Book excerpt: INTRODUCTION : Le nombre de patients âgés vivant à domicile avec des problématiques médico-psycho-sociales complexes et pris en charge en hospitalisation à domicile (HAD) est actuellement en forte augmentation en France. Mais nous avons peu d'informations sur leurs caractéristiques sociodémographiques et cliniques et les outils d'évaluation sont non validés.L'objectif de cette étude a été de décrire les personnes âgées suivies en HAD et de définir la proportion de personnes âgées en situation complexe.METHODE : Il s'agit d'une étude transversale descriptive menée du 2 juin au 4 aout 2016.Cinquante personnes âgées vivant à domicile (hors EHPAD) et suivi en HAD de l'APHP (Assistance Publique - Hôpitaux de Paris) depuis au moins 8 jours ont été évaluées. Le recueil des données a été multidimensionnel à partir de l'outil RAI-CA (Resident Assessment Instrument - Contact Assessment). Deux groupes d'âges étaient ensuite comparés (75-83 ans versus ≥ 84 ans), utilisant le test exact de Fisher.RESULTATS OBTENUS : L'âge moyen de l'échantillon était de 84 ans avec 48% de femmes. 26% vivaient seul. 96% avaient au moins un aidant, qui présentait des difficultés dans 33,3% des cas. L'indice de karnofsky moyen était 41,6/100 avec des difficultés cognitives dans 68%. 70,8% se déclaraient douloureux et 40% avaient des antalgiques de paliers 3. 42% avaient chuté dans les 3 derniers mois. 70% considéraient leur état de santé comme passable ou mauvais. 80% mobilisaient au moins 4 professionnels.74% des patients présentaient un niveau de complexité médico-psychosocial maximal ou fort. Les >84 ans étaient globalement plus dépendants pour les actes de la vie quotidienne et les activités instrumentales que ceux de 75 à 83 ans. Les plus âgés mobilisaient aussi davantage de professionnels (ergothérapeute, p=0,044 ; aide-ménagère, p=0,025), et ils chutaient moins (p=0,044). CONCLUSION : Les personnes âgées en HAD présentaient des situations médico-psycho-sociales complexes mobilisant de nombreux professionnels à domicile.L'outil interRAI garantit la qualité et le partage des données. Il permet de repérer cette population de patients à risque, de les orienter, et de renforcer les services pour répondre à leurs besoins.
