Aujourd'hui, il est établi que les taux de succès d'un traitement endodontique initial correctement réalisé et du traitement implantaire sont remarquablement similaires. Chacune de ces deux disciplines présentent des indications et des contre-indications qui leur sont propres, se situant dans un plan de traitement qui mettra toujours en avant la préservation, lorsqu'elle est possible, de l'organe dentaire. Cependant, il n'existe aucun consensus permettant de façon certaine d'orienter la prise en charge thérapeutique d'un patient présentant une dent au pronostic réservé vers l'endodontie ou bien vers l'implantologie. Pour prendre sa décision, le chirurgien-dentiste doit s'appuyer sur des facteurs liés à la situation clinique et au patient, mais il est montré également que l'expérience pratique et le jugement clinique du praticien vont influencer sa décision. Il nous a semblé dès lors intéressant de réaliser un questionnaire, adressé aux chirurgiens-dentistes omnipraticiens d'une part et aux spécialistes d'autre part (endodontistes et implantologistes). Le but étant de mettre en lumière la variabilité inter-praticiens qui existe dans la prise de décision, face à un cas clinique jugé difficile, présentant une dent traitée endodontiquement avec persistance d'une lésion apicale. Les données ont été collectées et analysées, et les résultats discutés dans une seconde partie.

L’étude des pronostics des deux types de retraitement aboutit à la conclusion que leurs taux de succès sont semblables à long terme.De nombreux auteurs ont établi la liste des indications et contre-indications des deux types de retraitement laissant finalement le praticien devant un choix thérapeutique compliqué. Ainsi, des études réalisées à l’étranger ont montré l’existence d’une importante disparité concernant la prise de décision thérapeutique. L’étude présentée ici a pour but d’établir un état des lieux de la situation actuelle en France, en étudiant la réaction de praticiens à partir de cas simulés. Celle-ci confirme la présence d’une variabilité inter-praticiens et que la spécialité et l’expérience principalement influencent le praticien confronté à un cas de retraitement endodontique.
