Book excerpt: L'HbA1c est un paramètre fondamental permettant d'évaluer le contrôle glycémique et d'adapter la prise en charge thérapeutique des patients atteints d'un diabète de type 2 (DT2). L'objectif de cette enquête observationnelle était d'évaluer le niveau de connaissances du paramètre HbA1c auprès de patients DT2 hospitalisés au CHNO des Quinze-Vingts ou consultant leur médecin généraliste. Le critère de jugement principal comprenait la connaissance par le patient de la définition du paramètre HbA1c, de sa dernière valeur objectivée et de sa valeur cible. Une brochure d'information sur le paramètre HbA1c a été élaborée puis délivrée à tous les patients avec explications orales. Parmi les 88 patients inclus, seuls 16 (18%) répondaient au critère principal (33% connaissaient le terme, 31% leur dernière valeur et 28% leur valeur cible). Leur durée de DT2 était plus longue (17 vs 11 ans, p=0,05) et leur pratique de l'autosurveillance glycémique plus fréquente (88 vs 53%, p=0,02). Ils étaient plus souvent traités par insuline (63 vs 19%, p=0,001) et atteints de rétinopathie diabétique (50 vs 22%, p=0,03). En analyse multi-variée et après ajustement sur ces facteurs, seul restait associé à une meilleure acquisition des connaissances le fait d'être traité par insuline (OR : 5,0 [1,14-22,3], p=0,03). Les résultats de notre étude témoignent d'une connaissance insuffisante et trop tardive du paramètre HbA1c chez les patients DT2, après instauration de l'insulinothérapie. D'autres études sont nécessaires pour évaluer l'impact de l'éducation thérapeutique et de la connaissance du paramètre HbA1c sur l'atteinte des objectifs cibles d'HbA1c.

Book excerpt: Objectifs : Déterminer le niveau de connaissances du diabète chez les patients atteints de diabète de type 2 et identifier les liens entre les caractéristiques des patients et leurs niveaux de connaissance à Caen Méthodes : Nous avons mené une étude descriptive transversale à Caen et ses environs d'octobre 2022 à mars 2023. Un auto-questionnaire a été utilisé pour déterminer le niveau de connaissances du diabète. Il était composé de cinq parties : caractéristiques du patient, diabète en général, complications, suivi et niveau d'information. Nous avons utilisé le test de Kruskal-Wallis pour rechercher les caractéristiques affectant le niveau de connaissances. Un p inférieur à 0,05 était considéré comme significatif. Résultats : Au total, 141 questionnaires ont été collectés. Le taux moyen de bonnes réponses était de 61,4 %. Les problématiques les mieux connues étaient les facteurs favorisants, les complications ainsi que le suivi. Les questions sur l'HbA1c ont donné des réponses plus hétérogènes. 68,3% des patients souhaitaient être mieux informés sur le diabète. Des associations significatives ont été mises en évidence chez les patients ayant un âge plus jeune, une formation sur le diabète et un suivi par un diabétologue. Conclusion : Les connaissances des patients sur le diabète se sont avérées correctes dans notre étude malgré de grandes variabilités interindividuelles. Par conséquent, les professionnels de santé devraient accorder plus d'attention à l'éducation du diabète, en particulier en ce qui concerne l'hémoglobine glyquée. Le recours aux plateformes d'éducation thérapeutique doit être encouragé, d'autant plus que le désir d'informations est exprimé par la plupart des patients.

Book excerpt: Tuberculosis (TB) strains with drug resistance (DR-TB) are more difficult to treat than drug-susceptible ones, and threaten global progress towards the targets set by the End TB Strategy of the World Health Organization (WHO). There is thus a critical need for evidence-based policy recommendations on the treatment and care of patients with DR-TB, based on the most recent and comprehensive evidence available. In this regard, the WHO consolidated guidelines on drug-resistant tuberculosis treatment fulfil the mandate of WHO to inform health professionals in Member States on how to improve treatment and care for patients with DR-TB. Between 2011 and 2018, WHO has developed and issued evidence-based policy recommendations on the treatment and care of patients with DR-TB. These policy recommendations have been presented in several WHO documents and their associated annexes, including the WHO treatment guidelines for multidrug- and rifampicin-resistant tuberculosis, 2018 update, issued by WHO in December 2018. The policy recommendations in each of these guidelines have been developed by WHO-convened Guideline Development Groups (GDGs), using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach to summarize the evidence, and formulate policy recommendations and accompanying remarks. The present Consolidated guidelines include a comprehensive set of WHO recommendations for the treatment and care of DR-TB, derived from these WHO guidelines documents. The consolidated guidelines include policy recommendations on treatment regimens for isoniazid-resistant TB (Hr-TB) and MDR/RR-TB, including longer and shorter regimens, culture monitoring of patients on treatment, the timing of antiretroviral therapy (ART) in MDR/RR-TB patients infected with the human immunodeficiency virus (HIV), use of surgery for patients receiving MDR-TB treatment, and optimal models of patient support and care.
