The post-implementation RN MD Collaboration SurveyMonkey℗ʼ revealed 70% of the nurses had a positive perception of strong communication and collaboration with the physicians. Each profession has an ethical duty to ensure safe quality care is provided to every patient every time. The importance of open communication between nurses and physicians is essential to the advancement of patient care quality and safety and the healthcare system at large. The patient's healthcare experience directly correlates when they perceive their nurse and physician collaborate on the plan of care. In healthcare, lack of communication can lead medical failures to patient errors, and even patient death. Communication failures account to 70% of 2455 annual sentinel events, and 76% of persons having a sentinel event and with subsequent death. Nurse-physician collaboration is defined as “an interpersonal process where physicians and nurses present with shared objects”. Both physicians and nurses need to have equity in decision-making, and in the responsibility of managing the patient in order to decrease patient mortality and improve quality of patient. In order to evoke effective communication between both parties, there needs to be elements of respect and mutual trust in the relationship. The purpose of this grant proposal is to describe research to explore and measure the differences in perception of collaboration amongst physician and nurses on the medical surgical, and intermediate care units of a southern California hospital. A healthcare association between the distinctive units regarding nurse-physician collaboration differences may suggest interventions to enhance collaboration. Enhanced collaboration may increase the nurse’s job satisfaction, decrease nurse burnout, facilitate retention of nurses and decrease hospital financial strain resulting from high staff turnover rates. Ultimately patient’s benefit from decreased medical errors resulting from miscommunications amongst physicians and nurses.

Book excerpt: Poor communication is identified as the root cause for the majority of sentinel events in hospitals, including wrong site surgery, medication errors, and failure to rescue. Interdisciplinary rounding (IDR), a long-standing practice in the Intensive Care Unit (ICU), provides a forum for communication and collaboration and has been linked to improved patient outcomes. Most of the research regarding IDR has been performed in the ICU setting within academic medical centers. IDR outside of the ICU has demonstrated similar clinical outcomes but a gap exists in the literature regarding the impact of IDR participation on the nurse, particularly for nurses working in the non-ICU setting within community hospitals. This led to the development of a research question. Basic Psychological Needs Theory was chosen as the theoretical framework -- to specifically assess how participation in IDR affected the nurses' sense of autonomy, competence, and relatedness. A mixed method study was conducted in a community hospital in Central Florida using surveys and semi-structured interviews. Results indicated ICU nurses perceived a higher level of collaboration with physicians than nurses working outside of the ICU but this did not correlate with satisfaction of the ICU nurses' basic psychological needs. Only the nurses' level of experience and advanced nursing education appeared to have any significant impact on satisfaction of the nurses' basic psychological needs. The interview responses confirmed the presence of different rounding processes and levels of collaboration outside of the ICU, which helped to explain and support study outcomes. Opportunities for process improvements were also identified.
