Ce travail a pour objectif de décrire le profil des adolescents accueillis en hôpital de jour psychiatrique, leur inscription dans les soins et leur devenir médico-social à moyen terme, à partir d'une cohorte de 100 patients. Cette étude repose sur un travail rétrospectif, basé sur une relecture des dossiers, et catamnestique, à partir d'une enquête téléphonique basée sur un questionnaire semi-structuré. Les données rétrospectives font ressortir l'hétérogénéité de la population accueillie, tant au niveau de la symptomatologie présentée et des diagnostics posés, qu'au niveau de la trajectoire médicale et sociale antérieure. La prise en charge est marquée par une difficulté d'inscription aux soins pour 53% de la population. Le taux de perdus de vue s'élève à 11%. Les données catamnestiques dessinent 3 profils évolutifs d'un point de vue clinique: 29% évoluent défavorablement vers l'instabilité avec ou sans soins, dont 3 décédés ; 34% évoluent vers une stabilité qui reste tributaire des soins ; 26% évoluent favorablement vers une stabilité sans soin. L'analyse statistique des données à partir du critère de l'inscription aux soins fait apparaitre deux populations distinctes, avec deux comportements différents face aux soins. Les troubles de l'adolescence qui échappent aux prises en charges institutionnelles proposées constituent un groupe à risque. Leur prise en charge et leur orientation impliquent une vigilance importante, avec une pratique de réseau et une vision à long terme des difficultés.

Flavigny, située sur le Centre Hospitalier du Vinatier. Elle inclut 180 patients âgés de 12 à 18 ans, hospitalisés de janvier 1999 à décembre 2001. L'objectif principal est d'établir le profil des adolescents admis en hospitalisation, le déroulement de leur prise en charge ainsi que leur devenir à l'âge adulte, dix ans plus tard; l'objectif secondaire est de connaître à distance leur vécu des soins hospitaliers à l' adolescence. Deux types de données sont analysés : des données rétrospectives, obtenues par la lecture des dossiers médicaux, ainsi que des données catamnestiques, basées sur l'envoi d'un questionnaire. Les deux pathologies les plus représentées sont la schizophrénie et le trouble bipolaire. Le diagnostic de schizophrénie, établi généralement en fin d'adolescence, reste stable à l'âge adulte, confirmé pour chaque adolescent de notre cohorte ; le diagnostic de trouble bipolaire est en revanche plus difficile à établir à l'adolescence, en lien notamment avec l'évolution imprévisible de l'état dépressif par lequel il débute. Cette étude confirme l'existence de troubles de l'adolescence s'inscrivant dans un continuum entre l'enfance et l'âge adulte, risquant ainsi d'entraîner des répercussions multiples sur la vie future de l'adolescent, concernant l'insertion sociale et professionnelle notamment. Ces résultats ne tiennent cependant pas compte de la dimension subjective des patients c'est-à-dire de la valeur qu'ils attribuent à leurs conditions de vie; à ce titre, une étude englobant le concept de qualité de vie, serait intéressante.

Ce travail a permis d’identifier les caractéristiques du soin particulièrement associées à une amélioration psychosociale et de relever les possibilités d’évolution de patients perturbés, confirmant les effets bénéfiques d’une intervention médico-pédagogique. A la lumière de ces résultats, nous avons approfondi notre réflexion sur les spécificités de la prise en charge institutionnelle soin-étude et l’avons illustrée par la présentation d’observations cliniques reflétant la complexité de parcours individuels au cours de cette prise en charge.
