Book excerpt: Twenty years ago, the enzyme superoxide dismutase which uses the superoxide radical anion as its specific substrate was reported. With this discovery was born a new scientific field, in which oxygen, necessary for aerobi c 1 ife on thi s planet, had to be cons i dered also in terms of its toxicity and stresses. This stimulated the search for knowledge of active oxygen species in biology and medicine. Superoxide and other reactive oxygen species are now implicated in many disease processes. Major advances have been achieved during these past years with respect to free radical generation and mechanisms of free radical action in causing tissue injury. In parallel, the possibil ity of influencing free radical related disease processes by antioxidant treatment was studied in various in vitro and in vivo systems. This was the unique theme of a conference organized in Paris by the Society for Free Radical Research (December 9-10, 1988) which brought together experts from basic sciences and clinicians in order to evaluate the current status of antioxidant therapy. The conference emphasized fundamental processes in antioxidant action. Among the major topics were superoxide dismutase (SOD) and low molecular weight substances with such activity, called SOD mimics. Other antioxidant enzymes were also considered. Antioxidant vitamins, in particular vitamins E and C, other naturally occurri ng antioxidants and vari ous synthet i c antioxidants were included in the presentations as there is now a rapidly developing series of compounds with potentially interesting clinical applications.

Available in PDF, EPUB and Kindle. Book excerpt: clinical efficacy of haemostatic agents had to be published work and had to fulftl most of the following minimum requirements: (1) Quantitation ofthe measured blood loss was required, and not merely a clinical impression of the amount of blood lost, if the document pertained to a planned but «open» clinical trial. (2) Only double-blind trials with random allocation of the placebo and experimental drug to preselected patients were considered suitable for discussion, if the blood loss had not been quantitated in a prospective trial. (3) Defmition and appropriate selection of patients admitted to the trial: all inclusion and exclusion criteria used to select patients had to be mentioned in detail. (4) Once included in the trial, patients could be withdrawn only on the basis of strict criteria for withdrawal which had been defined in advance. (5) A double-blind trial had to be continued for an adequate length of time if the haemostatic agent was being assessed in the prevention of bleeding in patients with a long lasting bleeding disorder . (6) A clear and detailed statistical analysis of the results was required. Moreover, a clear distinction between the therapeutic and prophylactic value of the haemostatic agent had to be made and applied separately to the group of patients without any major basic disorder and those with a bleeding disorder e.g. : chronic thrombocytopenia, haemophilia, Rendu-Osler telangiectasia ... General statements not substantiated by experimental data, even when issued by well-known authorities, were not considered a reasonable basis for discussion.

Trade group estimates suggest that total sales exceeded $4 billion dollars in 1999. Herbai remedies are for sale not just in health food stores, but in supermar kets, drug stores, and even discount warehouses. Along with the proliferation in sales has come a proliferation ofinformation sources. Not all ofthe sources are equally reliable, or even intelligible. Traditional herbalists c1assify thistle and mugwort as "cholagogues," substances used to make the gallbladder con tract and release bile. Medical school graduates are unlikely to have ever heard the term, or even accept the notion that most right-sided abdominal pain is a result of diminished bile flow. Heroin and cocaine may not be the only drugs to come from plants, but a practicing physician or toxicologist might be forgiven for thinking so. In 1998, 1264 papers were published about cocaine and only 17 about kava kava, an abused herb that is not without toxic side effects. Unfortunately, the majority of the papers about kava kava were published in journals not found in ordi nary hospitallibraries. In recognition ofthis fact, and ofthe obvious need for a reliable reference work on herbai toxicology, The Toxicology and Clinical Pharmacology 0/ Herbal Products was an early addition to our new series in Forensie Science and Medicine. It is very badly needed.

Book excerpt: A doctor on the front lines of hospital care illuminates one of the most important and controversial social issues of our time. It is harder to die in this country than ever before. Though the vast majority of Americans would prefer to die at home—which hospice care provides—many of us spend our last days fearful and in pain in a healthcare system ruled by high-tech procedures and a philosophy to “fight disease and illness at all cost.” Dr. Ira Byock, one of the foremost palliative-care physicians in the country, argues that how we die represents a national crisis today. To ensure the best possible elder care, Dr. Byock explains we must not only remake our healthcare system but also move beyond our cultural aversion to thinking about death. The Best Care Possible is a compelling meditation on medicine and ethics told through page-turning life-or-death medical drama. It has the power to lead a new national conversation.

Available in PDF, EPUB and Kindle. Book excerpt: Recent research on skin immunity and the skin microbiome reveals the complexity of the skin and its importance in the development of immunity against arthropod-borne diseases. In diseases such as malaria, borreliosis, leishmaniasis, trypanosomiasis, etc., the skin interface has been shown as an essential site for pathogens to hide from the immune system, and as a potential site of persistence. Only very few vaccines have been successfully developed so far against these diseases, likely because of an insufficient understanding on the development of skin immunity against pathogens. Skin and Arthropod Vectors expands our knowledge on the role of the skin interface during the transmission of arthropod-borne diseases and particularly its immunity. This work may support researchers who strive for developing more efficient diagnostic tools and vaccines. It also gives scientists and advanced students working in related areas a better insight on how humans and animals are attractive to arthropods to develop better repellents, or to set up transgenic arthropods. Offers the only compilation of research focusing on both the skin interface and arthropod vectors, with contributions from international experts Advances research in the effort toward generating more effective diagnostic tools and vaccines focusing on the skin interface Can also serve as supplemental material for dermatology lectures or specialized lectures on medical entomology and skin immunity

Book excerpt: Platelet Activating Factor (PAF) is a naturally occurring substance with potent biological effects which have a broad significant role in the activation of various inflammatory cells. It therefore contributes to a number of serious clinical conditions including anaphylaxis, allergic and inflammatory diseases (eg. asthma, rhinitis, thrombosis). This book brings together leading investigators in the field to present their latest research directed towards the biosynthesis of PAF, its cellular origins, PAF induced cellular and tissue responses, pathophysiological responses and, most important, to finding effective antagonistic drugs to control this inflammatory mediator.
