It has been determined that the dam would be overtopped for all storms exceeding approximately 32 percent of the Probable Maximum Flood (PMF). The spillway is, therefore, adjudged as 'seriously inadequate' and the dam is assessed as unsafe, non-emergency. It is recommended that, within 3 months of notification of the owner, detailed hydrologic and hydraulic investigations of the structure should be undertaken to more accurately determine the site-specific characteristics of the watershed and their effects upon the overtopping potential of the dam. The results of this investigation and analyses will determine the appropriate remedial measures which will be required. In the interim, a detailed emergency action plan must be developed and implemented during periods of unusually heavy precipitation. Also, around-the-clock surveillance must be provided during these periods.

The spillway is, therefore, adjudged as 'seriously inadequate' and the dam is assessed as unsafe, non-emergency. The classification of 'unsafe' applied to a dam because of a 'seriously inadequate' spillway is not meant to connote the same degree of emergency as would be associated with an 'unsafe' classification applied for a structural deficiency. It does not mean that there appears to be a serious deficiency in spillway capacity; and if a severe storm were to occur, overtopping and failure of the dam could take place significantly increasing the hazard to loss of life downstream of the dam.

However, the dam has some problem areas which require additional studies and remedial work. The most serious of these deficiencies are the two areas of seepage on the downstream slope of the dam. First is at the end of a grouted stone spillway channel and the second is at the toe in the center of the dam. It is recommended that within six months from notification, an engineering investigation is initiated to determine the sources of the seepage found. Using the Corps of Engineers' screening criteria for the initial review of spillway adequacy, it has been determined that the embankment would be overtopped by all storms in excess of 51% of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged as 'inadequate.' (Author).

Book excerpt: The Beaver Dam Lake, formerly known as Salisbury Mills Dam, consists of left and right non-overflow sections with an included spillway. The dam is about 335 feet long with a maximum height of about 35 feet. The width of the wall at its top, which is also the crest of dam, varies from 3.5 feet to 3.75 feet. The principal spillway is an ungated masonry and concrete gravity structure which is divided into two sections by an 8-foot wide stepped brick pier. The left and right spillway sections are about 30 feet and 35 feet long, respectively. Examination of available documents and a visual inspection of the dam and the appurtenant structures did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further investigation and remedial action. Using Corps of Engineers screening criteria, it has been determined that the dam would be overtopped for all storms exceeding approximately 37 percent of Probable Maximum Flood (PMF).

Information and analysis are based on visual inspection of the dam by the performing organization. Mine Lake Dam is classified as a small dam with a significant hazard potential. The dam is constructed of masonry and is 13.9 feet high and 516 feet in length. Over 300 feet of the dam's length is less that 8 feet in height. The dam's reservoir is only 25 acres. Popolopen Dam is located one mile above Mine Lake, while Stilwell Dam and reservoir is located a few hundred feet below Mine Lake Dam. The drainage area of the dam is 8.7 square miles. The structure forms an impoundment which is used for recreational purposes on the West Point Military Reservation. Hydrology computations prepared according to the Corps of Engineers' screening criteria establishes the spillway capacity of 1,492 cfs at 9.9 percent of the PMF with the PMF discharge at 15,070 cfs and the 1/2 PMF discharge at 7,170 cfs. Since the spillway cannot pass the 1/2 PMF discharge without overtopping the dam, the spillway is inadequate. The stability analysis determined three loading cases where the dam was unstable. Each of these cases involved uplift on the base of the dam due to hydrostatic forces in combination with the PMF, seismic loadings or ice loadings. Only the ice load occurs in a normal operating situation. The condition of uplift is plausible since seepage was observed at the juncture of the masonry spillway with the bedrock.

The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.
