Book excerpt: The efficiency of carotid surgery on an asymptomatic carotid artery stenosis and its superiority to conservative treatment was clearly demonstrated in the ACAS study. The stroke risk over a five year follow-up period could be reduced by 55 % and the combined stroke and mortality rate was shown to be a mere 2. 3 %. The efficacy of surgical treatment in also reducing stroke rates in the case of a symptomatic carotid stenosis was proven in the NASCET and ECTS prospective randomized studies. Of extreme importance in these procedures is, however, precise quality control and quality assessment. This is presently a topic of tremendous interest in reconstructive vascular surgery and is constantly being discussed in specialist circles and beyond. Documentation and the possibility of accurate reconstruction of the intraoperative situation are in high demand. Perioperative monitoring of neurological function, particularly the monitoring during carotid surgery, are the aspects underlying constant revision and reassessment to ensure quality control and assurance of a negligeable mortality and morbidity rate. The aim of this book is to discuss the presently available perioperative control methods, examinations, and quality assessments, and the critical consideration of these. We would like to express our thanks to all authors who helped achieve the sense and aim of this book. A special word of thanks to Dr. Kerstin Simons (assistant surgeon) for assisting in the editing of the numerous presentations in this book. Particularly we thank Dr.

Continuous EEG is one of the monitoring used to detect cerebral ischemia (1). When this appears, the EEG changes and the hemodynamic variations induced either spontaneously or by the anesthesiologist are not described. We therefore reviewed the peroperative electronic records of our patients anaesthetised for CEA to describe these modifications.Materials and Methods: This is a retrospective single institution study, approved by our IRB, that reviewed the data recorded by our anesthesia information management system (Innovian Anesthesia Drager) in 275 patients who underwent CEA from 01 2012 till 12 2016. All the cases were performed under general anesthesia with propofol and remifentanil controlled by TCI using respectively the Schnider or the Minto model. A continuous EEG using 10 leads placed as described by the.

Available in PDF, EPUB and Kindle. Book excerpt: This easy-to-read reference book contains much previously buried data about the carotid and other extracranial arterial lesions and their treatment. The collected facts deal with the anatomy, pathology and pharmacology of the carotid artery. Diagnosis by clinical, noninvasive and angiographic methods, results of operative and nonoperative treatment in various categories of patients, such as those with prior stroke, acute stroke, stroke in evolution, reversible ischemic neurologic deficit, transient ischemic attacks (TIAs), crescendo TIAs and asymptomatic patients with bruits and/or stenoses, are included. Hard data is presented on early and late results of endarterectomy, the causes of morbidity and mortality, the long-term life expectancy of these patients with and without surgery, by category of disease. It quantifies a surgeon's options with relation to shunting or not shunting during surgery, the use of various monitoring techniques to prevent perioperative stroke, when to consider coronary artery bypass before carotid surgery, and the pros and cons of antiplatelet therapy pre- and post-operatively. Recent controversy on endarterectomy for asymptomatic stenosis is analyzed, including prospective clinical and noninvasive follow-up studies of such lesions. The recent downgrading of the effectiveness of extracranial-intracranial bypass is also discussed. For neurologists, neurosurgeons and vascular surgeons, the text is arranged for easy access and references to source material are always given. Editorial comments by the author are found at the end of each chapter.

Book excerpt: Close monitoring of patients during anesthesia is crucial for ensuring positive treatment outcomes and patient safety. The increasing availability of new technologies and the repurposing of older monitors means more patient data is at anesthesiologists' fingertips than ever before. However, this flood of options can be overwhelming. A practical resource for understanding this array of clinical monitoring options in anesthesia, this important text focuses on real-world applications in anesthesia and perioperative care. Reviewing the evidence for improved patient outcomes for monitoring technology, neurological monitoring, echocardiography systems and ultrasound are amongst the techniques covered in a head-to-toe approach. Statistics used by manufacturers to gain approval for their technology are discussed, as well as the under-appreciated risks associated with monitoring such as digital distraction. Future monitoring technologies including wearable systems are explored in depth. Focusing on applied practice, this book is an essential text for front-line healthcare professionals in anesthesia.

Available in PDF, EPUB and Kindle. Book excerpt: Neurophysiologic intraoperative monitoring (IOM) neurologic monitoring during complex operative procedures is increasingly used to help prevent damage to the nervous system during surgery. Intraoperative Neurophysiology discusses all aspects of IOM with a hands- on approach to this challenging and exciting new frontier. Everything is covered from set-up, monitoring and mapping, troubleshooting, interpretation of results, and medical management. Interweaving contributions from neurologists and surgeons, the book presents a practical integrated blueprint for effective neurophysiological testing in the operating theater. Intraoperative Neurophysiology is visual and comprehensive in scope and coverage. It begins by reviewing basic neurophysiologic and neuroanatomic knowledge and presents detailed technical information on each basic test, providing the foundation necessary for choosing the right test and customizing monitoring and mapping according to the specifics of individual surgical procedures. Intraoperative Neurophysiology utilizes a unique structure to provide insights into successful monitoring practices and techniques. The book uses the steps of each surgical procedure as the skeleton upon which the IOM procedure is built, thereby presenting a developmental step-by-step approach to IOM procedures and the possible complications and pitfalls - that may arise at different moments of the surgery. In addition, it promotes and encourages the use of EEG in the operating room, and offers unprecedented coverage of ECoG, functional mapping, and EEG monitoring. With over 275 illustrations, numerous tables, and the most important clinical points made in writing and exemplified graphically, Intraoperative Neurophysiology: Monitoring and Mapping delivers in words and pictures everything one needs to know to master the art and science of intraoperative neurophysiologic procedure and reduce the operative risk of neurological damage in surgical patients.
