Book excerpt: The goal of this research program is to develop a sensitive diagnostic technique based on quantitative elasticity imaging permitting surrogate palpation of deep lying breast lesions. The remote measurement of elasticity in breast tissues may provide unique information which could increase detection and/or characterization of potentially malignant masses not accessible to manual palpation. The primary technical objective of this study is to refine and test an MRI method for the acquisition of high resolution 3- dimensional (3D) spatial displacement data through the imaged object for quantitative estimation of internal strain and elastic modulus. Proof-of-concept of the proposed 3D displacement-encoding, stimulated echo technique has been completed and published using two-dimensional test objects. A pneumatically-driven deformation device under acquisition sequence control has been designed, constructed, and demonstrated to produce highly reproducible deformations of the imaged object. The image acquisition sequence has been generalized to encode 3D displacements over a 3D volume using "classic" 3D and fast-spin- echo schemes. Volumetric datasets of 3D phantoms have been acquired and submitted for processing using newly developed 3D elasticity reconstruction algorithms. Comparisons between 2D and 3D elasticity reconstructions from simulated and experimental displacement data shows higher accuracy of the 3D elasticity reconstruction.

Since the dense tissue may obscure the lesion, breast density is always considered by the radiologist during the exam for detection and diagnosis of breast cancer. In this thesis we developed quantitative analysis methods based on image segmentation algorithms to measure breast density. The purpose is to develop different segmentation methods for breast MRI images and CT images, as well as to generate a display tool to assist radiologists' visual inspection of breast density. In the first part of this thesis, an automatic method to locate the chest wall muscle, based on intensity analysis and edge detection algorithm, is demonstrated. The identification of chest wall muscle is important to determine the posterior boundary of the breast. The developed method was incorporated into a template based breast segmentation method which was first proposed by Muqing Lin et al. in 2010. The developed method was applied to segment density on breast MR images, and demonstrated a substantial improvement in the segmentation quality compared to results obtained using the previous method. In the second part of the thesis, a method was developed to segment breast density on low-dose chest CT image based on Fuzzy-C-means (FCM) algorithm. It involves image enhancement and edge detection by analyzing gray level histograms and a sequence of morphological operations. Subjects who had received both breast MR and low-dose chest CT images were analyzed in this study. The breast density from the same woman based on MRI and CT images were segmented and compared to evaluate consistency; and the results were further compared to the radiologist's corrected segmentation as the gold standard. In the third part, a method to reconstruct the 3D breast model is proposed. It involves breast positional correction (alignment), 3D breast model reconstruction and movie generation. We used the 3D model to assess datasets that contain two breast MRI scans of the same women taken at different times. The difference in the breast density analyzed in the two different scans was evaluated by a radiologist based on the generated 3D models, as well as based on the original MRI slices and the aligned MRI slices after registration. The difference in the segmented quantitative volume was used as the gold standard to compare with radiologist's visual evaluation results by using three different methods, to find out which method can lead to the most accurate results, and which method is preferred by the radiologist. The accomplishment in this thesis will contribute to quantitative analysis of breast density based on images acquired using different imaging modalities, and it may also help evaluation of changes in breast density after interventions.
