Book excerpt: Preface This book describes problems and results of research in the gap between two fields: Human genetics, and clinical neurophysiology. Whenever I talked about my research on the genetics of the EEG, the answer of human geneticists was: "Very interesting, but I do not understand anything about the EEG. " On the other hand, EEG specialists usually remark: "Very interesting, but I do not understand anything about human genetics. " This is why I wrote this book. It tries to summarize results my own and from some others - and to point to problems. In the from researc- light of the recent progress especially in human molecular genetics, this field of research promises deep insights into biological mechanisms of brain function, as well as genetic variation involved in mental performance, and personality of humans. However, the logistic problems of such studies are not easy to overcome: It is necessary to study carefully ascertained population samples either of "normal" persons, or of persons selected for phenotypic characteristics that are not easy to diagnose. Moreover, EEG diagnosis and classification must be very specific, and is not trivial at all. All these problems require careful preparations at various levels, long-lasting efforts, and patience. Of this I am sure, however: The results would justify the efforts. I am too old to plan such a program myself; moreover, as an emeritus professor, I do not have the means for such studies.

Book excerpt: Although a number of studies have addressed epilepsy from a va riety of qualitative and quantitative factors, relatively little systematic or multidisciplinary work has been reported to date. The general purpose of the present study was to analyze specific kinds of data from a large series of epileptic patients to focus the significance of the findings, particularly in relation to previously published results. Correlations among the following parameters are presented: age, sex, age at onset of seizure, type of seizure, paroxysmal electroencephalogram (EEG) abnormalities, basic EEG rhythm, family predisposition to epilepsy, and the presence of exogenous factors. The basic material of the present study consists of records of approximately 7400 patients treated at the outpatient clinic of the Department of Neurology, University of Heidelberg, in whom the clinical diagnosis suggested some type of epilepsy. Initially, the first consecutive 500 patients in an alphabetically arranged file were chosen for inclusion within the study if their clinical diagnosis was supplemented by at least one EEG examination. The final study group consisted of 466 patients, since 34 patients had to be excluded because of the lack of sufficient clinical information concerning them.

Book excerpt: In order to supplement Dr. Johnson's review we wish to make two points: I) not all grossly similar lesions should be considered t. o be due to the same etiology, even if experimentally documented, and 2) relatively few infections (syphilis, toxoplasmosis, cytomegalovirus, herpesvirus and rubella) and no immunologic disorders (except for kernicterus due to hyperbilirubinemia) are known to affect the developing human nervous system. Admittedly, since most human malformations are of unproven cause, these two points are not mutually exclusive. Rather, the challenge remains to show that any of the experimental models relates to any of the human diseases. In particular, the porencephaly-hydranencephaly spectrum could be due to ischemia in the watershed distribution of one or more branches of the internal carotid ar teries; and aqueductal stenosis could be due to secondary blockage by blood clots or debris related to hemorrhage or necrosis in the germinal matrix or periventricular white matter, lesions most likely due to hypoxia-acidosis in premature infant, who die most often of the concomitant respiratory distress syndrome. At the end of three days of prolonged meetings, where each paper was given and extensively discussed, one feels that a number of referral points have emerged. On the one side the plas ticity of behaviour, on the other is reciprocity (between mother and child, father and child and perhaps we should add between mother and father). The third point, which perhaps has been only partially covered, concerns the relationship between these two variables, i. e. in terms of treatment of a child, the potential plasticity of his behaviour can be used to his great advantage if it is related to the historical common needs of the reciprocal relationship, for example, between the child and his parents.
