Un aidant sur deux considère qu'un soutien dans ce rôle est nécessaire. La Haute Autorité de Santé met le médecin généraliste au coeur de la prise en charge spécifique de l'aidant, dans une approche médico-psycho-sociale. L'objectif est de dresser, au travers du regard des aidants, un état des lieux de la situation palliative à domicile, et d'évaluer la place accordée au médecin généraliste tout au long de cette prise en charge. Méthode : Nous avons mené une étude qualitative auprès d'aidants de patients atteints de cancer au stade palliatif. Onze entretiens semi-dirigés ont ainsi été réalisés, Résultats : La place du milieu hospitalier est prépondérante dans le parcours de soins de l'aidant et de son proche atteint de cancer. L'apport des équipes de soins palliatifs, pluridisciplinaires, est précieux auprès de la dyade. A contrario, le médecin généraliste est peu présent aux yeux de l'aidant. Les limites de ses compétences médicales sont perçues. Le praticien s'investit davantage sur le plan humain, parfois en dépassant le cadre professionnel. Conclusion : ll existe un déséquilibre entre la place de la spécialité palliative et celle du médecin généraliste auprès de l'aidant et son proche. Des pistes d'améliorations sont suggérées, notamment la sensibilisation et la formation des médecins généralistes dans ce domaine. Un outil d'évaluation spécifique du fardeau de t'aidant en contexte carcinologique permettrait une prise en charge plus adaptée à ses attentes.

Matériel et méthode : Il s'agit d'une étude épidémiologique, observationnelle, de cohorte, multicentrique, comparative, avec recueil unique prospectif des données par autoquestionnaire. Cette étude inclura les aidants de patients en soins palliatifs suivis par l'HAD de Grenoble et ceux suivis en « soins de ville » inclus par les oncologues du CHUGA. Le nombre de sujets nécessaires est de 31 dans chaque cohorte. Les scores de qualité de vie de l'échelle CarGOQoL seront comparés entre les deux cohortes. Conclusion : Ce travail a permis l'élaboration d'un protocole de recherche pour comparer la qualité de vie des aidants de patients souffrant d'un cancer en soins palliatifs entre prise en charge en HAD et soins de ville.

Information ranges from explanations of how specific drugs work, through the precautions to take in the presence of certain disorders, to a list of factors that influence the effectiveness of opioids. Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain.
