Une prise en charge adaptée améliore considérablement le temps de cicatrisation de ces plaies et peut donc avoir un impact économique. Après quelques rappels sur la peau, les plaies chroniques et les pansements, nous avons souhaité étudier l'intérêt des pansements à l'argent sur la cicatrisation de ce type de plaies. Nous avons donc analysé les différentes études cliniques comparatives récentes, afin d'essayer de montrer l'éventuelle supériorité des pansements à l'argent en terme d'efficacité et de tolérance, versus les thérapeutiques traditionnelles. Face à la diversité des pansements à l'argent, et également au risque d'évolution défavorable d'une plaie infectée, des fiches conseils ont été rédigées pour chacun d'entre eux afin de guider les critères de choix du soignant et d'aider le pharmacien dans son rôle d'éducateur de la santé.

Malgré cela, et tout en considérant l'existence de germes résistants, l'utilisation de ces pansements sur des plaies en colonisation critique est envisageable. D'après un questionnaire qui leur a été distribué, les pharmaciens d'officine n'ont qu'une vision très partielle de l'utilisation des pansements à l'argent : la sulfadiazine argentique en prévention de l'infection des brûlures est principalement connue. Ils apprécieraient des formations continues sur les pansements et la prise en charge des plaies chroniques, et leur implication dans des réseaux de plaies et cicatrisation est à développer. Ceci leur permettrait de rester plus critiques face aux pansements à l'argent, aussi bien sur prescription qu'en conseil, et d'avoir pleinement un rôle à jouer dans la prise en charge des plaies à l'officine.

Plusieurs facteurs physiopathologiques peuvent perturber ce processus. L'infection des plaies constitue une des principales causes qui conduit à un retard de cicatrisation. Parmi les traitements proposés dans la prise en charge des plaies infectées, l'argent avec ces propriétés antimicrobiennes est une possibilité. Actuellement, de nombreux pansements à base d'argent sont commercialisés mais leur place dans la stratégie thérapeutique des plaies infectées n'est pas démontrée. Quelle est la conduite à tenir en secteur d'hospitalisation pour le référencement des pansements à l'argent ? Une étude menée au sein du CHU de Toulouse conclut au caractère non indispensable des pansements à l'argent dans la prise en charge des plaies infectées chez les patients hospitalisés. Ces résultats sont confortés par l'absence de démonstration scientifique de leur efficacité dans ces indications.
