Le médecin généraliste (MG) joue un rôle central dans l'après-cancer du sein. L'objectif principal de ce travail a été d'évaluer la coordination entre le centre Oscar Lambret (COL) et les MG. Méthode : De juin 2020 à décembre 2020, nous avons réalisé une enquête de pratiques professionnelles sous forme d'un questionnaire auprès de 303 médecins généralistes exerçant dans le Nord-Pas-de-Calais, en charge au minimum d'un patient suivi dans le programme du parcours de l'après-cancer du sein au COL. Résultats : 100 MG ont répondu au questionnaire. Dans l'après-cancer du sein, plus de huit médecins sur dix se sentent impliqués et 73% pensent qu'ils devraient être davantage impliqués. Très peu (14%) estime n'avoir à participer à aucune décision. 77% des MG trouvent la communication avec le COL satisfaisante. Les principaux freins sont les lacunes en matière de coordination et de transmission des informations, le manque d'expérience et de formation. 84% des MG ne connaissent pas la mesure du Plan cancer consistant en la mise en place systématique d'un « Programme Personnalisé de l'Après-Cancer ». 95% estiment avoir un rôle à jouer dans l'après-cancer et 73% pensent que le programme aurait un bénéfice dans leur pratique et attendent qu'il améliore qualité de vie des patients et la coordination. 8 MG sur 10 sont ainsi prêts à prendre le relais dans la surveillance du cancer du sein. Conclusion : En proposant un suivi personnalisé, organisé et coordonné avec tous les acteurs, le programme de l'après-cancer du sein au COL intègre le MG à une place centrale. Il améliore également la communication avec le MG, par le biais de nombreux outils et crée ainsi un réel partenariat, centré sur le patient.

Matériel et Méthodes : De février 2020 à novembre 2020 nous avons envoyé un questionnaire d'autoévaluation à 391 patients inclus dans le programme de l'après-cancer du sein du COL. Dans un second temps, nous avons réalisé trois focus groupes avec des médecins généralistes des Hauts-de-France pour trouver des axes d'amélioration du suivi des patients. Résultats : 360 patients ont répondu au questionnaire. Il y a 75% d'alertes. La moyenne des alertes par patient est de 3. La douleur représente 65% des alertes, la fatigue 55%, la mauvaise tolérance aux traitements 45%, l'impact psychologique 30%. Les médecins généralistes interrogés proposent d'améliorer la coordination entre le COL et la ville à travers une collaboration efficace pour optimiser la prise en charge du patient. La mise en place d'ateliers de travail oncologue-médecin traitant, la personnalisation du questionnaire d'autoévaluation à la médecine générale, la création d'un journal de bord du patient et l'organisation de réunions d'échange entre pairs sont des pistes suggérées. Conclusion : Le rôle du médecin traitant dans le suivi au long cours du patient apparait primordial pour dépister et prendre en charge les répercussions. Notre travail a mis en évidence l'intérêt pour tous d'un partenariat efficace entre chaque professionnel de santé.
