Book excerpt: It is common knowledge that healthcare professionals endure a great deal of stress when caring for their patients. Everyday nurses encounter overwhelming demands from their patients. They are overworked and sometimes feel unappreciated causing them to experience burnout. Health care providers who experience burnout begin to feel gradually overwhelmed by the patient's demands, high amount of work load and/ or feeling vulnerable due a non-supportive work environment and management. In addition, the individual may experience something even more debilitating such as Compassion fatigue (CF). The symptoms of CF are similar to burnout, however CF occurs in a sudden and acute onset instead of gradual. Healthcare providers such as nurses are vulnerable to CF. Psychiatric Mental Health nurses encounter and care for patients who are victims of trauma and/ or caring for individuals with complex mental health needs. It takes a great deal of compassion, empathy and emotional investment in caring for patients who are suffering and are in distress. However, nurses are merely human and have limits in being genuinely compassionate and empathetic when caring for their patients. These nurses are likely to become overwhelmed, provide ineffective care and lose their ability to nurture patients putting them at risk of developing CF. There are many studies on the prevalence of CF within various specialties in nursing such as critical care and emergency care; However, there have not been any studies done on the prevalence of CF among psychiatric mental health in the acute care setting.

Book excerpt: Compassion fatigue is a term used to describe the unique stressors affecting people in care-giving professions (Joinson in 1992). For nurses, the impact of compassion fatigue may result in stress-related symptoms, job dissatisfaction, decreased productivity, patient satisfaction, safety issues, and job turnover. The purposes of this non-experimental descriptive survey using electronic distribution to collect data were to identify the prevalence of compassion fatigue composed of burnout and secondary traumatic stress and compassion satisfaction among nurses and nursing staff who deliver direct patient care at the hospital and the relationship between demographic variables (age, work category, level of education, work experience) and the presence of compassion fatigue, burnout, secondary traumatic stress, and compassion satisfaction on selected nursing care units at a Midwest pediatric hospital. Swanson's (1991) Caring Theory was used to guide this study. Compassion fatigue was normally distributed throughout the sample. Results demonstrated that nurses who work on the cardiology unit had higher compassion satisfaction scores than nurses who worked in the pediatric intensive care unit, cardiothoracic care unit and the emergency unit. Nurses working in the pediatric intensive care unit had higher secondary traumatic stress scores than nurses who work in the emergency unit and cardiology unit. Additionally, nurses who worked in the pediatric intensive care unit reflected lower compassion satisfaction scores as well as higher burn out and secondary traumatic stress scores than nurses in all of the other nursing care units studied. Compassion satisfaction, burn-out and secondary traumatic stress scores did not statistically differ by age, work category, level of education, or work experience. The results of this study demonstrate the existence of compassion fatigue at a Midwest pediatric hospital. This study provided data necessary to develop a program addressing compassion fatigue at this hospital. Results also suggest that nurses who work in the pediatric intensive care unit must be a priority group targeted for immediate intervention.

Repeated exposure to children's pain and suffering can lead to compassion fatigue. Scant literature describes either (a) the prevalence of compassion fatigue among pediatric nurses or (b) the personal, experiential, or work factors contributing to compassion fatigue. The purposes of this study were three-fold: first, to examine the prevalence of compassion fatigue among pediatric nurses in free-standing pediatric hospitals; second, to explore personal, experiential, and work characteristics as potential antecedents to compassion fatigue; and third, to determine whether or not these antecedents can help identify nurses with compassion fatigue or proclivity for compassion fatigue. A descriptive correlational research design was utilized. Established self-report survey instruments were used to gather information from nurses working at least fifty percent of their time in clinical care. The Professional Quality of Life Inventory was utilized for the variables of compassion fatigue, secondary traumatic stress, burnout, and compassion satisfaction. The Differentiation of Self and Role Inventory was utilized to assess self/role differentiation. The response rate was 16% (n=463). Compassion fatigue was present in 11% of the total sample. Often the healthcare workers will put their patients’ needs before their own. After an extended period of neglect nurses and healthcare workers often feel some burnout (BO) level, which leads to compassion fatigue. Healthcare workers cannot take care of their own physical and emotional needs, but they cannot be there for their patients like they should be able to provide optimum care. This can lead to medical errors, slower recovery time for patients and eventually nurses leaving the profession altogether due to BO. The purpose of this grant proposal is to examine if there is a relationship between different health promotion behaviors that California State University San Marcos (CSUSM) advanced practice nursing (APN) students may engage in and if there any long term effect on the level of compassion fatigue or BO that nurses experience as well as their varying levels of compassion satisfaction. The study’s dependent variable will be the CSUSM APN students’ responses on the self-reported CS and CF scores that will be measured by the Professional Quality of Life Survey version 5 (ProQOL). The relationship between the ProQOL scores and health promotion behaviors will then be evaluated to see if there are any patterns of significance. The data will be collected over a One month period of time. This study’s significance is to aid in the level of knowledge that we have about CF, BO, and its relationship to CS, improving patient care and retention of employees.

Book excerpt: Burnout is a construct first proposed by Freudenberger to describe a condition that develops in people who work with people such as teachers, nurses and first responders (Shan, 2005). Professionals who are burned out demonstrate a lack of concern for the people they service and often perform poorly on the job. In recent years, the construct of Burnout has been largely replaced by the construct of Compassion Fatigue. Interestingly, mental health professionals who work with children as their predominant client base have not been extensively studied, although these professionals arguably could be experiencing some of the highest levels of Compassion Fatigue (Eastwood, 2008). Research shows that Burnout and Compassion Fatigue impact the work that an individual performs, but the impact of these on the use of particular therapeutic practices as an area is less studied. The present study sought to determine if there is a relationship between Burnout, Compassion Fatigue, and the use of evidence-based practices in mental health professionals. Using research by Craig and Sprang (2010) as the starting point, this study also investigated whether mental health professionals who work with children experience more or less Burnout and Compassion Fatigue than those who work with adults. Mental health professionals from a variety of mental health facilities in the Philadelphia region served as subjects in this study. Participants were given the Trauma Practices Questionnaire (TPQ), a 22-item treatment practices utilization scale, the Professional Quality of Life Scale-V (PRoQOL-V), a 30-item scale that required respondents to assess their thoughts and feelings in relation to their work, and a questionnaire formulated by this investigator that included demographic information, background information, as well a question that focused on the age of the mental health professional's client base and a question that focused on years of professional experience. The results showed that the mental health professionals who worked with adults were older, more typically white, had higher levels of education and had undergraduate majors that were in psychology or a psychology-related area (e.g., social work) than mental health professionals who worked with children. Burnout and Compassion Fatigue correlated negatively with all practices, evidence based and non-evidence based. Males used more evidence-based and total practices than females. Females had a higher level of Compassion Satisfaction but also a higher level of Burnout. The results showed that the mental health professionals who worked with children had higher levels of Compassion Satisfaction, but also higher levels of Burnout. The data show that the group that works with children used less cognitive and behavioral approaches. The broader implications of the results are discussed in the conclusion.
