Ce travail vise donc à faire un point sur l'état des connaissances et des pratiques en matière de substitution aux opiacés, à connaitre le ressenti des patients vis-à-vis de leur prise en charge à l'officine et celui des pharmaciens concernant leur pratique dans la prise en charge de ces patients ainsi qu'à laisser entrevoir des pistes d'amélioration. Pour mener à bien ce travail, j'ai réalisé deux questionnaires, l'un à destination des patients diffusé via les CSAPA et les officines et l'autre à destination des pharmaciens d'officine diffusé via les grossistes et les réseaux sociaux, sur tout le territoire national durant 6 mois. L'analyse des résultats obtenus a été faite via les programmes Python v3.10.1 et Pingouin statistic library v0.5.1. Concernant les patients, les résultats montrent qu'une grande partie sont suivis en CSAPA et sont pris en charge par de la méthadone principalement sous forme de gélule pour des addictions à l'héroïne, la codéine ou encore la morphine. La quasi-totalité des patients interrogés vont chercher leur traitement systématiquement dans la même pharmacie. D'ailleurs, ces patients évaluent globalement de façon très positive leur prise en charge à l'officine en termes d'accueil, de relations et de communications avec les équipes officinales. En revanche, on observe que leur avis concernant les compétences du pharmacien en matière de sevrage est mitigé. Très peu de patients se sont vu proposer un entretien pharmaceutique de suivi concernant leur sevrage, mais tous avaient des attentes d'écoute, de conseils et d'aide dans la gestion du traitement s'ils se voyaient inclus dans une telle démarche. Concernant les pharmacies, les résultats montrent un profil de pharmacie majoritairement urbaines et rurales, de taille moyenne, avec un environnement médical correct et un centre d'addictologie à proximité. Les patients suivis par ces pharmacies sont principalement traités par de la buprénorphine haut dosage et de la méthadone gélule dans le cadre d'une addiction préalable à l'héroïne. On remarque cependant qu'une majorité des pharmaciens interrogés ignorent pour quelle addiction leurs patients sont suivis. Ces mêmes pharmaciens évaluent les relations et la communication avec les patients plutôt positivement. En revanche, ils évaluent la qualité de la communication avec les autres professionnels de santé de façon mitigée et jugent leur formation en addictologie insuffisante, quand bien même les résultats montrent qu'une bonne formation universitaire et une bonne communication interprofessionnelle sont corrélées avec de bonnes relations avec les patients. Une majorité des pharmacies interrogées avait mis en place des procédures spécifiques concernant la prise en charge des patients sous TSO. En somme, cette thèse a permis de montrer d'une part que les patients sont globalement satisfaits de leur prise en charge en officine mais attendent de leur pharmacien de plus amples connaissances concernant les thématiques de l'addiction et du sevrage, ainsi qu'un accompagnement dans la gestion de leur traitement. Et autre part, que les pharmaciens entretiennent globalement de bonnes relations avec leurs patients sous TSO, mais déplorent un manque de formation universitaire et de communication interprofessionnelle.
