Book excerpt: Objectifs : évaluer les pratiques professionnelles concernant la prise en charge des patients présentant un sepsis grave au sein du services des urgences de l'hôpital Pellegrin du CHU de Bordeaux. Patients et méthodes : tous les patients de plus de 15 ans et 3 mois, admis aux urgences adultes de l'hôpital Pellegrin du CHU de Bordeaux entre le 1er janvier 2016 et le 31 décembre 2016 ont été inclus. Les prises en charge diagnostiques et thérapeutiques sont comparées avec les recommandations actuelles de la Surviving Sepsis Campaign 3 dans le cadre d'une évaluation des pratiques professionnelles. Résultats : sur les 54827 admissions aux urgences en 2016, 162 patients ont été inclus. Les hémocultures ont été́ réalisées chez 75,3% des patients, et les prélèvements des sites infectieux suspectés chez 61,7%. 56,8% ont bénéficié d'un remplissage vasculaire. Une antibiothérapie a été prescrite dans 92,5% des cas. 35,2% des patients ont été orientés par l'IOA vers la SAUV. La mortalité est de 23,45%. Les pratiques professionnelles sont en accord avec les recommandations chez 41,2% des patients vis à vis de la réalisation des hémocultures, du remplissage vasculaire, du dosage des lactates, de l'antibiothérapie, ainsi que de l'avis d'un réanimateur. Conclusion : la prise en charge des sepsis graves est un enjeu majeur de santé publique et il est nécessaire d'améliorer la prise en charge diagnostique et thérapeutique dès l'accueil aux urgences. Les axes d'amélioration sont la formation des IOA à la détection des sepsis avec l'utilisation systématique du score qSOFA à l'admission chez tous les patients suspects d'infection, la mise à jour des connaissances des équipes médicales à travers un protocole et une procédure d'autoformation par e-learning. L'impact de cette intervention sera évalué grâce à un questionnaire informatisé spécifique au sepsis.
