Book excerpt: INTRODUCTION : Avec une prévalence estimée à 1%, la schizophrénie est la psychose chronique la plus fréquente. Son apparition précoce et les troubles qu'elle engendre en font une maladie coûteuse pour la société. Elle est associée à des comorbidités somatiques importantes, expliquant en grande partie la diminution de l’espérance de vie des patients schizophrènes. Les soins en ambulatoire se sont développés pour ces patients dans les dernières années, et les médecins généralistes ont été amenés à les prendre en charge. La baisse programmée du nombre de psychiatres dans les prochaines années laisse présager une prise en charge de plus en plus fréquente de ces patients par les généralistes. Notre objectif était d’étudier les modalités de prise en charge de la schizophrénie par les médecins généralistes français, ainsi que la perception de leur rôle et compétence dans cette prise en charge. METHODES : après étude de la littérature, nous avons élaboré un court questionnaire en ligne. Les médecins ont été recrutés par mail ou par téléphone. RESULTATS : Au total, 266 généralistes ont répondu à notre questionnaire. La répartition du nombre de patients schizophrènes suivis par an était hétérogène : 14% n’en suivaient aucun, 36% en suivaient 1 ou 2 et 50% 3 ou plus. 20% des généralistes interrogés avaient des patients qu’ils suivaient seuls. Environ 95% des généralistes considéraient que c’était à eux de s’occuper de la santé physique des patients schizophrènes et se sentaient compétents dans ce rôle. Les généralistes estimaient être dans leur rôle pour le soutien et l’information de l’entourage. En revanche, ils estimaient que la prise en charge de la santé mentale (gestion des crises, suivi, reconduction du traitement antipsychotique) de ces patients incombait plutôt aux services de psychiatrie. Ce résultat était fortement corrélé à une compétence qu’ils jugeaient faible en la matière. CONCLUSION : Les rôles des généralistes et des psychiatres dans la prise en charge de la schizophrénie sont intriqués compte tenu de l'interaction importante entre psychique et somatique, de l’importance de leur entourage dans la vie de certains patients, mais aussi du refus des soins psychiatriques par beaucoup d’entre eux. Il parait une nouvelle fois essentiel de développer la collaboration entre ces différents acteurs.

Il existe une réticence d'une partie des médecins généralistes (MG) à prendre en charge ces patients, et une difficulté des patients à adhérer aux soins somatiques. Objectif Obtenir un consensus sur les difficultés prioritaires rencontrées par les médecins généralistes dans la pratique du suivi des patients schizophrènes. Méthode Une étude Delphi a été menée auprès de 29 médecins généralistes en France. Les problématiques ont été obtenues à partir d'une revue de la littérature. Au cours des trois tours, les participants ont évalué chaque problématique en fonction de sa pertinence dans leur pratique. Résultats Au terme des trois tours, 9 propositions ont atteint un consensus. Il s'agit de : «Ges-tion des addictions », «Accès à un avis psychiatrique diagnostique et thérapeutique, adressage pour suivi conjoint», «Informations sur l'hospitalisation: entrée, courrier de sortie, projet de suivi», «Contact avec le psychiatre référent : connaissance du psychiatre référent, communi-cation au cours du suivi »,« Prévention somatique »,« Surveillance somatique des traitements antipsychotiques »,« Gestion des urgences psychiatriques »,« Prise en charge sociale »et« Observance médicamenteuse ». Conclusion Cette étude nous a permis d'identifier les difficultés auxquelles sont confrontés les médecins généralistes dans leur suivi des patients schizophrènes. Les neuf problématiques identifiées serviront de support à la recherche de solutions à l'aide d'un groupe nominal pour l'élaboration d'un guide pratique.

Méthode : Un groupe nominal en deux séances réunissant 9 à 10 professionnels (MGs, psychiatres, infirmière, travailleuses sociales) expérimentés dans la prise en charge de patients schizophrènes a été organisé afin de trouver des solutions aux problématiques. Six vignettes cliniques illustraient les neuf problématiques. Les solutions proposées aux problématiques ont été exposées, discutées puis votées par ordre d’importance. Résultats : Cinq solutions ont été retenues par problématique. Les autres propositions et les commentaires des experts sont des résultats secondaires. D’une manière générale, les experts attachent beaucoup d’importance aux relations, médecin-malade et interprofessionnelles (travail en réseau). Ils accordent une grande valeur à la construction d’une alliance thérapeutique solide et préfèrent privilégier le choix du patient pour favoriser son implication dans les soins. Conclusion : Cette étude a permis de proposer des solutions concrètes, suggérées par des experts de terrain, aux problématiques rencontrées par les médecins généralistes dans le suivi des patients schizophrènes. Ces solutions seront évaluées lors d’une étude d’intervention et serviront à l’élaboration d’un guide pratique à destination des médecins généralistes.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.
