Book excerpt: Excerpt from Outline of Psychiatry in Clinical Lectures The importance, which the focal symptoms attain accord ing to this definition in our subject, makes it seem advis able to keep their nature fully in mind. Through Meynert we have learned that the voluntary muscles and sense organs are connected with the cortex of the cerebrum by conduction paths, which extend in physiological continuity through the brain, spinal cord and peripheral nervous system. The sum of these paths, in which the law of isolated conduction pre vails, he calls the projection system, and thus clearly and unmistakably expresses the fact of physiological continuity in contradistinction to anatomical continuity - a fact which by recent research is proven to be perfectly true. Corres ponding to the division of the cerebrum into two hemispheres, the projection system is also divided into two halves, as is known, with the arrangement that motion and sensation of each half of the body is associated with the opposite cere bral hemisphere.* The expression projection is evidently borrowed from optics; as here the course of the rays through a system of lenses is to be followed exactly, so, in spite of all interpolation of ganglion elements in the paths of the projection system, the physiological continuity and isolated conduction are universally preserved. The majority of the focal symptoms may be readily traced to local interruption or stimulation of the conduction paths in the projection system. No claim whatever is made for exhaustiveness, completeness or originality - it is merely intended to afford a helpful guide to my students, so that they can follow my lectures more easily and more satisfactorily. I have endeavored to describe in a manner as clearly as possible and in a way to be easily understood by the students the principal types of mental disorder, especially with a view of providing the young physician just entering on practice with a working knowledge of the subject. To these descriptions I have prefaced a short account of the nature of the human mind and the fundamental psychological processes for the purpose of providing a proper foundation for what follows and also to place the student in possession of certain general facts so that he will be better able to orient what he may observe. In arranging the work the first consideration has been the convenience of the student. Important words are italicized and these words occurring in the index can thus be readily found by reference to the page, while the student will find it easier to skim over the pages rapidly and pick out just what he wants. While current literature on insanity has been freely consulted, references and foot-notes have been avoided so as not to detract in any way from the continuity of the text, and the incorporation of extended case histories has also been avoided for the same reason. We have not, however, been able to discover the least indication of his excesses being connected with regularly recurring depression. The patient's moral incapacity from childhood contrasts very sharply with his intellectual talents. But it is well known from every-day life that morality and intellect are to a great extent independent of each other. These cases are therefore generally designated as moral imbecility or moral feeble-mindedness. Such men are born criminals by nature, and are only distinguished from ordinary criminals by the great extent of their moral incapacity, by their having wills completely unaffected by the restraining experiences of life, and by their being fundamentally incorrigible. There is, therefore, as a rule, no other course to be taken, for their own sake, and for the sake of those around them, than to isolate them as being unfit for society, and as far as possible to find them occupation.* A still more fantastic personality is presented by an actor, aged thirty, who was brought to us three weeks ago because he had swindled a prostitute of a diamond ring under an assumed name. He simply put it on his finger, and made an appointment with the girl for the next day at a fashionable hotel, where he was not to be found. On his arrest he declared that the hussy had given him the ring, and that he had sold it since. When psychology is divorced from psychiatry, and the study of psychiatry is prosecuted along the lines of advance in general medicine, our knowledge of mental diseases cannot fail to be added to. The matter contained in the following pages is based on work so conducted: psychology is omitted; pathology, in so far as the changes in the brain cells and nerve fibres are concerned, is practically ignored, because until we know what the causes of such changes are we are.
