In 2006, expenditures for prescription drugs among the entire U.S. civilian noninstitutionalized population accounted for 21.6 percent of total health care expenditures. In 1996, prescription drug expenditures accounted for only 11.9 percent of total health care expenditures. While prescription drug expenditures are a significant portion of total health care expenditures, the percentage of individuals that have them, as well as the average expenditure amount and the proportion paid by each source can vary considerably across states. Using data from the Household Component of the Medical Expenditure Panel Survey (MEPS-HC), this Statistical Brief presents estimates of prescription drug expenditures for persons under age 65 in the 10 largest states for the year 2006 and compares these to the national average. The focus is on persons under age 65 to limit the effect of age differences between the state populations on their prescription drug use differences. Only those estimates with statistically significant differences from the national average using a multiple comparison procedure at the .05 level are noted in the text.

By 2005, working age persons generally had prescription coverage available through private insurance, but for the most part persons age 65 and over did not have prescription drug coverage available through traditional Medicare. However, persons age 65 and over may have had prescription drug coverage available through optional private health insurance from present or past employers, individually purchased private supplemental plans, optional Medicare Advantage plans, or Medicaid for those eligible for both Medicaid and Medicare. With the phasing in of Medicare Part D (an optional prescription drug coverage plan for Medicare beneficiaries for an additional premium charge) in 2006, average out-of-pocket costs per prescription drug purchase for persons age 65 and over were expected to drop. This Statistical Brief analyzes average out-of-pocket costs (excluding premiums) for adults age 65 and over in 2005 and 2006 for the top five prescription drugs purchased in 2005 ranked by total expenditures. Similar data for these same top prescription drugs for adults age 65 and over are provided for those ages 45-64 with any private insurance for comparative purposes. The estimates are derived from the Household Component of the Medical Expenditure Panel Survey (MEPSHC). Estimates for 2005 were adjusted to 2006 dollars based on the Gross Domestic Product (GDP) Price Index, National Income and Product Accounts, Bureau of Economic Analysis (http://www.bea.gov/national/nipaweb/index.asp). Unless otherwise indicated, all differences discussed in the text are statistically significant at the .05 level or better.

Book excerpt: In 2007, prescription drugs accounted for 20.7 percent of the overall health care expenditures among the U.S. civilian noninstitutionalized population. 61.7 percent of the population had a prescription drug expenditure, and the average annual expense amount for them was $1,251. Prescription drug expenditures are distinguished from overall health care expenditures in the distribution by sources of payment. A higher percentage of the prescription drug expenditures was paid out of pocket (27.7 percent for drugs versus 16.2 percent overall) and a lower percentage was paid by private insurance (35.5 percent versus 42.4 percent) in 2007. Prescription drug and overall expenditures were similar regarding the percentage paid by Medicare (24.7 percent versus 24.8 percent) and Medicaid (7.8 percent versus 8.7 percent) in 2007. Many factors can influence health care expenses in a particular state, including the demographic, socioeconomic, and health status characteristics of the population. Further, the prevalence and types of health insurance coverage in a state can impact access to care, the level of expenditures, and the extent to which different sources finance health care services. This Statistical Brief presents estimates based on data from the Household Component of the Medical Expenditure Panel Survey (MEPS-HC) on the use, expenditures, and sources of payments for prescription drug expenditures for persons in the U.S. civilian noninstitutionalized population in 2007 for the 10 largest states, and compares the results to the national average in 2007. GAO found that: (1) more than 75 percent of persons older than 65 in the United States use prescription drugs; (2) 90 percent of the elderly who are chronically ill use prescription drugs; (3) persons 65 and older use 30 percent of all the prescription drugs used in the United States, at a rate approximately three times that of the remainder of the population; (4) for three out of four elderly persons, prescription drugs are the largest out-of-pocket health care expense; (5) drug expenditures for persons 65 and older are estimated at $9 billion annually, $7.3 billion of which is out-of-pocket; (6) from January 1980 through 1986, the cost of prescription drugs rose about 80 percent, or 250 percent faster than the rise in consumer prices in general; and (7) 15.5 percent of the elderly patients who require prescriptions are reportedly unable to pay for them. GAO also found that: (1) Medicare generally covers inpatient drugs, but pays for outpatient drugs in only a few instances; (2) 22 of the 48 states with a prescription drug program under Medicaid charge recipient copayments, which range from $0.50 to $3.00; (3) only nine states offer specific programs covering some drug benefits for eligible residents; and (4) Congress has proposed legislation which would reduce out-of-pocket expenditures for Medicare beneficiaries and require states to extend Medicaid coverage to elderly persons who are below the federal poverty level.

At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development. It makes recommendations for policy actions that could address drug price trends, improve patient access to affordable and effective treatments, and encourage innovations that address significant needs in health care.

Medicare is a health insurance program for people age 65 or older, people under age 65 with certain disabilities, and people of all ages with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a kidney transplant). Medicare Prescription Drug Coverage or Medicare Part D is a program that started January 1, 2006, which offered and made available Medicare Prescription Drug Coverage to everyone with Medicare. Medicare beneficiaries were able to enroll in the drug plan of their choice. For the years 2005 and 2006, this Brief presents the number of persons who reported having Medicare as well as total outpatient prescribed drug expenditures for the total population and the Medicare population. The Brief also compares the proportion of the Medicare population that reported at least one drug purchase and the proportion that had at least one Medicare payment for a prescribed drug. Moreover, the Brief compares the proportion of 2005 and 2006 total prescribed drug expenditures by source of payment for the Medicare population, and presents the average Medicare expenditure for prescribed drugs for the Medicare population for 2005 and 2006. All expenditure estimates are in nominal dollars. The estimates in this Brief are derived from the Household Component of the 2005 and 2006 Medical Expenditure Panel Survey (MEPS-HC). Only prescribed medicines purchases in an outpatient setting are included in the estimates. Insulin and diabetic supplies and equipment are included in MEPS prescribed medicines estimates. Over-the-counter medicines are excluded from these estimates as are prescription medicines administered in an inpatient setting or in a clinic or physician's office. All differences discussed in the text are statistically significant at the 0.05 percent level.

Part D coverage may increase access to medicines. This study evaluated patterns of Part D uptake and costs and assessed the effects of coverage on hospitalizations and emergency department (ED) use among people with colorectal cancer (CRC). We analyzed Surveillance, Epidemiology, and End Results (SEER)–Medicare linked data on fee-for-service (FFS) Medicare beneficiaries with at least 36 months of follow-up who were diagnosed with CRC at any point from January 2007 through December 2010, and a matched cohort of beneficiaries without cancer. Dual (Medicare/Medicaid) enrollees were excluded because they are automatically enrolled in Part D. Among beneficiaries with CRC (n = 12,774), 39 percent had complete Part D coverage, defined as coverage in the diagnosis year and 2 subsequent years; the rate was 38 percent in the matched comparison cohort (P = .119). Among those with complete Part D coverage, there was no significant difference in annual prescription drug costs between people with CRC ($3,157, 95% confidence interval [CI]: $3,098–$3,216) and without ($3,113, 95% CI: $3,054–$3,172). Among people with CRC, odds of ED use ranged from unchanged to marginally higher for those with no or partial Part D coverage, (adjusted odds ratio: 1.09, 95% CI: 1.00–1.18), compared with those with complete Part D coverage. Lack of continuous Part D coverage was associated with more ED use among Medicare FFS beneficiaries with CRC in 2007–2013. Among people with Part D coverage, prescription drug costs varied little between those with CRC and matched beneficiaries without cancer.
