Book excerpt: Longtemps méconnue de la communauté médicale, la douleur chez le patient psychiatrique est devenue sujet d'intérêt; ceci concerne aussi bien ses aspects diagnostiques que thérapeutiques. Entre janvier et mai 2014, nous avons mené par questionnaire une enquête descriptive transversale auprès de 258 maîtres de stage généralistes et 156 internes en médecine générale de la région Midi-Pyrénées, avec un taux de réponse satisfaisant permettant de dégager un certain nombre d'idées forces. Notre étude est un constat éclairé qui fait apparaître les interrelations fortes entre la douleur chronique et la pathologie psychiatrique, et le fait que cette douleur se manifeste avec des modalités différentes selon la pathologie psychiatrique existante. Les échelles d'évaluation de la douleur chronique sont peu utilisées car peu adaptées au contexte psychiatrique. Les principales difficultés thérapeutiques relevées sont éventuellement la peur du risque de dépendance aux antalgiques opioïdes et les problèmes d'interactions médicamenteuses, en dépit de l'utilisation possible de tous les médicaments, en particulier des psychotropes avec indication douleur. Les thérapeutiques non médicamenteuses sont un recours pour assurer une prise en charge globale. Enfin, une meilleure collaboration pluridisciplinaire entre médecins généralistes, algologues et praticiens en santé mentale apparaît également souhaitable. L'introduction récente de ce thème dans la formation initiale devrait permettre aux médecins d'être plus à l'aise avec les patients psychiatriques souffrant de douleurs chroniques.

Il offre aux professionnels de santé des possibilités de formation et de sensibilisation à la prise en charge de la douleur, ainsi qu’un compagnonnage téléphonique et des réunions de dossiers pluridisciplinaires. Depuis l’obtention de l’agrément à l’évaluation des pratiques professionnelles, en 2007, il leur permet de satisfaire leur démarche qualité au sein du Réseau. Les Rémunérations Spécifiques pour la prise en charge de séances de psychologie et de kinésithérapie sont une aide supplémentaire à cette prise en charge. En évaluant tous les six mois, les douleurs des patients et leur retentissement, il s’inscrit dans une volonté d’amélioration de leur prise en charge, en accord avec les plans gouvernementaux successifs de lutte contre la douleur. L’enquête médico-économique réalisée depuis janvier 2006 par le Réseau LCD-Paris montre un impact de son intervention sur le nombre et la durée des arrêts de travail, ainsi qu’une baisse de la consommation de soins. Cette enquête doit être poursuivie - avec de nouveaux moyens mis en place- et améliorée afin de fournir des données sur une plus large population, et de mieux juger de l’impact d’une prise en charge pluridisciplinaire et coordonnée des patients douloureux chroniques.

Le Plan Douleur 2006-2010 traitant de la prise en charge de la douleur chez les personnes vulnérables a permis de donner une réelle impulsion pour ce sujet. Le Centre Hospitalier Sainte Marie Clermont Ferrand est un centre psychiatrique comprenant environ 350 lits d'hospitalisations complètes, 200 places en hôpitaux de jours, et 75 lits de longs séjours. Avant de conduire toute action d'amélioration de la prise en charge de la douleur somatique, un état des lieux a été effectué par le biais d'une enquête de terrain. Cette dernière s'est déroulée auprès de différents acteurs de santé, dans 13 services différents, sur une période de 3 mois. Quatre grands thèmes ont été sondés : le diagnostic, l'évaluation, le traitement et la conclusion se tournant vers les attentes des personnes interrogées, médecin, infirmier, aides-soignants et kinésithérapeutes. Cette enquête a permis de conforter les constats effectués dans d'autres CH psychiatriques montrant un certain retard sur les modalités de prise en charge des douleurs somatiques par rapport aux centres hospitaliers de type MCO (médecine chirurgie obstétrique). En revanche, elle a permis de souligner les besoins de formation et la volonté du personnel soignant pour s'investir dans ce domaine. Grâce à ce travail le CH Ste Marie s'est vue octroyer une subvention de la Fondation de France pour organiser des formations sur la douleur et donc lancer les projets d'évaluation de la douleur chez les patients atteints de troubles mentaux.
