les prélèvements des hémocultures et à visée bactériologique sont réalisés chez 52 patients (92,86%). Quarante et un patients (73,21%) ont bénéficié d'un remplissage vasculaire, exclusivement par du sérum salé isotonique. Huit personnes (14,29%) ont bénéficié d'un traitement par vasopresseurs. L'antibiothérapie est débutée dans un délai inférieur à 3 heures chez 24 patients (55,81%). Aucune surveillance de la lactacidémie et de la glycémie veineuse n'a été réalisée. Le taux de mortalité du choc septique est de 42,86%. CONCLUSION : La prise en charge du sepsis sévère et choc septique peut et doit être optimisée. Il serait intéressant de créer un protocole de service à l'image de ceux déjà existant (« alerte thrombolyse », « syndrome coronarien aigu »).

L'objectif principal était l'observation de l'application des bundles, du délai de leurs réalisations. Puis en objectifs secondaires, l'utilisation de la noradrénaline, la surveillance de la diurèse horaire, le taux de mortalité à J28, ainsi que l'impact sur la réalisation des bundles, de l'âge des médecins, de l'orientation initiale de l'infirmière d'accueil et d'orientation (IAO) et du mode d'entrée aux urgences. Un questionnaire a été remis aux urgentistes pour évaluer les connaissances théoriques sur la prise en charge du sepsis grave et ses freins. Résultats : Parmi les cinquante patients inclus consécutivement avec un syndrome septique grave, 34% présentaient un choc septique et 66% un sepsis sévère. Une pneumopathie infectieuse était retrouvée dans 36% des cas, une infection urinaire dans 28% des cas. Dans les trois premières heures, le taux de remplissage vasculaire était de 44.19%, d'hémocultures de 74%, d'antibiothérapie de 50%, de dosage du lactate de 76%. Les quatre mesures réalisées ensemble dans les 3 heures suivant l'entrée, étaient présentes chez 26% des patients. La noradrénaline était instaurée dans 29.41% des cas indiqués et la surveillance de la diurèse horaire chez 38% des patients inclus. Il n'y a pas de différence significative dans la mise en oeuvre des bundles selon l'âge des urgentistes et l'orientation initiale par l'IAO dans les filières de soins. L'administration des antibiotiques est significativement plus rapide (p=0.016) lors d'une prise en charge par le SMUR. Le taux de mortalité à 28 jours est de 46%. Le questionnaire objective des lacunes théoriques. Conclusion : La mise en oeuvre des « bundles » n'est pas optimale. L'amélioration de leur application passe par la réalisation de formation interne, l'apport de nouveaux outils pour apprécier la gravité des patients, la présence d'une équipe dédiée. Les freins mis en évidence semblent être l'importance du flux de patients, le tri à l'accueil, la difficulté diagnostique du SSG.
