Book excerpt: Suite à différent scandales sanitaires ayant touché le monde du médicament et leurs conséquences pour la santé des patients, une prise de conscience, au niveau mondial, des autorités de santé sur le besoin de faire évoluer et harmoniser les réglementations internationales et nationales a eu lieu. Cette prise de conscience, par les pays membre de l'ICH, a permis la création d'un ensemble de guidelines encadrant toutes les phases du cycle de vie du médicament, en mettant à disposition des outils et systèmes de gestion de la qualité permettant d'augmenter le niveau de qualité des productions pharmaceutiques. C'est notamment sur la base de ces guidelines que les pays membres de l'ICH ont créé des guides de bonnes pratiques de fabrications (BPF pour la France) pour encadrer et inspecter, sur la base d'un référentiel, les pratiques de production des médicaments. Il est donc impératif pour une industrie pharmaceutique d'être conforme à ces critères et de maitriser toutes les étapes de sa production pharmaceutique. L'un des principaux outil qualité permettant cette maitrise est la rédaction d'instructions de production. L'objectif de cette thèse sera de présenter quelles sont précisément les exigences requises par les BPF pour l'élaboration d'une instruction de production et également les outils qualités permettant de répondre à ces exigences. C'est sur la base de ces éléments que sera créé un mode d'emploi opérationnel permettant d'établir rapidement si une instruction de production est conforme aux BPF ou non. Cette thèse présentera enfin une démonstration de l'utilisation de ce mode d'emploi dans un cas pratique réel rencontré dans l'industrie pharmaceutique.

La deuxième partie présente les médicaments injectables et leurs procédés de fabrication, dont le procédé de fabrication par filtration stérilisante et répartition aseptique. La troisième partie est l'étude d'un cas pratique d'une contamination fongique détectée au cours de la répartition aseptique d'un médicament injectable.

Les institutions, à travers l'élaboration du Code de la Santé Publique et des Bonnes Pratiques de Distribution en gros de médicaments, posent les fondements d'un système de gestion de la qualité rigoureux. La bonne application de ces textes par les grossistes-répartiteurs et l'importance du rôle des pharmaciens-inspecteurs de la Santé Publique montrent la force de notre système de santé.De manière complémentaire, les avancées technologiques contribuent à sécuriser le circuit d'approvisionnement des médicaments, notamment par une traçabilité des produits de plus en plus précise. L'industrie pharmaceutique a donc à disposition les outils nécessaires pour lutter contre des menaces telles que la contrefaçon. Le manque d'harmonisation des pratiques à l'international et entre les entreprises demeure un obstacle à la mise en application définitive de ces outils.En parallèle de ces actions, certains grossistes-répartiteurs s'engagent dans des démarches de certification de leur système qualité pour instaurer une dynamique d'amélioration continue. C'est dans cette optique que la société OCP Répartition décide de se mettre en conformité avec le référentiel ISO 9001 : 2008. L'obtention du certificat lui permettra de prouver qu'elle fournit régulièrement un produit conforme aux exigences réglementaires et aux exigences du client. L'intérêt est donc de renforcer la chaîne d'approvisionnement des médicaments, d'augmenter la satisfaction des clients et accroître ses avantages concurrentiels.

This document is rooted in two policy documents, the IASC Reference Group s (2010) "Mental Health and Psychosocial Support in Humanitarian Emergencies: What Should Humanitarian Health Actors Know?" and the "Sphere Handbook's Standard on Mental Health" (Sphere Project, 2011). It is written primarily for public health actors. As the social determinants of mental health and psychosocial problems occur across sectors, half of the tools in the accompanying toolkit cover MHPSS assessment issues relevant to other sectors as well as the health sector.
