This review will summarise both current clinical management of MND and our present understanding of the molecular pathogenesis of MND. Study of C9ORF72-MND has the potential to rapidly advance both of these aspects in the coming years. In the first section, we will discuss the clinical features of MND and describe how patients with this devastating condition present, are investigated, and managed in the 21st century. Although, currently, management is limited by an incomplete understanding of disease pathophysiology, there is much which can be done to assist and support patients with MND. In the following sections, we will discuss molecular mechanisms implicated in MND, highlighting observations which unify different theories. The objectives of the Varese conference were the following: a. To provide a forum for the proponents of the various paths of research into ALS. b. To correlate the useful therapies employed regionally. for the purpose of developing a common guide for patients, families, and supporting professionals. c. To encourage self-examination by the health care professionals into the psychological barriers imposed by a diagnosis of terminal illness for which there is no known cause or cure. Not surprisingly, there were no announcements of "breakthroughs" or "miracle cures", which are nonetheless hoped for in the confrontation of a disease such as ALS. It is fair to say, however, that Varese provided the context for a thorough review of what is known about ALS and we hope that the papers will renew some of the enthusiasm which has characterized this conference. The book contains six sections: Basic Research Aspects, Diagnostic Tools, Clinical Management, Therapeutic Trials, Psychological Aspects and the Epidemiology of ALS. Certain aspects, such as pathological studies and animal models, have not been covered; these subjects were partially treated during the informal sessions. Nevertheless, the large number of papers bears evidence to the growing interest in ALS and to the success of the Varese meeting.

Book excerpt: Motor neuron disease (MND) is a progressive and fatal neuromuscular disease; the most common and severe form of MND presentation is amyotrophic lateral sclerosis (ALS), commonly known as Lou Gehrig,Äôs disease. The majority of ALS patients die within 2,Äì5¬†years of receiving a diagnosis. Familial ALS is a hereditary form of the disease and accounts for 5,Äì10% of cases, whereas the remaining cases have no clearly defined etiology. ALS affects persons of all ethnicities and races; currently, no curative treatment for ALS is available worldwide. ALS is also the major adult-onset MND and is clinically, pathologically, and genetically associated with fronto-temporal dementia in some cases, which is the second cause of dementia in elderly people. However, MND does not affect sphincter, sexual function, or eye movements. MND is the most common degenerative disorder affecting the upper and lower motor neurons at the same time. Most of the patients presenting MND in our series complained of muscle weakness, muscle wasting, fasciculation, and spasticity plus lower cranial nerve disturbances. According to our bibliographic studies, apart from nusinersen, it seems to be that riluzole and edaravone also improve motor neuron function by acting on SK channels.
