When determining the appropriate educational plan, the educator must take all factors into consideration, including culture. For diabetic patients, the educational plan is the basis of their treatment. Education becomes especially important in the care of the diabetic patient who must provide self-care management in order to maintain health. According to the literature, the educational plan for a diabetic patient requires cultural consideration and to be individualized for the learner. The gaps in the literature indicate that there has been little research done on specifically Hispanic Spanish speakers and gestational diabetes. This study focused on Spanish speaking women with gestational diabetes. The objective of this study was to gain insight into how diabetic education that is neither culturally sensitive, nor individualized, is perceived as effective by the Hispanic gestational diabetic patient. This qualitative, ethnographic study explored the perceptions of 10 Hispanic gestational diabetic patients in the last weeks of their pregnancies. The patients were Spanish speaking only and were interviewed by a native speaker. Thematic analysis revealed perceptions regarding cultural views on diet practices and family support, and how these impact health. The analysis also revealed cultural misperceptions of health and treatment of disease, as well as a perception of feeling restrained. The findings indicated that Hispanic patients perceived the need for an individualized, culturally sensitive education plan for their gestational diabetes care. These findings indicate recommendations for racially/culturally matched providers or nursing navigators in order to facilitate the needs of this high-risk group and, thereby, potentially decrease the risk of adverse perinatal outcomes.

However, data about characteristics and barriers of diabetes education in clinical practice in Thailand is limited.AimTo describe characteristics of diabetes education and identify perceptions of barriers to diabetes education among administrators, doctors and other healthcare professionals working as diabetes educators (DEs) in Thailand.MethodA survey was conducted in 613 hospitals (508 public and 105 private hospitals) across Thailand from December 2016 to March 2017. A self-completion questionnaire was sent to administrators, diabetes clinic doctors, and health professionals working as a DE to collect data about the workload, characteristics of diabetes education from DEs and perceived barriers to diabetes education from 3 groups of respondents.ResultsThe overall response rate was 54.8% (58.3% public and 38% private hospitals). The response rate of administrators, doctors and DEs was 48.3%, 49.9% and 65.7%, respectively. The median (IQR) number of DEs per hospital surveyed was 4 (2,6). The median (IQR) number of patients per educator per week was higher in public hospitals than private hospitals (70 [30,150] vs. 20 [10,30]).The health professionals working as a DE were outpatient nurses (84.1%), nutritionists (59.8%), pharmacists (53.6%), doctors (44.7%), inpatient nurses (34.2%), physical therapists (18.4%), health educators (15.1%), Thai traditional medicine staffs (7.7%) and dentists (3.2%). DEs self-reported the percentage+SD of patients receiving diabetes education in 30, 31-60, 61-120, 121-500 and 500 beds hospitals were 70u00b123, 73u00b121, 62u00b126, 63u00b126 and 46u00b127, respectively. Characteristics of diabetes education in public and private hospitals are presented in table 1. Book excerpt: In the United States (U.S.), Type 2 Diabetes Mellitus (T2DM) has reached epidemic portions with a prevalence of approximately 29.1 million people and is the seventh-leading cause of death (Centers for Disease Control and Prevention [CDC], 2014; Healthy People 2020, n.d.). The U.S. spends an estimated $245 billion in direct and indirect medical costs. The indirect costs include disability and lost productivity (CDC, 2014). Previous studies focusing on diabetes education acknowledge the significance of self-management activities in managing T2DM. Extant studies examining self-management and treatment outcomes for persons with T2DM have paid limited attention to the relationship between patients' disease perceptions, diabetes knowledge, self-efficacy, self-management practices, and glycemic control. The purpose of this investigation was to identify factors and outcomes associated with healthy diabetes self-management practices among a cohort of individuals with T2DM residing in southern California. Nola Pender's Health Promotion Model (Pender et al., 2015) guided the study. Specific aims include: 1) Provide conceptual clarity for the phenomenon of perception through a concept analysis using Walker and Avant (2011) methodology, 2) Identify factors and outcomes associated with diabetes self-management through a systematic review of the literature, and 3) Examine relationships among perceived self-efficacy, specific patient characteristics, diabetic knowledge, self-management practices, and HbA1C values among a sample of individuals with T2DM living in southern California. The research aims provided the structure to generate new scientific data and evidence towards the improvement of self-management practices and those characteristics leading to improved HbA1C outcomes.
