Book excerpt: It was not too long ago that many physicians and biomedical scientists felt that the era of 'vaccines' for protecting mankind against infectious disease was coming to an end. During the 1 940s and 50s the widespread use of newly developed antibiotics and antimicrobial chemotherapeutic agents suggested a new era in medicine, i. e. the control and eventual elimination of all infectious diseases, at least those caused by bacteria, by' chemical means. The magic 'bullet' proposed by Paul Ehrlich in the early 1900s seemed to be the method of choice for controling infection. However, it is now quite evident that those high expectations were unwarranted. Although many acute infections, especially those caused by pyogenic cocci, have been controlled by antibiotics, it is quite evident that infectious diseases, even those caused by bacteria, still are a major problem. Thus, the old 'standby' of preventative vaccination is making a strong comeback, not only for viral but also for bacterial infections. However, except for a relatively small number of viral diseases and those bacterial diseases due to toxin elaborated by microorganisms rather than invasion and replication of the microbe per se, preventative vaccination still has not fulfilled the expectations of their proponents. There has been a recent resurgence of interest concerning all aspects of vaccines, not only their preparation and administration, but also the nature and mechanism of the host immune response to the constituent micro organisms and their products.

These disparities result, in part, from fragmented publicâ€"private financing in which a large number of children and adults face limited access to immunization services. Access for adults lags well behind that of children, and rates of immunizations for those who are especially vulnerable because of chronic health conditions such as diabetes or heart and lung disease, remain low. Financing Vaccines in the 21st Century: Assuring Access and Availability addresses these challenges by proposing new strategies for assuring access to vaccines and sustaining the supply of current and future vaccines. The book recommends changes to the Advisory Committee on Immunization Practices (ACIP)-the entity that currently recommends vaccines-and calls for a series of public meetings, a post-implementation evaluation study, and development of a research agenda to facilitate implementation of the plan.

However, low profitability, expense and liability have hindered research and development of vaccines. Recently, increasing realization (enhanced by the AIDS pandemic) of the need to overcome such difficulties has led to steps being taken by national authorities, non-profit and commercial organizations to resolve them. This has been facilitated by developments in recombinant DNA techniques, the advent of monoclonal anti bodies and progress in the understanding of the immunological structure of proteins which have laid the foundation of a new generation of vaccines. Such vaccines are defined at the molecular level, can elicit immune responses controlling infectious organisms and are therefore potentially free of the problems encountered in conventional ones. Unfortunately, subunit and synthetic peptide vaccines are often only weakly or non inmunogenic. However, developments in both antigen production and immuno potentiation of weak antigens have opened new avenues with exciting prospects for vaccine design.
