Book excerpt: Introduction : chez la personne âgée, les prescriptions inappropriées (PI) d'aspirine sont très fréquentes. Ce traitement est pourtant pourvoyeur d'accidents iatrogènes et d'admissions aux urgences. L'objectif est d'évaluer la pertinence des prescriptions d'aspirine chez le sujet âgé hospitalisé en court séjour gériatrique. Méthodes : une étude prospective multicentrique a été menée entre mars 2017 et avril 2018. Tous les patients âgés d'au moins 75 ans ayant une prescription d'aspirine ont été inclus. Les prescriptions ont été réévaluées au regard des données médicales, des critères STOPP/START v2 et des recommandations de bonnes pratiques des autorités de santé ainsi que des sociétés savantes. Résultats : 500 patients d'un âge moyen de 87 ans ont été inclus dans l'étude. Une indication théorique à un traitement par aspirine a été retrouvée dans 77% des cas. Après réévaluation, 157 PI (31%) ont été interceptées. 117 prescriptions (23%) ont été arrêtées du fait d'une prescription d'aspirine en prévention primaire (71%) et d'une prescription d'aspirine pour la prise en charge d'une fibrillation atriale (25%). Une modification de la posologie a été effectuée pour 40 prescriptions (8%). Conclusion : cette étude souligne la forte prévalence des PI d'aspirine chez le sujet âgé. La révision pluriprofessionnelle des ordonnances est une solution primordiale, simple et efficace pour détecter les prescriptions inappropriées et limiter les évènements indésirables liés aux médicaments. Promouvoir des outils d'aide à l'optimisation des aussi bien en milieu hospitalier, médico-social ou ambulatoire est nécessaire pour améliorer les pratiques de prescriptions.
