L'objectif de notre étude est d'explorer la gestion de la diversification par le médecin généraliste et recueillir son avis sur l'utilisation d'une fiche-conseil. Matériel et méthode : une étude qualitative a été réalisée, basée sur 15 entretiens semi-dirigés de généralistes installés en région PACA. Nous avons relevé les difficultés rencontrées avec la diversification, les solutions proposées et leur avis sur une fiche. Nous sommes allés jusqu'à "saturation des données". Les données ont été étudiées par analyse de contenu thématique à l'aide du logiciel NVivo. Résultats : dans la diversification alimentaire, les médecins estiment qu'ils doivent surtout surveiller la croissance, rassurer les parents et donner des conseils personnalisés mais généraux. Les difficultés rencontrées concernent la diversité des recommandations, la gestion de l'angoisse parentale et le manque de temps. Ils proposent de publier des recommandations claires, de renforcer l'éducation à la santé et de s'appuyer sur un support écrit. La fiche-conseil leur apparait comme une source pertinente d'informations, de réassurance, permettant d'éviter les oublis, et d'optimiser le temps des consultations. Cependant leur bénéfice reste limité sans accompagnement. Discussion : la gestion de la diversification varie selon les médecins. Les attentes parentales et leur perception par le généraliste se rejoignent sur les grandes lignes mais s'opposent sur certains points. Les nombreuses variations des recommandations ces dernières années compliquent parfois leur application. Conclusion : le généraliste met la relation de confiance et la surveillance de la croissance de l'enfant au cœur de la gestion de la diversification. Sa prise en charge consiste à articuler la norme des recommandations avec la singularité du couple parents/enfant. La fiche-conseil comme outil d'aide au dialogue apparait comme un intermédiaire adapté pour atteindre cet objectif.

Les pratiques ne sont pas trop éloignées des recommandations mais il existe un manque de conviction dans les conseils donnés par les médecins. Pour encourager le report de la diversification à 6 mois, il faut mettre au point un référentiel unique et le diffuser à tous les professionnels de la petite enfance et aux parents.

L'objectif principal de cette étude était de faire l'état des lieux de la pratique de la diversification alimentaire des nourrissons par les médecins généralistes de Guadeloupe et de les comparer aux données de la littérature. L'objectif secondaire est de réaliser une fiche résumant les conseils à donner aux parents de que les médecins pourront remettre lors de la visite du 3ème mois. Méthode: Il s'agit d'une étude observationnelle descriptive transversale réalisée entre Avril et Août 2016 à l'aide d'un questionnaire envoyé par mail aux médecins généralistes installés et non installés exerçant en Guadeloupe. Résultats: 269 médecins inclus. Nous avons obtenu 73 réponses (taux de réponse 27,1%). Seuls 54! ,8% des médecins abordent ce sujet de manière systématique. Mais bien souvent de manière tardive. Les nouvelles recommandations concernant l'âge de début de la diversification alimentaire et la conduite à tenir en cas de risque allergique sont mal connues par les médecins généralistes. Discussion : dans d'autres pays les recommandations sont plus claires et plus faciles d'accès. Un effort national est nécessaire. Des messages spécifiques à l'exercice en Guadeloupe doivent être abordés par les médecins. D'autres études sont nécessaires afin d'estimer l'impact d'une information sur la diversification alimentaire plus ciblée et faisant la part belle à l'individualité des interlocuteurs, sur les comportements alimentaire à l'âge adulte et par conséquent sur les facteurs de risques cardiovasculaires.

Book excerpt: Introduction : le nourrisson est un enfant âgé de 1 mois à 2 ans. La diversification alimentaire est l'introduction d'aliments autres que le lait. En France, la méthode la plus utilisée est la diversification classique, par des aliments mixés administrés à la cuillère par un adulte. En parallèle, une autre approche est pratiquée par certaines familles : la DME ; alors même que les recommandations françaises à son sujet sont pauvres. L'objectif était donc d'étudier les connaissances et pratiques des médecins exerçant la médecine générale en ce qui concerne la DME. Matériel et Méthodes : l'étude quantitative a consisté en un recueil de données par un questionnaire en ligne diffusé entre juin et octobre 2022. Il s'adressait aux médecins exerçant la médecine générale dans l'ex-région Midi-Pyrénées, ayant fini leurs stages d'internat, thésés ou pas encore. Des analyses descriptives et univariées des données ont été réalisées. Résultats : 317 médecins ont répondu au questionnaire. Parmi eux, on retrouve plus de femmes de moins de 40 ans et de parents. En outre, dans mon échantillon, 71,9% des participants ont déjà entendu parler de la DME et parmi ces connaisseurs, 87,4% sont de bons connaisseurs. Par ailleurs, leur avis est majoritairement mitigé par rapport à la DME, voire ils n'en ont pas. De plus, une minorité de ces connaisseurs pratique personnellement et professionnellement la DME. Très peu instaurent un suivi spécifique pour les nourrissons diversifiés de la sorte. Discussion et Conclusion : les caractéristiques démographiques de cet échantillon peuvent être expliquées par l'histoire récente de la DME. Son manque de connaissances détaillées et d'accompagnement des familles lui, est probablement lié au défaut de résultats scientifiques rassurants de bon niveau de preuve à ce sujet. Des notions doivent être fournies aux médecins généralistes en formation initiale et continue afin qu'ils puissent informer et accompagner les familles souhaitant malgré tout se lancer dans cette aventure.

Motivated by an impulse that, as he himself says, ‘was civic rather than political’, nothing seemed more important to him than to understand the Algerian situation and provide the elements that would enable others to come to an informed judgement about it. In extremely tough conditions and along with a small group of students, Bourdieu undertook a series of studies across an Algeria that was tightly patrolled by the army, leading him to discover the shocking reality of the resettlement camps and to analyse the mechanisms of destruction of Algerian society of which they were emblematic. To achieve the objectives he had set himself, Bourdieu had to carry out a genuine intellectual conversion, acquiring an ethnographic understanding of Algerian society, learning sociological analysis at a breakneck pace and inventing new instruments - both theoretical and empirical - that would enable him to understand the relations of domination specific to colonialism. These new tools also enabled him to analyse the nature of the crisis that the war had both produced and manifested. The Cognitive Behavioural Tsunami: Managerialism, Politics, and the Corruptions of Science provides a powerful critique of CBT’s understanding of human suffering, as well as the apparent scientific basis underlying it. The book argues that CBT psychology has fetishized measurement to such a degree that it has come to believe that only the countable counts. It suggests that the so-called science of CBT is not just "bad science" but "corrupt science". The rise of CBT has been fostered by neoliberalism and the phenomenon of New Public Management. The book not only critiques the science, psychology and philosophy of CBT, but also challenges the managerialist mentality and its hyper-rational understanding of "efficiency", both of which are commonplace in organizational life today. The book suggests that these are perverse forms of thought, which have been institutionalised by NICE and IAPT and used by them to generate narratives of CBT’s prowess. It claims that CBT is an exercise in symptom reduction which vastly exaggerates the degree to which symptoms are reduced, the durability of the improvement, as well as the numbers of people it helps. Arguing that CBT is neither the cure nor the scientific treatment it claims to be, the book also serves as a broader cultural critique of the times we live in; a critique which draws on philosophy and politics, on economics and psychology, on sociology and history, and ultimately, on the idea of science itself. It will be of immense interest to psychotherapists, policymakers and those concerned about the excesses of managerialism.
