L'une de ses formes cliniques est la résorption radiculaire interne. Cette atteinte intéressant les parois canalaires est difficile à diagnostiquer car elle évolue à bas bruit. La thérapeutique de référence est le traitement endodontique mais la présence d'une résorption radiculaire interne crée de nombreuses difficultés opératoires. La prise en charge thérapeutique n'est pas aisée et requiert bien souvent un matériel adapté spécifique. Afin de l'optimiser, de nouveaux moyens thérapeutiques tels que la potentialisation ultrasonique de l'irrigation ainsi que l'obturation à l'aide de biocéramiques ont été développés. Après avoir énoncé les principes fondamentaux des résorptions radiculaires internes, l'objectif de ce travail était de décrire l'ensemble des options thérapeutiques disponibles et d'en illustrer l'efficacité au travers de la prise en charge d'un patient atteint d'une résorption radiculaire interne.

Book excerpt: La résorption radiculaire interne, définie comme la destruction progressive de la dentine intra-radiculaire des parois du canal radiculaire à la suite de l'activité des cellules clastiques, est un phénomène rare. L'origine de ces résorptions reste le plus souvent due à un phénomène inflammatoire, induit généralement par des traumatismes dentaires. La stratégie de prise en charge est donc principalement centrée sur l'élimination de cette inflammation à la suite d'un diagnostic précis. La prise en charge endodontique non chirurgicale peut représenter un défi thérapeutique pour le praticien. L'élimination totale de la pulpe est complexe par la difficulté d'accessibilité instrumentale de la totalité du réseau canalaire modifié par la résorption, sur un organe dentaire déjà fragilisé mécaniquement. L'objectif de cette thèse est de proposer un protocole de prise en charge idéal des résorptions internes par la lecture de différentes études réalisées au cours de ces dix dernières années.

Elle peut être coronaire ou radiculaire ; transitoire ou évolutive, allant parfois jusqu'à la perforation. Cette hétérogénéité des formes cliniques complique l'abord de la stratégie thérapeutique. L'étiopathogénie de ces lésions est complexe. Plusieurs facteurs étiologiques concomitants peuvent entrer en cause.Les dents atteintes de résorptions internes sont le plus souvent asymptomatiques, et les signes cliniques sont discrets, voire inexistants. Leur découverte est habituellement fortuite, lors d'un examen radiographique de routine. Une prise en charge trop tardive aura pour conséquence la perte de l'organe dentaire. L'attitude clinique diffère selon la localisation et l'étendue de la résorption ; la finalité escomptée étant toujours l'élimination complète du tissu inflammatoire, ainsi que l'obturation étanche et tridimensionnelle de la lacune. Les approches chirurgicale ou endodontique pourront être envisagées, en fonction de la présence ou de l'absence de perforation. L'évolution des techniques d'instrumentation, d'imagerie - en trois dimensions notamment - des aides optiques et des biomatériaux permettent un diagnostic et une prise en charge optimisés de ces cas, corrélés à une diminution du risque de récidive. Les indications d'avulsion sont de ce fait constamment repoussées.
