Such wounds were frequently multiple and severe, and almost invariably became infected. Surgical experience from previous conflicts was of little value, and it became quickly apparent that early surgical intervention with radical removal of all dead and revitalised tissue was absolutely vital to help reduce the chances of infections, especially the lethal gas gangrene, from developing. War Surgery 1914-18 explains how medical services responded to deal with the casualties. It discusses the evacuation pathway, and explains how facilities, particularly casualty clearing stations, evolved to cope with major, multiple wounds to help reduce mortality. There are chapters dealing with the advances made in anaesthesia, resuscitation and blood transfusion, the pathology and microbiology of wounding and diagnostic radiology. There are also chapters dealing with the development of orthopaedic surgery, both on the Western Front and in the United Kingdom, the treatment of abdominal wounds, chest wounds, wounds of the skull and brain, and the development of plastic and reconstructive surgery for those with terribly mutilating facial wounds. The gradual evolution over the last 20 years has been towards a more precise definition of the limits for occupational exposure. These have been created not only in terms of short-term effects but also more importantly in terms of long-term risks involving such problems as the potential for carcinogenesis and genetic change. In the United States the National Committee for Radiation Protection has recom mended that 5 rems (50 mSv) should remain as the maximum permissible dose equiva lent for total body exposure. This would represent the sum of internal and external ex posure and should be regarded as the upper limit allowed. The community of radiation users is required to conduct its operations in such a man ner that the absolute value of the individual's dose equivalent in rems does not exceed his age in years. There should be additional limits for tissues and organs based on short term effects. Therefore, individual organs are limited to dose equivalents low enough to ensure that the dose threshold values are not exceeded.

Book excerpt: Although the number of published papers on traumatic hip disloca tion in childhood is large, our actual current knowledge on this inju ry in this age group is still full of obscure points and unsolved prob lems. Luxation of the hip of traumatic origin in children is infrequent, and the small number of cases reported by authors and institutions, as well as the usually short follow-up in many cases, are responsible for the many doubts and questions, especially related to complications and outcome following the injury. Consequently the treatment, especially the postreduction management, has not been adequately defined. Dr. Antonio Barquet has made the great effort to analyze the ex perience gained from 38 cases in Uruguay and to review more than 500 references related to the subject, covering more than 1400 cases. He began to study this topic in 1976. It was the subject of his postgraduate monograph in 1979, which was followed by a statisti cal analysis of the major complication, i. e., avascular necrosis of the proximal femoral extremity, which was presented as his doctoral thesis in 1981. In addition, he has published numerous papers deal ing with several aspects of traumatic hip dislocation in children.
