Book excerpt: Introduction : à̀ une époque où l’alimentation végétarienne séduit de plus en plus de monde, on peut imaginer que les régimes végétariens prendront une place croissante dans l’exercice des médecins généralistes. Méthode :Une étude descriptive transversale par questionnaire internet a été réalisée à destination des médecins non thésés et internes en médecine générale exerçant en Île-de-France. Le questionnaire a été publié sur les réseaux sociaux. L’objectif de cette étude est d’évaluer les connaissances et les pratiques des internes en médecine générale en matière de régimes végétariens en Île-de-France. Résultats : entre le 13 novembre 2020 et le 21 janvier 2021, 175 réponses ont été recueillies. La médiane de l’âge des médecins est de 27 ans. 69,7% des médecins se déclarent favorables au régime végétarien contre 5,7% qui se déclarent plutôt défavorables et aucun ne se déclare très défavorable. Seulement 1,7% des médecins déclarent déconseiller un régime végétarien et 12,5% un régime végétalien à un adulte sain. Les médecins dissuadant leur patient de suivre un régime végétarien/végétalien ont tendance à plus fréquemment pratiquer un examen biologique. Chez les végétaliens, 53,1% des médecins dosent les folates et la carence en vitamine B12 n’est pas évoquée par 28% des médecins. 92,6% des médecins se déclarent insuffisamment informés sur ces régimes et 94,9% sont intéressés par des informations. Conclusion : les jeunes médecins de notre étude s’avèrent moins réfractaires et plus sensibilisés aux régimes végétariens/végétaliens que dans les études précédentes sur des médecins plus âgés, ce qui est un point positif pour l'évolution de la prise en charge de ces patients. Il existe un principe de précaution de la part des praticiens insuffisamment informés sur les risques réellement liés à ces régimes. Quelques messages fondamentaux (supplémentation en B12, dosage des folates inutile, absence de carence protéique...) pendant leur cursus pourraient demander un investissement minimal et avoir de réels bénéfices dans l'attente de recommandations officielles.

Objectif : Effectuer un état des lieux des pratiques des médecins généralistes de la région PACA concernant les conseils nutritionnels donnés aux patients végétariens te végétaliens avec comme hypothèse l'existence d'une grande variabilité de pratiques en lien très probable avec l'absence de recommandation précise sur le sujet. Matériel et méthode : une étude observationnelle quantitative descriptive a été réalisée auprès de 70 médecins généralistes et internes en médecine générale possédant leur licence de remplacement exerçant en région PACA sur une période de 6 mois. Le recueil des données a été fait via un questionnaire en ligne adressé par mail sur une période de 6 mois. Résultats : 78,6% des médecins interrogés ont rapporté ne pas interroger leurs patients sur leur régime alimentaire lors de leur interrogatoire initial. 71,4% des médecins interrogés se disaient moyennement à pas du tout à l'aise lorsqu'il s'agissait de donner des conseils nutritionnels aux patients végétariens, 80% d'entre eux concernant les patients végétaliens. 50% des praticiens prescrivaient un bilan sanguin annuel à leurs patients végétariens, 45,7% d'entre eux à leurs patients végétaliens. 84,3% des praticiens prescrivaient un bilan martial aux patients végétariens, 82,9% d'entre eux aux patients végétaliens. 71,4% des praticiens prescrivaient un dosage de la vitamine B12 aux patients végétariens, 72,9% d'entre eux aux patients végétaliens. 30% des praticiens prescrivaient un dosage de la vitamine D aux patients végétariens, 34,3% d'entre eux aux patients végétaliens. 12,9% des praticiens prescrivaient un dosage du zinc à leurs patients végétariens et végétaliens. 8,6% des praticiens prescrivaient un dosage du sélénium aux patients végétariens, 7,1% d'entre eux aux patients végétaliens. 94,3% des médecins interrogés ne prescrivaient pas de complément vitaminique à leurs patients végétariens, 84,3% d'entre eux n'en prescrivaient pas à leurs patients végétaliens. 10% des médecins prescrivaient un arrêt systématique de leur régime aux patients végétariens, 25,7% d'entre eux le faisaient chez les patients végétaliens. Conclusion : ce travail met en évidence des pratiques de prise en charge par les médecins généralistes des patients végétariens et végétaliens variables et pas toujours adaptées ainsi que la viabilité de ce type de régime dès lors qu'une attention est portée sur les apports en vitamine B12, calcium, vitamine D, fer, zinc et sélénium.

Notre objectif était de mettre en évidence l'existence de différences de besoins et de demandes de prise en charge nutritionnelle chez les sujets suivant un régime alimentaire flexitarien, végétarien ou végétalien, afin de permettre aux médecins généralistes d'adapter leur suivi en fonction du régime alimentaire des patients et de leurs attentes. Résultats : Parmi les résultats principaux nous pouvons noter que les raisons du régime alimentaire dépendent du régime, de l'âge, du sexe et de la profession. Les sujets se disent principalement informés sur la nutrition. Les sujets les plus informés sont les végétaliens, puis les végétariens et enfin les flexitariens. Le nombre de demandes des sujets à leur médecin généraliste dépend de leur âge, de leur sexe, de leurs connaissances sur la nutrition et sur leur alimentation, et ne dépend pas de leur régime alimentaire. Quel que soit leur régime, une majorité des sujets flexitariens, végétariens ou végétaliens souhaiterait consulter un professionnel de santé formé à l'alimentation spécifiquement flexitarienne, végétarienne ou végétalienne. Concernant les médecins généralistes, thésés ou non, ils sont en moyenne 800/0 à être intéressés par la nutrition, 800/0 à être intéressés par une formation sur la nutrition, et 200/0 à être déjà formés en nutrition. Un quart des médecins disent être à l'aise pour conseiller et suivre des patients suivant au moins un de ces régimes. Parmi ceux qui ne sont pas à l'aise pour le faire, 91 0/0 ne sont pas formés en nutrition. Le pourcentage de médecins étant à l'aise pour conseiller et suivre les patients diminue graduellement en fonction de la spécificité du régime : 52,50/0 pour les flexitariens, 45 0/0 pour les végétariens, 27,50/0 pour les végétaliens. La moitié des médecins connaissent le régime alimentaire de leurs patients. Conclusion : Il existe une inadéquation entre les demandes des patients et le suivi proposé par les médecins, notamment concernant la fréquence du bilan biologique et la prescription de compléments alimentaires. Ceci est le reflet de la formation et des connaissances théoriques de la plupart des médecins généralistes, qui sont limitées sur ce sujet particulier. Des supports d'information spécifiques et la publication de recommandations nutritionnelles françaises pour ces régimes alimentaires permettent d'aider les patients et leur médecin généraliste pour préserver la santé des patients, quel que soit leur choix de régime alimentaire.
