Book excerpt: INTRODUCTION : Peu de données existent dans la littérature sur l'évaluation par les patients de leur prise en charge médicale en soins primaire. L'objectif principal de cette étude est d'évaluer le pourcentage de patients estimant avoir subi un soin inadapté, une erreur de diagnostique (ED) ou thérapeutique (ET) en soins primaires, et l'impact du ressentiment perçu. Les objectifs secondaires : identifier les facteurs associés à la perceptions d'une erreur, connaître les motifs les plus fréquents de changement de médecin traitent (MT). MATERIELS ET METHODES : Il s'agit d'une étude étiologique transversale française réalisée de décembre 2012 à juin 2013 auprès de patients majeurs consultant dans 11 cabinets de médecins généralistes, répondant à un questionnaire anonyme. RESULTATS : 145 patients ont été inclus : 35,9% ont perçu un soin inadapté et/ou une erreur médicale, 24,1% ont peçu une ET (54,5% avec préjudice important voire plus). 17,9% ont alors changé de MT. La pathologie cardiaque chronique et les informations partielle délivrées en consultation sont les seuls facteurs indépendants associés de manière statistiquement significative à la perception d'une erreur (respectivement p=0,05, p=0,01). Le changement de MT à la suite d'une erreur médicale est le 3ème motif de changement après le déménagement du patient, le déménagement et/ou la retraite du MT. CONCLUSION : La perception d'erreur médicale en ambulatoire par les patients est fréquente. L'amélioration de la communication médecin-patient est un élément clé pour réduire les perceptions erronées des patients

Book excerpt: L'apparence physique des médecins peut jouer un rôle important dans l'exercice de leur métier, en particulier pour les jeunes praticiens qui cherchent à établir leur crédibilité auprès des patients. Cette thèse a pour objectif de comprendre la vision du patient sur l'apparence physique de son médecin et son retentissement sur la relation médecin-patient. Cette étude qualitative a été menée avec 13 entretiens semi-dirigés auprès de patients de médecin générale en Ile-de-France. Les résultats montrent que les patients s'attendent à une apparence cohérente avec le métier, mais préfèrent une apparence en santé, neutre, sobre, marquée par une forme d'authenticité. Une apparence extravagante peut nuire à leur empathie envers les patients. L'apparence physique peut influencer la relation à différents niveaux, tels que le ressenti, la confiance, l'observance thérapeutique et le choix du médecin traitant. Cette influence dépend de la compétence du médecin et de sa capacité à communiquer avec le patient, et est conditionnée par le sentiment du patient de se sentir à l'aise dans la relation. Ce travail contribue à une meilleure compréhension de la complexité du rôle de l'apparence physique dans la relation médecin-patient, du point de vue des patients. Les résultats obtenus apportent une perspective différente sur la relation entre le médecin généraliste et ses patients, et soulèvent des questions quant aux perceptions des médecins sur ce sujet.

Devant les enjeux importants de la confiance dans la relation médecin-patient, il nous a semblé important d'explorer la confiance accordée aux médecins remplaçants. L'objectif principal de notre étude était de comparer la confiance du patient entre le médecin traitant et le médecin remplaçant. METHODE : Pour mesurer la confiance nous avons utilisé l'échelle de Wake Forest version française. La comparaison des scores de confiance utilisait un test t de student. Les différentes dimensions de la confiance explorées par l'échelle de WF (la fidélité, la compétence, la confiance globale et l'honnêteté) ont été comparées. Les variables pouvant influencer le résultat ont été étudiées en analyse multivariée par une régression linéaire multiple. RESULTATS : 289 patients ont participé à l'étude. Book excerpt: The Violence of Modernity turns to Charles Baudelaire, one of the most canonical figures of literary modernism, in order to reclaim an aesthetic legacy for ethical inquiry and historical critique. Works of modern literature are commonly theorized as symptomatic responses to the trauma of history. In a climate that tends to privilege crisis over critique, Debarati Sanyal argues that it is urgent to rethink literary experience in terms that recall its contestatory potential. Examining Baudelaire's poems afresh, she shifts the focus of critical attention toward an account of modernism as an active engagement with violence, specifically the violence of history in nineteenth-century France. Sanyal analyzes a literary current that uses the traditional hallmarks of modernism—irony, intertextuality, self-reflexivity, and formalism—to challenge the historical violence of modernity. Baudelaire and the committed ironists writing in his wake teach us how to read and resist the violence of history, and thereby to challenge the melancholy tenor of our contemporary "wound culture." In a series of provocative readings, Sanyal presents Baudelaire's poetry as an aesthetic form that contests historical violence through rhetorical strategies of complicity, counterviolence, and critique. The book develops a new account of Baudelaire's significance as a modernist by dislodging him both from his traditional status as a practitioner of "art for art's sake" and from his more recent incarnation as the poet of trauma. Following her extended analysis of Baudelaire's poetry, Sanyal in later chapters considers a number of authors influenced by his strategies—including Rachilde, Virginie Despentes, Albert Camus, and Jean-Paul Sartre—to examine the relevance of their interventions for our current climate of trauma and terror. The result is a study that underscores how Baudelaire's legacy continues to energize literary engagements with the violence of modernity.
