This policy brief shows that in 2007 one-fifth of all Californians ages 0-64 who lived in households where at least one family member was employed did not have access to job-based coverage. Among adults with no access to job-based coverage through their own or a spouseâ€TMs job, nearly two-thirds remained uninsured. In contrast, the majority of children with no access to health insurance through a parent obtained public health insurance, highlighting the importance of such programs. Low-income, Latino and small business employees were more likely to have no access to job-based insurance. Provisions enacted under national health care reform (the Patient Protection and Affordable Care Act of 2010) will aid some of these populations in accessing health insurance coverage.

Book excerpt: Almost 4.7 million nonelderly adults and children of the seven million Californians who were uninsured for all or part of 2009 will be eligible for insurance as a result of last year's health care reform legislation, according to new data from the 2009 California Health Interview Survey (CHIS 2009). Eligible Californians will obtain coverage either through Medi-Cal or through subsidies to purchase private health insurance in the new California Health Benefit Exchange (CHBE) starting in 2014. The CHBE will also be open to 1.2 million uninsured persons who do not qualify for subsidized premiums due to their income exceeding eligibility levels, but who will benefit from the new marketplace created through the Patient Protection and Affordable Care Act (PPACA). Just over one million uninsured persons do not qualify to participate in either the CHBE or in the Medi-Cal expansion due to their citizenship status. With seven million uninsured residents of California in 2009, the new insurance options made available by the PPACA could face challenges in enrolling these uninsured individuals.

However, a large portion of America's neediest will remain uninsured due to regional limitations or lack of information. This research examines access to healthcare in non-elderly adults and distinctions between the population, observing racial or ethnic group characteristics, health condition and insurance status. The analysis supported a relationship between access to healthcare services and health insurance coverage or lack thereof, but this did not seem to cause substantial delays in obtaining medical care. The analysis also supported a relationship in access to care among different racial groups, with a measurable difference when controlling for socioeconomic factors, federal poverty level (FPL) and educational attainment, highlighting a considerable disparity in access to care between White and non-White populations in California.

Over 41 million -- 17.7 percent of nonelderly Americans -- did not have private or public health insurance during any part of 1996. This is up from 40.3 million, or 17.4 percent, in 1995. However, further analysis reveals that children experienced a disproportionate increase in the probability of being uninsured. In 1996, 14.8 percent of children were uninsured, compared with 13.8 percent in 1995. Furthermore, while the number of nonelderly uninsured increased by 1.1 million between 1995 and 1996, children felt this increase the most, as the number of uninsured children increased by 800,000 and the number of uninsured adults increased by 300,000.

The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: Although the United States has the highest health care spending per capita of any industrialized country, there are over 48 million nonelderly Americans lacking health insurance coverage, which translates to more than 18% of the nonelderly being uninsured. Currently, governments around the world are pursuing strategies to incorporate well-being measures to advance public policy, yet there are few studies that focus on the effects of health insurance coverage on well-being. This study fills the gap by exploring the relationship between well-being, health insurance coverage, health care cost, and Medicaid factors in the United States. Data for this study come from the Centers for Disease Control and Prevention's Behavioral Risk Factor Surveillance System (BRFSS) 2010 survey. The BRFSS houses the world's largest ongoing telephone health survey system with over 350,000 adults interviewed each year. It is designed to measure behavioral risk factors for the adult populations to identify emerging health problems, establish and track health objectives, and develop and evaluate public health policies and programs. In addition to the BRFSS data set, this study also uses data on states' ranking of Medicaid programs from a 2007 report published by the Public Citizen Health Research Group. Controlling for confounding factors, results from my logistic regression analysis indicated that individuals who could not see a doctor due to cost were 40.7% less likely to be satisfied with life. Similarly, individuals without health insurance coverage were 9.4% less likely to be happy. Moreover, individuals residing in states with a 1-standard-deviation-higher percentage of Medicaid scope of services ranking and Medicaid eligibility ranking have lower odds of being satisfied with life by 6.1% and 2.5%, respectively. However, low-income, self-employed, and unemployed individuals residing in states with more generous Medicaid ranking were found to be happier. These findings add to the existing literature by suggesting that health insurance coverage, ability to see a doctor, and residency in states with more generous Medicaid ranking substantially impact individual well-being. These effects held across income categories and health status, which further emphasized their significant influence on happiness. These findings have major implications for where policymakers should focus their attention.
