The IARC Monographs Working Group relied on epidemiological studies to evaluate the carcinogenic hazard to humans exposed to the drugs digoxin (widely prescribed for the treatment of chronic heart failure), pioglitazone (used for the treatment of type 2 diabetes mellitus), and hydrochlorothiazide (used to treat hypertension). Other agents evaluated included the drugs primidone, sulfasalazine, pentosan polysulfate sodium, and triamterene, and five herbal products (or their components): Aloe vera whole leaf extract, goldenseal root powder, Ginkgo biloba leaf extract, kava extract, and pulegone. In view of the limited agent-specific information available from epidemiological studies, assessments of these agents relied mainly on carcinogenicity bioassays to reach conclusions as to the carcinogenic hazard to exposed humans.

U.S. forces have been deployed many times over the years to areas in which malaria is endemic, including in parts of Afghanistan and Iraq. Department of Defense (DoD) policy requires that antimalarial drugs be issued and regimens adhered to for deployments to malaria-endemic areas. Policies directing which should be used as first and as second-line agents have evolved over time based on new data regarding adverse events or precautions for specific underlying health conditions, areas of deployment, and other operational factors At the request of the Veterans Administration, Assessment of Long-Term Health Effects of Antimalarial Drugs When Used for Prophylaxis assesses the scientific evidence regarding the potential for long-term health effects resulting from the use of antimalarial drugs that were approved by FDA or used by U.S. service members for malaria prophylaxis, with a focus on mefloquine, tafenoquine, and other antimalarial drugs that have been used by DoD in the past 25 years. This report offers conclusions based on available evidence regarding associations of persistent or latent adverse events.

The out come of this was the small symposium at SchloE Rauischholzhausen near GieEen (Germany), and the reader of this small volume of articles on topics discussed will judge to what extent this aim was realized. Although F.e. Steward was unable to attend the meeting, we were happy that he contributed a review on some important turning points of the cell culture method in the past, and at the end of the book some participants of the meeting attempted some prognosis on possible future developments, summarized by Mike Fowler. It was the wish of our publisher to organize the contributions as a summary of the major topics of research of the individual groups represented at the meeting. Due to this, some readers will certainly feel that important areas of research in the cell culture field are missing. This is regrettable, but space had to be limited to keep this volume at a reasonable price.

This symposium was devoted to pharmacokinetics, drug metabolism, analytic procedures, blood level determinations, and their interpretation both for thera peutic and toxic situations. Considerable progress has been made during the last years in this area of digitalis research. The time was suitable for a critical reap praisal of facts and theories and for future planning. Our main intention was to relate analytic data and biochemical findings to clinical problems and questions. Despite the undoubtedly basic character of clinical pharmacology, it is neverthe less an applied science which should help to develop the rational basis of thera peutics. We are particularly grateful to the active participants who bore the burden of preparing presentations and - even worse - manuscripts. Book excerpt: The bicentenary of William Withering's now famous medical report entitled "An Account of the Foxglove and Some of its Medical Uses" has given us the occasion to analyze the state of the art. Cardiac glycosides in 1985 are considered tobe the basis for medical treatment of myocardial failure, together with diuretics and vasodilators in the more severe cases. Nevertheless, the controversy as to their exact place in the treatment of heart failure with sinus rhythm has never ceased. Although cardiac glycosides are of unquestionable value in tachycardia caused by atrial fibrillation or atrial flutter, the chronic use of these drugs in sinus rhythm is not generally accepted. The development of tolerance has been reported. It is of interest that Withering hirnself wrote: "-let it (foxglove) be continued until it either acts on the kidneys, the stomach, the pulse, or the bowels; Iet it be stopped upon the first appearance of any of these effects, and I will maintain that the patient will not suffer from its exhibition, nor the practitioner be disappointed in any reasonable expectation. " In West Germany alone, more than three million patients (about 5% of the total population) are constantly taking cardiac glycosides. These drugs are the most prescribed medicaments in many countries. Thus, we considered it worthwhile and necessary to review in a critical way today's knowledge about the foxglove; to report new findings and to evaluate old Statements.
