Méthode : nous avons réalisé une enquête qualitative par entretiens semi-directifs auprès de sept femmes et de dix professionnels de santé dans deux cantons ruraux du Morbihan. L'analyse était descriptive pour les récits des parturientes, et thèmatique pour ceux des soignants. Résultats : quels que soient leurs facteurs de vulnérabilité, toutes ces femmes témoignent de difficultés dont l'ampleur ne semble pas toujours mesurée par les professionnels. Il existe un manque de moyens (financiers, culturels et politiques) pour les soignants pour effectuer ce repérage. Les femmes ont pour de multiples raisons, des réserves de paroles les conduisant à ne pas exprimer leurs difficultés. Des problèmes de communication ont été relevés auprès des professionnels, avec les patients mais aussi avec leurs collègues. Les parcours coordonnés sont rares, les intervenants sont multiples et les suivis de grossesse sont alors complexifiés. Conclusion : les parcours de soins doivent être plus adaptés à chaque patiente, coordonnés entre les professionnels de santé, entre les différentes disciplines et entre les institutions et le monde libéral. Ainsi les vulnérabilités seraient mieux repérées et les parcours de soins seraient plus efficients, améliorant le vécu de grossesse des femmes enceintes.

Elles constituent un facteur de risque de survenue de complications au cours de la grossesse et de l'accouchement. L'objectif principal de l'étude était de décrire le parcours de soins des femmes rurales en situation de VSE pendant la grossesse. Etude de cohorte longitudinale, reposant sur une méthode quali-quantitative au sein d'une maternité de niveau I de la région Auvergne. La sélection des patientes était fonction de l'exposition VSE/ruralité, avec un suivi jusqu'à deux mois durant le post-partum. Le critère de jugement principal était le taux d'adéquation entre le suivi médical des parturientes et le parcours de soins tel que défini par la Haute Autorité de la Santé. La capacité de recrutement était estimée à 85 femmes pour une précision de la mesure de 0,071 (taux d'adéquation de 50% ; IC 95% [0.429 - 0.571] ; risque [alpha] = 5% ; taux de perdues de vue = 10% ; taux de VSE = 27%). Durant notre période d'étude, 25 femmes rurales précaires ont pu être recrutées. Elles étaient en moyenne âgées de 27 ans, multipares et vivaient seules. Seulement cinq d'entre elles ont réussi un parcours de soins adéquat à 100%. La prévalence de la VSE était de 25%. Le score EPICES moyen de notre échantillon était de 47,86. Les principales difficultés déclarées étaient en majorité liées à la garde des enfants, aux situations familiales individuelles handicapantes, aux transports et aux problèmes financiers. Les séances de préparation à la naissance et l'entretien du 4eme mois n'ont pas été réalisées. Le médecin généraliste n'était pas identifié comme un professionnel du suivi de la grossesse. Différentes stratégies ont pu être mises en oeuvre pour pallier les difficultés déclarées : covoiturage, personnes ressources, soins à domicile. L'étude PRUGNANCY a permis de comprendre les difficultés rencontrées et les stratégies mises en oeuvre pour les surmonter. Elle a pu indiquer comment dépister précocement les situations de VSE afin d'orienter les patientes vers un parcours de soin adéquat et réduire les inégalités de santé dues à une offre de soin fragile et à l'isolement géographique.

L'objectif de cette étude était d'explorer le vécu du suivi de grossesse des femmes en situation vulnérable en milieu rural en Auvergne et la place qu'elles accordent à leur médecin généraliste dans ce suivi. Etude qualitative exploratoire par entretiens individuels semi-dirigés avec analyse thématique de contenu. Pour être incluses les femmes devaient être majeures, parler français, avoir un score EPICES supérieur ou égal à 30, vivre en milieu rural en Auvergne, avoir accouché depuis un mois maximum au moment de l'entretien. 17 entretiens ont été réalisés. Les difficultés rencontrées par les femmes dépassaient leurs capacités d'adaptation, pourtant importantes. Les plus évoquées étaient l'éloignement géographique et le manque de personnalisation du suivi, l'insuffisance d'offre de soins spécialisés dans la grossesse à proximité, les peurs liées à la maternité. La place accordée au médecin généraliste dans le suivi était très restreinte. Le renoncement aux consultations de suivi était rare sauf pour la préparation à la naissance et à la parentalité dont la représentation était souvent limitée à la gestion de la douleur de l'accouchement par voie basse. Les femmes identifiaient rarement les difficultés rencontrées comme telles. Cette ambivalence complique le repérage de la vulnérabilité. Une communication accrue envers les femmes quelque soit leur situation socio-économique apparente est nécessaire. La coopération interprofessionnelle reste à développer. L'exploration des freins à la coopération entre médecins généralistes, sages-femmes et gynécologues pourrait aider à améliorer l'inter professionnalité.
